ABSTRACT
Introduction: Plantar fasciitis is one of the most common causes of pain in the heel region of the foot and significantly affects walking, functional ability and quality of life. In some patients, symptoms persist despite conservative treatment and progress to a chronic form, for which the term plantar fasciopathy is also frequently used in the literature. In such cases, extracorporeal shockwave therapy is used as one of the non-invasive treatment options. Purpose: The purpose of this thesis was to review the literature on the efficacy of extracorporeal shockwave therapy in patients with chronic plantar fasciitis compared with placebo or sham therapy. Methods: The literature was selected from the PubMed, Cochrane Library and PEDro databases. The review included randomized controlled trials in English that compared extracorporeal shockwave therapy with placebo or sham therapy in adults with chronic plantar fasciitis. The included studies were compared in terms of participant characteristics, therapeutic protocols, assessment tools and findings. Results: Six studies were included in the review. In most included studies, extracorporeal shockwave therapy was associated with greater pain reduction than placebo or sham therapy, whereas one study did not confirm such an advantage. Individual studies also reported improvements in walking ability, foot function, quality of life and other secondary outcomes; however, they differed in terms of the measurement tools and therapeutic protocols used, making a direct comparison of results difficult. Discussion and conclusion: Extracorporeal shockwave therapy may contribute to a greater reduction in pain than placebo in some patients with chronic plantar fasciitis, but its efficacy was not consistently confirmed across all studies. Due to methodological differences between studies and the heterogeneity of treatment protocols, the optimal treatment protocol cannot be reliably determined and firm conclusions about long-term efficacy cannot be drawn. Nevertheless, extracorporeal shockwave therapy is used as one of the possible treatment options for plantar fasciitis in patients in whom conservative measures have not been successful.
