Abstract
Uvod: Fournierjeva gangrena je redka, hitro napredujoča nekrotizirajoča okužba mehkih tkiv, ki prizadene genitalno, perinealno in perianalno področje. Zaradi hitrega poteka bolezni, tveganja za sepso, večorganske odpovedi ter visoke stopnje umrljivosti zahteva takojšnje kirurško, antibiotično in podporno zdravljenje. Pooperativna zdravstvena nega ima pomembno vlogo pri spremljanju stanja pacienta, zgodnjem prepoznavanju zapletov in oskrbi obsežnih ran. Namen: Namen diplomskega dela je bil s pregledom strokovne in znanstvene literature raziskati pooperativno zdravstveno nego pacienta s Fournierjevo gangreno ter opredeliti načine oskrbe rane s poudarkom na sodobnih pristopih in vlogi zdravstvene nege. Metode dela: Uporabljena je bila deskriptivna metoda dela s pregledom strokovne in znanstvene literature. Iskanje je potekalo v podatkovnih bazah COBISS in Medline, s pomočjo iskalnikov Pubmed in Google scholar ter z analizo referenc v že identificiranih virih. V pregled so bili vključeni viri, objavljeni med letoma 2016 in 2026, izjemoma tudi starejši vsebinsko relevantni viri. V končno analizo je bilo vključenih 12 člankov. Rezultati: Rezultati pregleda literature so pokazali, da je pooperativna zdravstvena nega pacienta s Fournierjevo gangreno usmerjena v kontinuirano spremljanje vitalnih funkcij, bolečine, diureze, laboratorijskih izvidov, glikemije, znakov sepse in odziva na zdravljenje. Pomembni so tudi aplikacija antibiotične terapije, uravnavanje tekočinskega in elektrolitskega ravnovesja, nadzor pridruženih bolezni, preprečevanje kontaminacije rane ter zagotavljanje psihološke podpore pacientu. Pri oskrbi rane je temeljni ukrep zgodnji in obsežni kirurški debridement, po katerem rane pogosto ostanejo odprte in zahtevajo redno spremljanje ter prevezovanje. Kot sodobnejši pristop se uporablja terapija z negativnim tlakom, ki lahko zmanjša pogostost prevezovanja, bolečino in količino izločka ter spodbuja nastanek granulacijskega tkiva. Razprava in zaključek: Pooperativna obravnava pacienta s Fournierjevo gangreno je kompleksna, dolgotrajna in multidisciplinarna. Zdravstvena nega ima pomembno vlogo pri zgodnjem prepoznavanju poslabšanja, preprečevanju zapletov, oskrbi rane in podpori pacientu med zdravljenjem. Ugotovitve kažejo, da terapija z negativnim tlakom predstavlja koristen dodatek k oskrbi rane, vendar ni primerna za vse paciente in ne nadomešča kirurškega debridementa. Zaradi omejenega števila raziskav, ki neposredno obravnavajo zdravstveno nego pacientov s Fournierjevo gangreno, bi bile potrebne dodatne raziskave s poudarkom na vlogi medicinskih sester pri pooperativni obravnavi.

Title:Postoperative care of a patient with Fournier’s gangrene : diploma work
Abstract:
Introduction: Fournier’s gangrene is a rare, rapidly progressive necrotizing soft tissue infection affecting the genital, perineal, and perianal regions. Due to its rapid progression and the risk of sepsis, multiple organ failure, and high mortality, it requires immediate surgical, antibiotic, and supportive treatment. Postoperative nursing care plays an important role in patient monitoring, early recognition of complications, and management of extensive wounds. Purpose: The purpose of this diploma work was to review professional and scientific literature on postoperative nursing care of patients with Fournier's gangrene and to define wound care approaches, with an emphasis on modern methods and the role of nursing care. Methods: A descriptive research method was used, based on a review of professional and scientific literature. Literature was searched in the COBISS, and Medline databases, using PubMed and Google Scholar, as well as by reviewing references from already identified sources. The review primarily included sources published between 2016 and 2026, while other sources were included exceptionally when they were relevant to the topic. A total of 12 articles were included in the final analysis. Results: The literature review showed that postoperative nursing care of a patient with Fournier's gangrene is mainly focused on continuous monitoring of vital signs, pain, urine output, laboratory results, blood glucose levels, signs of sepsis, and response to treatment. Important aspects also include the administration of antibiotic therapy, management of fluid and electrolyte balance, control of comorbidities, prevention of wound contamination, and psychological support. In wound care, early and extensive surgical debridement is the key intervention. After debridement, wounds are often left open and require regular assessment and dressing changes. Negative pressure wound therapy is used as a modern wound care approach and may reduce the frequency of dressing changes, pain, and exudate, while promoting the formation of granulation tissue. Discussion and conclusion: Postoperative care of a patient with Fournier's gangrene is complex, long-term, and multidisciplinary. Nursing care has an important role in the early recognition of clinical deterioration, prevention of complications, wound management, and patient support during treatment. The findings show that negative pressure wound therapy can be a useful addition to wound care, but it is not suitable for all patients and does not replace surgical debridement. Due to the limited number of studies directly addressing nursing care of patients with Fournier's gangrene, further research is needed, especially regarding the role of nurses in postoperative care.
