Introduction: Echocardiography is a non-invasive imaging technique used to assess the anatomy and function of the heart. Cardiac sonographers play an important role in obtaining high-quality echocardiographic images, correctly setting ultrasound parameters and performing post-examination measurements. Purpose: The purpose of the study was to evaluate the quality of echocardiographic images and Doppler measurements performed by master students in the cardiac ultrasound module. We compared echocardiographic images at the beginning and end of the 10-week clinical placement to assess students’ progress in performing transthoracic echocardiography (TTE). Methods: A descriptive method and a retrospective study conducted at the Department of Non-Invasive Cardiac Imaging Unit, University Medical Centre Ljubljana were used. We analyzed 30 echocardiographic examinations from the beginning and 30 from the end of the clinical placement. The echocardiographic images were assessed by two cardiology specialists and one cardiology resident. They evaluated the quality of the three standard views (PLAX, A4C and A2C) as well as the pulsed (PW) and continuous wave (CW) Doppler signals across the aortic valve. Results: All five evaluated features in the three standard views – PLAX, A4C, and A2C – were better in the end. The greatest improvement was for the A2C view in the standard view (0.55 or 14%), visualization of major anatomic structures (0.35 or 9%), and overall image quality score (0.37 or 9%). No statistically significant differences were found for the PLAX and A4C views (p > 0.05), while statistically significant differences were found for the A2C view in the standard view (p = 0.003) with better results at the end of the clinical placement. The mean values for the quality of PW and CW Doppler signals were also higher at the end of clinical placement. No statistically significant differences were found for CW Doppler signals. For PW Doppler signals, statistically significant differences were found in the baseline position (p = 0.018) and in the overall image quality score (p = 0.043), again in favor of the end of placement. Discussion and conclusion: The study showed significant improvement in students’ TTE performance during the 10-week clinical placement. There was a statistically significant increase in the number of correct images. The greatest improvements were seen in the A2C view and PW Doppler measurements across the aortic valve.
