Molekularna in fenotipska karakterizacija klinično pomembnih sevov bakterije Campylobacter concisus

Abstract
V raziskavi smo proučevali klinični pomen bakterije C. concisus pri akutni infekcijski driski pri odraslih z namenom ugotoviti njen prispevek k etiologiji driske, fenotipsko in genotipsko občutljivost za antibiotike, prisotnost virulentnih dejavnikov ter genetsko raznolikost izolatov. Skupaj smo analizirali 142 izolatov, pridobljenih iz retrospektivnega in prospektivnega dela raziskave. C. concisus je bil edini identificirani črevesni patogen pri 24 od 246 bolnikov (9,8  %), pri čemer sta bila patotipa AICC in/ali AToCC prisotna v 3,2 % primerov, večina izolatov pa ni pripadala znanim patotipom, kar nakazuje prisotnost dodatnih, še neznanih virulentnih dejavnikov. Neposredna PCR detekcija iz vzorcev iztrebkov je imela omejeno klinično uporabnost. Fenotipsko testiranje je pokazalo visoko občutljivost za eritromicin (94–100 %) in tetraciklin (99 %), medtem ko je bila občutljivost za ciprofloksacin nekoliko nižja (86 %). Molekularne analize so razkrile, da prisotnost genov ali mutacij, povezanih z odpornostjo, ne sovpada vedno s fenotipsko odpornostjo. WGS je potrdil visoko genetsko heterogenost, odsotnost klonalnega širjenja ter prisotnost plazmidov in sekrecijskih sistemov, kar nakazuje kompleksno epidemiologijo in možnost horizontalnega prenosa genov. Rezultati podpirajo ugotovitev, da je C. concisus pomemben, vendar pogosto spregledan enterični patogen, katerega patogenost je heterogena in verjetno odvisna od interakcij z gostiteljem ter še neznanih genetskih dejavnikov.

Title:Molecular and phenotypic characterization of Campylobacter concisus clinically relevant strains
Abstract:
This study investigated the clinical relevance of C. concisus in acute infectious diarrhoea in adults, aiming to elucidate its contribution to disease aetiology, phenotypic and genotypic antimicrobial susceptibility, the presence of virulence factors, and genetic diversity. A total of 142 isolates were analysed from both retrospective and prospective components of the study. C. concisus was identified as the sole intestinal pathogen in 24 of 246 patients (9.8 %), with AICC and/or AToCC patotypes present in 3,2 % of cases, while most isolates did not belong to known patotypes, suggesting the presence of additional, yet uncharacterised virulence determinants. Direct PCR detection from stool samples showed limited clinical utility. Phenotypic testing revealed high susceptibility to erythromycin (94-100 %) and tetracycline (99 %), whereas susceptibility to ciprofloxacin was slightly lower (86 %). Molecular analyses demonstrated that the presence of resistance-associated genes or mutations did not consistently correlate with phenotypic resistance. WGS confirmed pronounced genetic heterogeneity, the absence of clonal dissemination, and the presence of plasmids and secretion systems, indicating a complex epidemiology and the potential for horizontal gene transfer. Overall, these findings support the conclusion that C. concisus is an important yet often overlooked enteric pathogen, whose pathogenicity is heterogeneous and likely influenced by host interactions and additional, as yet unidentified, genetic factors.
