Abstract
Uvod: Mamografija je ključna slikovna metoda za zgodnje odkrivanje raka dojk, pri kateri sta za kakovostno diagnostiko bistveni pravilna tehnika slikanja in ustrezna namestitev pacientke. Kljub njeni diagnostični vrednosti lahko pacientke med pregledom občutijo bolečino ali nelagodje. Čeprav je bolečina zaščitni mehanizem telesa, lahko neprijetna izkušnja vpliva na odnos posameznic do preventivnih pregledov in njihovo pripravljenost za nadaljnjo udeležbo. Namen: Namen diplomskega dela je bil narediti sistematični pregled literature o bolečini med mamografijo, z namenom ugotovitve vzrokov za njen pojav ter možnih načinov za lajšanje. Cilji so preučiti psihološke in fiziološke vzroke bolečine ter oceniti tehnološke, farmakološke in nefarmakološke ukrepe, ki zmanjšujejo bolečino med mamografijo. Metode dela: Izveden je bil sistematični pregled strokovne in znanstvene literature v skladu s smernicami PRISMA. Predstavljeno literaturo smo zbrali v podatkovnih bazah: Web of Science, PubMed Central, Science Direct, EBSCO in Scopus, pri čemer smo uporabljali ključne besede: mamografija, bolečina, kompresija, presejalna mamografija in ženske. Vključili smo članke, ki so bili dostopni brezplačno in v celoti, literaturo, ki obravnava mamografijo žensk in članke napisane po letu 2000. Zaradi pomanjkanja ustreznih slovenskih člankov smo se osredotočili na angleško literaturo. Iskanje literature je potekalo med 7. januarjem in 18. februarjem 2026. Rezultati: V diplomsko nalogo smo vključili 15 študij iz 9 različnih držav. Večinoma so bile uporabljene presečne opazovalne študije. Za prikaz korakov, ki so vodili do končnega nabora člankov smo uporabili model PRISMA. Razprava in zaključek: Ugotovili smo, da na doživljanje bolečine med mamografijo vplivajo tako psihološki kot tudi fiziološki dejavniki. Zato je potreben individualen pristop za lajšanje nelagodja, ki vključuje ustrezno komunikacijo, optimizacijo postopka ter uporabo nefarmakoloških in po potrebi farmakoloških ukrepov. S skrbno načrtovano in celostno obravnavo pacientk lahko bistveno zmanjšamo njihovo nelagodje, izboljšamo zaznavanje udobja med preiskavo in zmanjšamo strah, kar posledično poveča pripravljenost žensk za redno udeležbo pri ponovnih mamografijah. Takšen pristop ne izboljša le trenutne izkušnje posameznic, temveč ima tudi dolgoročne pozitivne učinke na uspešnost presejalnih programov in zgodnje odkrivanje raka dojke. Pomembno je, da se pri tem aktivno vključujejo tudi radiološki inženirji, saj njihova strokovnost, empatična komunikacija in pozornost do pacientk neposredno vplivajo na zaznavanje bolečine.

Title:Analysis of causes and measures for pain prevention during mammography - a systematic literature review : diploma work
Abstract:
Introduction: Mammography is a key imaging method for the early detection of breast cancer, where proper imaging technique and correct patient positioning are essential for high-quality diagnostics. Despite its diagnostic value, patients may experience pain or discomfort during the examination. Although pain is a protective mechanism, an unpleasant experience may affect women’s attitudes towards preventive examinations and their willingness to attend future screenings. Purpose: The purpose of this thesis was to conduct a systematic literature review on pain during mammography to identify the causes of its occurrence and possible methods for alleviation. The objectives were to examine the psychological and physiological causes of pain and to evaluate technological, pharmacological, and non-pharmacological interventions that reduce pain during mammography. Methods: A systematic review of professional and scientific literature was conducted in accordance with PRISMA guidelines. The literature was collected from the following databases: Web of Science, PubMed Central, ScienceDirect, EBSCO, and Scopus, using the keywords: mammography, pain, compression, screening mammography, and woman/women. We included articles that were freely available in full text, literature addressing mammography in women, and articles published after 2000. Due to the lack of relevant Slovenian articles, we focused on English-language literature. The literature search was conducted between 7 January and 18 February 2026. Results: A total of 15 studies from 9 different countries were included in the thesis. Most of the included studies were cross-sectional observational studies. The PRISMA model was used to present the steps that led to the final selection of articles. Discussion and conclusion: The findings showed that both psychological and physiological factors influence the perception of pain during mammography. Therefore, an individualised approach is needed to reduce discomfort, including effective communication, procedure optimisation, and the use of non- pharmacological and, when necessary, pharmacological interventions. With carefully planned and comprehensive patient care, it is possible to significantly reduce discomfort, improve the perception of comfort during the examination, and reduce fear, thereby increasing women’s willingness to participate regularly in repeat mammography screenings. Such an approach not only improves the immediate experience of individual patients but also has long-term positive effects on the success of screening programmes and the early detection of breast cancer. It is important that radiographers are actively involved in this process, as their professional competence, empathetic communication, and attention to patients directly influence the perception of pain.
