Abstract
Uvod: Nagib medenice je pomemben biomehanski dejavnik, ki vpliva na poravnavo hrbtenice, delovanje kolčnega sklepa in razporeditev obremenitev v ledveno-medenično-kolčnem segmentu. Spremembe v nagibu medenice lahko vodijo v prilagajanje ledvene lordoze ter s tem vplivajo na gibljivost in obremenitev kolkov. Neuravnotežen položaj medenice je pogosto povezan z bolečino in omejeno funkcionalnostjo kolčnega sklepa ter lahko prispeva k razvoju utesnitvenega sindroma kolka. Raziskave kažejo, da lahko ciljana vadba, stabilizacijske tehnike in terapevtsko lepljenje trakov vplivajo na položaj medenice, izboljšajo biomehaniko gibanja ter nadzor trupa. Kljub temu ostaja nejasno, v kolikšni meri je mogoče z vadbo dolgoročno spremeniti nagib medenice in ali te spremembe pomembno vplivajo na zmanjšanje bolečine ali preprečevanje patoloških stanj kolčnega sklepa. Namen: Namen diplomskega dela je pregledati literaturo o povezavi med položajem medenice, bolečino v kolku in utesnitvenim sindromom kolka ter ugotoviti, ali vadba lahko vpliva na spremembo položaja medenice. Metode dela: Iskanje literature je potekalo oktobra 2025 v podatkovni bazi PubMed. V pregled so bile vključene raziskave, objavljene med letoma 2020 in 2025. Uporabili smo kombinacijo iskalnih besed: »nagib medenice«, »fizioterapija«, »sklepna mobilizacija«, »vadba gibalnih vzorcev«, »lepljenje trakov«, »možganska kap«, »ledvena lordoza«, »gibljivost kolka« in »sistematični pregled«. Rezultati: V pregled je bilo vključenih šest raziskav s skupno 310 preiskovanci iz heterogenih populacij. Intervencije, usmerjene v nagib medenice, so v večini raziskav privedle do zmanjšanja anteriornega nagiba medenice in izboljšanja poravnave hrbtenice, zlasti ob kombinaciji vadbe in uporabe terapevtskih trakov. V nekaterih raziskavah so bile opažene tudi izboljšave aktivacije in zmogljivosti mišic trupa ter kolka. Več raziskav je pokazalo izboljšano funkcijsko biomehaniko kolka in klinično pomembno zmanjšanje bolečine, pri čemer med primerjanimi intervencijami večinoma ni bilo statistično značilnih razlik. Razprava in zaključek: Pregled literature kaže, da ima nagib medenice pomembno vlogo pri biomehaniki kolka, ledveni lordozi in pojavu bolečine. Ciljno usmerjena vadba za moč in stabilizacijo trupa v kombinaciji z raztezanjem in po potrebi uporabo terapevtskih trakov lahko izboljša položaj medenice in zmanjša simptome, medtem ko samostojne intervencije brez aktivnega mišičnega nadzora ne zagotavljajo trajnih učinkov.

Title:The impact of exercise on pelvic tilt and hip biomechanics - literature review : diploma work
Abstract:
Introduction: Pelvic tilt is an important biomechanical factor that affects spinal alignment, hip joint function, and load distribution in the lumbopelvic–hip segment. Changes in pelvic tilt can lead to adaptations in lumbar lordosis, thereby influencing hip mobility and joint loading. An imbalanced pelvic position is often associated with pain and limited hip function and may contribute to the development of femoroacetabular impingement. Research indicates that targeted exercise, stabilization techniques, or taping can influence pelvic position and improve movement biomechanics and trunk control. However, it remains unclear to what extent exercise can produce long-term changes in pelvic tilt and whether these changes significantly reduce pain or prevent pathological hip conditions. Purpose: The purpose of this thesis is to review the literature on the relationship between pelvic position, hip pain, and femoroacetabular impingement and to determine whether exercise can influence pelvic tilt. Methods: Literature was searched in the PubMed database. We used a combination of the following keywords: »pelvic tilt« AND »physiotherapy«, »joint mobilization« AND »movement pattern training«, »pelvic tilt« AND »taping« AND »stroke«, »posterior pelvic tilt taping« AND »lumbar lordosis« and »posterior pelvic tilt« AND »hip motion« AND »systematic review«. Results: Six studies involving a total of 310 participants from heterogeneous populations were included in the review. Interventions targeting pelvic tilt mostly led to a reduction of anterior pelvic tilt and improved spinal alignment, particularly when exercise was combined with taping. Some studies also reported improvements in trunk and hip muscle activation and strength. Several studies demonstrated improved functional hip biomechanics and clinically significant pain reduction, with no statistically significant differences between the compared interventions in most cases. Discussion and Conclusion: The literature review indicates that pelvic tilt plays an important role in hip and lumbar spine biomechanics and in the occurrence of pain. Targeted exercise for strength and trunk stabilization, combined with stretching and, if necessary, taping, can improve pelvic position and reduce symptoms, whereas interventions without active muscular control do not provide lasting effects.
