Vezikularna miRNA-223-3p iz urina : nov obetaven neinvazivni biološki označevalec zavrnitve presajene ledvice

Abstract
Izhodišča: Presaditev ledvice je optimalno zdravljenje končne ledvične odpovedi. Uveljavljeni laboratorijski kazalci delo-vanja presadka so premalo občutljivi in nespecifični, medtem ko pa je histopatološka analiza ledvične biopsije invazivna, draga preiskava, povezana z možnimi zapleti. V raziskavi smo preverili uporabnost miRNA, zaščitenih znotraj urinskih zu-najceličnih veziklov (uZV) kot možnih bioloških označevalcev za neinvazivno odkrivanje zavrnitve presajene ledvice. Metode: V prospektivno opazovalno raziskavo smo vključili bolnike s presajeno ledvico ob nadzorni ali indikacijski biopsiji in jih na podlagi histopatoloških izvidov razdelili v 3 skupine: normalna histologija (NH, n = 16), druge okvare (DO, n = 13) in zavrnitvena okvara (ZO, n = 19). S kvantitativno verižno reakcijo s polimerazo smo analizirali izražanje izbranih miRNA iz uZV, ki smo jih prej z velikostno izključitveno kromatografijo izolirali iz drugega jutranjega urina. Rezultati: Skupine so se statistično značilno razlikovale glede na vrsto ledvične biopsije (p < 0,001), čas od presaditve do biopsije (p = 0,006), prisotnost protiteles proti neskladnim humanim levkocitnim antigenom darovalca (angl. donor-speci-fic antibodies, DSA; p < 0.001), serumski kreatinin (p = 0,021) in ocenjeno glomerulnofiltracijo (p = 0,019). Skupini ZO in DO sta imeli v primerjavi z bolniki NH značilno višje normalizirane koncentracije uZV (p = 0,019). Pri bolnikih z ZO so bile ravni miR-223-3p iz uZV zvišane (p = 0,035). Višje izražanje miR-155 (p = 0,012) in miR-181a (p = 0,004) iz uZV je bilo povezano z vrsto biopsije, višje izražanje miR-223-3p iz uZV pa s prisotnostjo DSA (p = 0,024). Zaključki: Zvišana koncentracija uZV je povezana z okvaro presajene ledvice. Rezultati kažejo, da je miRNA-223-3p iz uZV obetaven neinvazivni biološki označevalec zavrnitve presadka.

Title:Extracellular vesicle-associated miRNA-233-3p in urine : promising non-invasive biomarker candidate for kidney allograft rejection
Abstract:
Background: Kidney transplantation is the optimal treatment for end-stage kidney disease. The analytes currently used to assess allograft function lack sensitivity and specificity, while histopathologic analysis of biopsy tissue is invasive, costly, and associated with potential complications. We investigated the applicability of extracellular vesicle (uEV)-associated miRNAs in urine as potential non-invasive biomarkers for the detection of kidney allograft rejection. Methods: Using quantitative PCR, we analyzed the expression profile of miRNA extracted from uEVs, which were isolated from second-morning urine by size exclusion chromatography, and associated their expression with kidney allograft injury. Based on histopathological characterization of surveillance or for-cause allograft biopsies, we divided patients into 3 groups: normal histology (NH, n = 16), non-rejection injury (NRI, n = 13), and rejection injury (RI, n = 19). Results: Patient groups showed significant differences in type of biopsy (p < 0.001), time between transplantation and biopsy (p = 0.006), presence of donor-specific antibodies (DSA; p < 0.001), serum creatinine (p = 0.021), and estimated glomerular filtration rate (p = 0.019). The NRI or RI group had significantly higher normalized uEV concentrations compared to NH patients (p = 0.019). Higher expression of miR-223-3p was found in the RI group (p = 0.035). miR-155 (p = 0.012) and miR-181a (p = 0.004) expression were associated with the type of biopsy, while miR-223-3p correlated with the presence of DSA (p = 0.024). Conclusion: Increased levels of uEVs are associated with kidney allograft injury. miR-223-3p, extracted from uEVs, may serve as a potential non-invasive biomarker for kidney allograft rejection.
