Abstract
Uvod: Možganska kap povzroča vrsto motoričnih in kognitivnih posledic, med katerimi je tudi afazija. Ta motnja razumevanja in izražanja govora močno omejuje posameznika pri vsakodnevni komunikaciji, zmanjšuje njegovo socialno vključevanje ter negativno vpliva na kakovost življenja. Čeprav je obravnava afazije tradicionalno v domeni logopedije, je za celostno povrnitev funkcioniranja nujna vključitev delovne terapije. Ta z uporabo smiselnih vsakodnevnih aktivnosti spodbuja samostojnost, krepi posameznikovo okupacijsko identiteto in omogoča aktivno vključevanje v družbo kljub komunikacijskim oviram. Namen: Namen diplomskega dela je raziskati značilnosti delovnoterapevtske obravnave oseb z afazijo po možganski kapi s poudarkom na vlogi terapevta, poteku obravnave, komunikacijskih strategijah, timskem sodelovanju in vključevanju svojcev. Raziskava se osredotoča na pogled delovnih terapevtov na značilnosti tovrstne obravnave. Metode dela: V raziskavi smo uporabili kvalitativni pristop z namenskim vzorčenjem. Podatke smo zbrali s polstrukturiranimi intervjuji, nato pa izvedli kvalitativno vsebinsko analizo. Rezultati: Identificirali smo šest ključnih kategorij: značilnosti in pojavnost afazije po možganski kapi, proces in okoliščine delovnoterapevtske obravnave, strokovni razvoj in doživljanje terapevta, komunikacijske strategije pri obravnavi afazije, multidisciplinarni pristop ter vključevanje svojcev v obravnavo. Razprava in zaključek: Ugotavljamo, da je delovnoterapevtska obravnava oseb z afazijo po možganski kapi kompleksen, na uporabnika usmerjen proces, ki zaradi komunikacijskih primanjkljajev zahteva pomembne prilagoditve na vseh stopnjah – od seznanitve z uporabnikom, začetnega ocenjevanja ter vse do vrednotenja izidov. Uspešnost obravnave sloni na terapevtski uporabi sebe ter prilagojeni interakciji z uporabo alternativnih načinov komunikacije. Ključna sta vključevanje celotnega interdisciplinarnega tima in aktivno sodelovanje svojcev, ki lahko delujejo kot pomemben podporni dejavnik za prehod v domače okolje. Obravnava za delovne terapevte predstavlja strokovni in čustveni izziv, pri čemer k njihovi samozavesti pomembno prispevajo specifična nevrološka znanja, izkušnje ter sistemska in timska podpora.

Title:Occupational therapy for stroke survivors with aphasia : diploma work
Abstract:
Introduction: Stroke causes a range of motor and cognitive consequences, including aphasia. This disorder of language comprehension and expression severely limits individuals in their daily communication, reduces their social inclusion, and negatively impacts their quality of life. Although the management of aphasia is traditionally within the domain of speech and language therapy, the inclusion of occupational therapy is essential for a holistic recovery of functioning. By utilizing meaningful daily activities, occupational therapy promotes independence, strengthens the individual's occupational identity, and enables active integration into society despite communication barriers. Purpose: The purpose of this thesis is to explore the characteristics of occupational therapy intervention for individuals with post-stroke aphasia, with an emphasis on the role of the therapist, the intervention process, communication strategies, teamwork, and the involvement of relatives. The research focuses on the occupational therapists' perspective regarding the characteristics of such interventions. Methods: A qualitative approach with purposive sampling was used in the study. Data were collected through semi-structured interviews, followed by qualitative content analysis. Results: Six key categories were identified: the characteristics and incidence of post-stroke aphasia, the process and circumstances of occupational therapy intervention, the professional development and experiences of the therapist, communication strategies in aphasia management, the multidisciplinary approach, and the involvement of relatives in the intervention process. Discussion and conclusion: We conclude that occupational therapy intervention for individuals with post-stroke aphasia is a complex, client-centered process that, due to communication deficits, requires significant adaptations at all stages—from the initial introduction and assessment to the evaluation of outcomes. The success of the intervention relies on the therapeutic use of self and adapted interaction using alternative methods of communication. The involvement of the entire interdisciplinary team and the active cooperation of relatives, who can act as an important supportive factor for the transition to the home environment, are crucial. For occupational therapists, this intervention represents both a professional and emotional challenge, where specific neurological knowledge, experience, and systemic and team support contribute significantly to their confidence.
