Introduction: Breastfeeding is an important part of early motherhood and represents a complex occupation as well as a co-occupation of the mother and child. Its performance is influenced by numerous physical, psychological, social, and environmental factors. Through a holistic approach, occupational therapy can support the mother and child in overcoming barriers and integrating breastfeeding into everyday life. Purpose: We aimed to explore the role of occupational therapy during the breastfeeding period, identify the barriers mothers face while breastfeeding, and determine which occupational therapy interventions are used to support breastfeeding. Methods: A descriptive research method with a systematic literature review was used. The literature search was conducted in February and March 2026 in the Web of Science, CINAHL, and PubMed databases, using the following search string: (»occupational therap*« OR occupation* OR »co-occupation«) AND (breastfeed* OR lactati*). The search was limited to publications in Slovenian and English, published from 2013 onwards. The eligibility of the publications was assessed against predefined inclusion and exclusion criteria, and their methodological quality was appraised with the JBI and MMAT tools. Results: Eight publications were included in the final analysis. Three main themes were identified: the role of occupational therapy during the breastfeeding period, barriers and challenges in breastfeeding; and occupational therapy interventions to support breastfeeding. Discussion and conclusion: The findings show that occupational therapy plays an important role in supporting mothers and infants during the breastfeeding period. Occupational therapy interventions focus on maternal education and empowerment, ergonomics and environmental adaptations, establishing daily routines, supporting maternal well-being, and social support. From an occupational therapy perspective, breastfeeding is an important occupation and co-occupation of the mother and infant, and its performance is influenced by numerous interrelated factors. The literature review supports the relevance of occupational therapy in identifying barriers and providing holistic, client-centred care during the breastfeeding period. Despite the recognised role of occupational therapy, further research is needed to better understand its contribution and build stronger evidence.
