Uporabnost ultrazvočne elastografije v študijah na področju kineziologije in fizioterapije : primer iliotibialnega trakta in mišice tensor fascia lata

Abstract
Ultrazvočna elastografija je metoda, ki se čedalje več uporablja v kineziologiji in fizioterapiji za vrednotenje elastičnosti tkiv. Ta pregledni članek prikazuje možnost uporabe elastografije pri iliotibialnem traktu (ITB) in mišici tensor fasciae latae (TFL), strukturah, ki so ključne za biomehaniko spodnjega uda in pogosto povezane s poškodbami. Hkrati želimo s tem primerom opozoriti na nekatere pomembne metodološke dejavnike pri uporabi te metode. Sistematično iskanje člankov smo izvedli v podatkovnih zbirkah PubMed in PEDro, pri čemer smo glede na vnaprej določene vključitvene in izključitvene kriterije v končno analizo vključili 10 študij. Študije kažejo, da je elastografija strižnih valov potencialno uporabna metoda za merjenje togosti teh tkiv, pri čemer na rezultate pomembno vplivajo dejavniki, kot so usmeritev in pritisk ultrazvočne sonde, globina tkiva ter položaj preiskovanca. V sklopu našega pregleda literature je bila ena izmed analiziranih raziskav osredotočena na sindrom iliotibialnega trakta (ITBS). V njej so nakazali, da bi bila elastografija lahko uporabna metoda pri diagnostiki in spremljanju rehabilitacije ITBS, saj omogoča ocenjevanje mehanskih lastnosti tkiva in natančnejše sledenje spremembam med zdravljenjem. Raztezanje in miofascialno sproščanje z valjčkom sta pogosto uporabljeni metodi rehabilitacije, vendar njun vpliv na togost ITB ostaja nejasen. Elastografija omogoča kvantitativno oceno mehanskih lastnosti mehkih tkiv, kar ponuja možnosti za izboljšano diagnostiko in spremljanje rehabilitacije.

Title:The usefulness of ultrasound elastography in studies in the field of kinesiology and physiotherapy : the case of the iliotibial tract and the tensor fascia lata muscle
Abstract:
Ultrasound elastography is a method increasingly used in kinesiology and physiotherapy to assess tissue elasticity. This review article explores the potential application of elastography through the example of the iliotibial band (ITB) and the tensor fasciae latae (TFL) muscle, structures essential for lower limb biomechanics and frequently associated with injuries. At the same time, we want to use this example to highlight some important methodological factors in the use of this method. We conducted a systematic search of articles in the PubMed and PEDro databases, and based on predefined inclusion and exclusion criteria, we included 10 studies in the final analysis. Studies indicate that shear wave elastography is a potentially useful method for measuring the stiffness of these tissues, with factors such as transducer orientation and pressure, tissue depth, and patient positioning significantly affecting the results. As part of our literature review, one of the analyzed studies focused on iliotibial band syndrome (ITBS). It suggested that elastography could be a valuable tool for diagnosing and monitoring ITBS rehabilitation, as it allows for the assessment of tissue mechanical properties and more precise tracking of changes during treatment. Stretching and foam rolling are commonly used interventions, but their effect on ITB stiffness remains unclear. Elastography provides a quantitative assessment of the mechanical properties of soft tissues, offering opportunities for improved diagnostics and rehabilitation monitoring.
