Analiza telesne dejavnosti in parametrov kakovosti življenja sladkornih bolnikov tipa 2 v času epidemije COVID-19 : magistrsko delo

V magistrskem delu smo raziskali stopnjo telesne dejavnosti sladkornih bolnikov tipa 2 v času epidemije COVID-19 in ugotavljali morebitne povezave med telesno dejavnostjo in parametri kakovosti življenja ter zdravljenja pri teh bolnikih. Telesno dejavnost smo raziskovali s pomočjo GPAQ vprašalnika. Glede na priporočila Svetovne zdravstvene organizacije (WHO) smo ločili tiste, ki dosegajo priporočila (≥600 MET) in tiste, ki ne dosegajo priporočil (<600 MET). Nadaljnje smo jih delili še na visoko telesno dejavne, zmerno telesno dejavne in nizko telesno dejavne. Na kliničnem oddelku za endokrinologijo, diabetes in bolezni presnove, v UKC Ljubljana, smo pred rednim ambulantnim pregledom, bolnike seznanili z možnostjo sodelovanja pri omenjeni raziskavi. Končni vzorec je predstavljalo 54 posameznikov z diagnozo sladkorne bolezni tipa 2, od tega je večinski vzorec predstavljal moški spol (n=41), vključenih ženskega spola je bilo 13. Povprečna starost preizkušancev moškega spola je znašala 57,3 ± 8,3 let, od diagnoze sladkorne bolezni je v povprečju minilo 12,8 ± 9,2 let, telesna masa je znašala 97,1 ± 17,5 kg, telesna višina pa 177,1 ± 7,7 cm. Pri preizkušankah (ženski spol) je bila povprečna starost 54,4 ± 9,8 let, trajanje sladkorne bolezni 9,2 ± 7,9 let, izmerjena telesna masa je znašala 81,3 ± 15,7 kg, telesna višina pa 164,7 ± 6,4 cm. Vključeni v raziskavo so individualno, preko telefonskega pogovora odgovorili na vprašalnik o telesni dejavnosti (GPAQ), vprašalnik o pojavnosti hipoglikemij in v slovenščino preveden PROM in PREM vprašalnik Švedskega nacionalnega registra diabetesa (NDR). Klinične podatke (glikiran hemoglobin, krvni tlak, terapija, pridružene bolezni, ipd.) smo pridobili pred rednim ambulantnim pregledom. Rezultati raziskave so pokazali, da je večina (88,9 %) preizkušancev kljub omejitvam v času epidemije COVID-19 dosegala zadostno tedensko količino telesne dejavnosti, glede na priporočila za telesno dejavnost sladkornih bolnikov tipa 2. Skoraj polovica (48,1 %) preizkušancev je ocenila, da je ostala enako telesno dejavna v času epidemije. Vseeno jih je 31,5 % poročalo, da so bili manj telesno dejavni, 11,1 % pa, da so bili dosti manj telesno dejavni. Pogostejše hipoglikemije v času epidemije je v našem vzorcu navedlo 14,8 % preizkušancev, v 57,3 % pa so navedli, da še nikoli niso občutili simptomov hipoglikemije. Korelacij med telesno dejavnostjo preizkušancev in parametri kakovosti življenja in oskrbe (PROM in PREM) sladkorne bolezni nismo zaznali. Zaključki našega dela so pokazali, da so preizkušanci v času epidemije COVID-19 dosegli priporočene minute telesne dejavnosti na teden. Pojavnost hipoglikemij se ni bistveno povečala. Prav tako pa nismo ugotovili nobene povezanosti med telesno dejavnostjo in kakovostjo življenja ter boljšo oskrbo sladkorne bolezni.

Naslov:Analysis of physical activity and life quality parameters of type 2 diabetes during the COVID-19 epidemic

In the master's thesis, we researched the level of physical activity of type 2 diabetics during the COVID-19 epidemic and identified possible links between physical activity and quality of life and treatment parameters in these patients. Physical activity was surveyed using a GPAQ questionnaire. According to the recommendations of the World Health Organization (WHO), we distinguished between those who meet the recommendations (≥600 MET) and those who do not meet the recommendations (<600 MET). Further, we divided them into high physically active, moderate physically active, and low physically active. At the Clinical Department of Endocrinology, Diabetes and Metabolic Diseases, at the University Medical Center Ljubljana, we informed patients about the possibility of participating in the study before their regular outpatient examination. The final sample consisted of 54 individuals diagnosed with type 2 diabetes, of which the majority were male (n = 41) and 13 were female. The average age of male subjects was 57.3 ± 8.3 years, an average of 12.8 ± 9.2 years had passed since the diagnosis of diabetes, the body weight was 97.1 ± 17.5 kg, and the body height was 177. 1 ± 7.7 cm. In the test subjects (female), the average age was 54.4 ± 9.8 years, the duration of diabetes was 9.2 ± 7.9 years, the measured body weight was 81.3 ± 15.7 kg, and the body height 164.7 ± 6.4 cm. Over a telephone conversation, the participants in the research individually answered the Global Physical Activity Questionnaire (GPAQ), the Hypoglycemia Questionnaire, and the into Slovene translated PROM and PREM questionnaire of the Swedish National Diabetes Registry (NDR). Clinical data (glycated hemoglobin, blood pressure, therapy, associated diseases, etc.) were obtained before the regular outpatient examination. The results of the study showed that the majority (88.9%) of the subjects achieved a sufficient weekly amount of physical activity, despite the restrictions during the COVID-19 epidemic, according to the recommendations for physical activity of type 2 diabetics. Almost half (48.1%) of the test subjects estimated that they remained equally physically active during the epidemic. However, 31.5% reported being less physically active, and 11.1% reported being much less physically active. More frequent hypoglycemia during the epidemic was reported in our sample by 14.8% of subjects. 57.3% stated that they had never experienced symptoms of hypoglycemia. No correlations were found between the physical activity of the test subjects and the quality of life and care parameters (PROM and PREM) of diabetes. The conclusions of our work showed that the subjects achieved the recommended minutes of physical activity per week during the COVID-19 epidemic. The occurrence of hypoglycemia did not increase significantly. We also found no association between physical activity and the quality of life and better diabetes care.
