Vpliv izražanja genov RUNX1, RUNX2, TGFB in VEGFA na histološke spremembe kostnega tkiva pri osteoporozi in sarkopeniji

Abstract
V populaciji je vedno več starejšega prebivalstva, saj se življenjska doba povečuje. S tem narašča pojavnost s staranjem povezanih bolezni in potreba po razvoju novih pristopov ter odkrivanju novih terapevtskih tarč za te bolezni. S staranjem mišično-skeletnega sistema se povečuje pojavnost osteoporoze, dodatna prisotnost mišične oslabelosti, tj. pri osteosarkopeniji, pa pomeni še izrazitejše funkcionalno opešanje in izgubo samostojnosti pri starostnikih ter predstavlja obremenitev ne le za paciente, temveč tudi za svojce in zdravstveni sistem. RUNX1, RUNX2, TGF-β1 in VEGFA so znani endogeni regulatorji kostne remodelacije. Zanimalo nas je, ali izražanje teh genov, ki uravnavajo nastajanje in delovanje osteoblastov, vpliva na strukturo kostnega tkiva tako pri osteoporozi kot tudi pri osteosarkopeniji. Preveriti smo želeli hipotezo, da je uravnavanje kostne tvorbe pri osteosarkopeniji še bolj moteno kot pri sami osteoporozi, kar vodi v hujše spremembe kostnega tkiva in s tem v še večjo lomljivosti kosti, kot pri osteoporozi. V raziskavo smo vključili 58 preiskovank, pri katerih so zaradi zloma kolka izvedli artroplastiko kolka. Pripravili smo 55 histoloških preparatov kostnega tkiva osteoporoznih in osteosarkopeničnih bolnic, ki smo jih barvali s hematoksilinom in eozinom ter izvedli trikromno barvanje po Massonu. Na podlagi mikroskopskega pregleda preparatov smo izmerili povprečni volumski delež kosti in povprečni premer kostnih trabekul, dva ključna parametra za oceno poroznosti kosti. Pri vseh preiskovankah smo z indeksom mišične mase ocenili količino mišic, s testom stiska pesti pa jakost mišic. V kostnem tkivu smo izmerili izražanje genov z uporabo metode kvantitativne verižne reakcije s polimerazo v realnem času. Rezultate smo z uporabo Mann-Whitneyjevega U testa, t-testa neodvisnih vzorcev ter Pearsonove in Spearmanove korelacije analizirali v programu SPSS. Naši rezultati so pokazali, da je pri osteosarkopeničnih bolnicah v primerjavi z osteoporoznimi bolnicami statistično značilno višje izražanje gena RUNX2 (p = 0,041) ter značilno nižje izražanje gena VEGFA (p = 0,041). V skupini osteosarkopeničnih pacientk smo dokazali pozitivno in signifikantno korelacijo med izražanjem teh dveh genov (p = 0,001). Pri tem je izražanje gena RUNX2 pri osteosarkopeničnih bolnicah značilno koreliralo tudi z izražanjem gena RUNX1 (p = 0,001) in izražanejm gena TGFB1 (p = 0,015), izražanje gena VEGFA v tej skupini pa prav tako z izražanji genov RUNX1 (p = 0,045) in RUNX2 (p = 0,001). Od ostalih povezav pri osteosarkopeniji se je kot značilna pokazala povezava med izražanjem genov RUNX1 in TGFB1 (p = 0,001). Pri osteoporozi so povezave med izražanji genov že znane in smo jih potrdili tudi v naši raziskavi, in sicer povezave med izražanjem gena RUNX1 in izražanjem gena RUNX2 (p = 0,001) ter TGFB1 (p = 0,038). Naši rezultati kažejo na pomembne razlike v izražanju regulatornih genov pri osteosarkopeniji glede na osteoporozo. To je verjetno posledica večjega oksidativnega stresa in vnetja ter drugih procesov, ki so zaradi dodatno prisotne sarkopenije pri osteosarkopeniji bolj izraženi kot pri sami osteoporozi. Nadaljnje raziskave za odkritje ključnih dejavnikov v teh procesih so potrebne, saj so lahko prav te molekule izhodišča za učinkovitejše zdravljenje osteosarkopenije oziroma starostne krhkosti.

Title:The effect of RUNX1, RUNX2, TGFB and VEGFA gene expression on bone tissue histological changes in osteoporosis and sarcopenia
Abstract:
The proportion of the elderly population is increasing as life expectancy continues to rise. Consequently, the prevalence of age-related diseases is also increasing, along with the need to develop new approaches and identify novel therapeutic targets for their treatment. With aging of the musculoskeletal system, the incidence of osteoporosis increases, while the additional presence of muscle weakness, i.e., osteosarcopenia, leads to even more pronounced functional decline and loss of independence in older adults. This represents a burden not only for patients but also for their caregivers and the healthcare system. RUNX1, RUNX2, TGF-β1, and VEGFA are well-known endogenous regulators of bone remodeling. The aim of this study was to investigate whether the expression of these genes, which regulate the formation and function of osteoblasts, influences bone tissue structure in both osteoporosis and osteosarcopenia. We aimed to test the hypothesis that the regulation of bone formation is more severely impaired in osteosarcopenia than in osteoporosis alone, leading to more pronounced alterations in bone tissue and consequently increased bone fragility. The study included 58 female participants who underwent hip arthroplasty due to a hip fracture. A total of 55 histological bone tissue samples from osteoporotic and osteosarcopenic patients were prepared and stained with hematoxylin and eosin, as well as Masson’s trichrome stain. Based on microscopic evaluation, the mean bone volume fraction and the average trabecular thickness—two key parameters for assessing bone porosity—were measured. In all participants, muscle mass was assessed using the skeletal muscle mass index, and muscle strength was evaluated using the handgrip strength test. Gene expression in bone tissue was measured using quantitative real-time polymerase chain reaction (qPCR). Statistical analysis was performed using the Mann–Whitney U test, independent samples t-test, and Pearson and Spearman correlation coefficients in the SPSS software. Our results showed that, compared to osteoporotic patients, osteosarcopenic patients exhibited significantly higher expression of RUNX2 (p = 0,041) and significantly lower expression of VEGFA (p = 0,041). In the osteosarcopenic group, a positive and significant correlation between the expression of these two genes was observed (p = 0,001). Furthermore, in this group, RUNX2 expression significantly correlated with RUNX1 (p = 0,001) and TGFB1 (p = 0,015), while VEGFA expression also correlated with RUNX1 (p = 0,045) and RUNX2 (p = 0,001). Among other associations in osteosarcopenia, a significant correlation between RUNX1 and TGFB1 expression was identified (p = 0,001). In osteoporosis, the relationships between gene expression levels are already well established and were confirmed in our study, specifically the associations between RUNX1 and RUNX2 (p = 0,001), as well as TGFB1 (p = 0,038). Our findings indicate important differences in the expression of regulatory genes in osteosarcopenia compared to osteoporosis. This is likely due to increased oxidative stress, inflammation, and other processes that are more pronounced in osteosarcopenia due to the additional presence of sarcopenia. Further research is needed to identify the key factors involved in these processes, as these molecules may represent potential targets for more effective treatment of osteosarcopenia and age-related frailty.
