Abstract
Uvod: Pri samopoškodovanju gre za namerno poškodovanje lastnega telesa na različne načine, vendar brez namena samomora. Samopoškodovalno vedenje lahko predstavlja tudi beg pred različnimi čustvi žalosti in praznine, za reševanje stisk in notranje napetosti ter izražanje jeze. Namen: Namen diplomskega dela je predstaviti vrste samopoškodovanja ter dejavnike, ki vplivajo na razvoj tega vedenja. Prav tako želimo opozoriti na pomembne intervencije, ki jih izvaja medicinska sestra pri mladostniku s samopoškodovalnim vedenjem. Želimo izpostaviti njeno vlogo pri izdelavi varnostnega načrta in uporabnosti tega v klinični praksi. Metode dela: Diplomsko delo je napisano na podlagi deskriptivnega sistematičnega pregleda slovenske in tuje literature. Slovenski in tuji članki so bili pridobljeni preko podatkovnih baz PubMed/Medline, ScienceDirect, CINAHL in s Kooperativnim online bibliografskim sistemom in servisi (COBBIS. SI), oddaljenim dostopom Digitalne knjižnice Univerze v Ljubljani (DiKUL) in spletnim brskalnikom Google učenjak. Rezultati: Ker je samopoškodovanje zelo kompleksen pojav, od zaposlenih v zdravstveni negi zahteva visoko stopnjo znanja na strokovnem področju in izkušnje iz klinične prakse. Vzpostavitev zaupnega odnosa in zagotavljanje varnosti je ključna naloga pri delu z mladostniki. Za izboljšanje kakovosti in varnosti obravnave mladostnikov je bila na Enoti za adolescentno psihiatrijo v okviru Univerzitetne psihiatrične klinike Ljubljana v klinično prakso uvedena zloženka, poimenovana Samopoškodovanje in varnostni načrt. Glavni cilj tega je priprava načrta ravnanja ob nevarnosti za pojav samopoškodovanja, ki vodi mladostnika in zaposlene v zdravstveni negi v ustrezno ukrepanje. Razprava in zaključek: Ker gre pri samopoškodovanju za perečo javnozdravstveno težavo, je za čim bolj kakovostno in celostno obravnavo pomembno vključevanje zdravstvenih in drugih služb. Znanje s področja uporabe varnostnega načrta, terapevtske komunikacije, ki je temeljni gradnik medosebnega terapevtskega odnosa, je za zaposlene v zdravstveni negi ključnega pomena. Varnostni načrt je bil v slovenski prostor vpeljan v želji po varnejši in kakovostnejši zdravstveni obravnavi. Za boljše razumevanje tega pojava in zmanjševanje stigmatizacije je zelo pomembno izobraževanje in ozaveščanje vseh deležnikov, ki se srečujejo z mladostnikom s samopoškodovalnim vedenjem.

Title:Nurse as a key person in ensuring safety for adolescent with self-harm behaviour : diploma work
Abstract:
Introduction: Self-harm behaviour is the intentional damage of own body in various ways, but without the intention of committing actual suicide. Self-harm behaviour represents escape from many different feelings of sadness and emptiness, looking for ways to deal with inner stress and tensions. It can also be the accumulation of anger that adolescents cannot or do not wish to express nor share and they bottle it up. Purpose: The purpose of diploma work is to introduce different kinds of self-harm behaviour and sources that affect the development of such behaviour. We would also like to pay attention to all the important interventions that a nurse should be acquainted with when dealing with such patients and define the making of safety a plan and the use of it in practice. Methods: The diploma work is written based on descriptive and systematic check-up of Slovene and foreign literature. Slovene and foreign articles were acquired through following databases: PubMed/Medline, ScienceDirect, CINAHL and with the help of Cooperative online bibliographic system (COBBIS. SI), Digital Library of Ljubljana University (DiKUL) and online browser Google. Results: Since self-harm behaviour is a severely complex phenomenon, it requires a high degree of professional knowledge and experiences from clinical practice. In a desire to improve the quality and safety treatment of adolescents was at the Department for Adolescent Psychiatry at University Psychiatric Clinic Ljubljana introduced folder, named selfharm and safety plan. The main purpose of making such a plan is to be prepared in case of occurrence of self-harm, thus helping the adolescent and the health care professionals to act accordingly. Discussion and conclusion: Because self-harm behaviour is a serious public health issue, is very important that health and other services are involved for qualitative and entirely treatment. Knowledge of using the safety plan, therapeutic communication, which is the base element of an interpersonal therapeutic relationship, is for nursing health care professionals’ key importance. A safety plan was introduced in the Slovenian area in a desire to improve safety and quality health treatment. For a better understanding of self-harm and to reduce stigma, it is very important to educate and aware all who deals with adolescents with self-harm behaviour.
