Učinkovitost transkutane avrikularne električne stimulacije vagusnega živca na motorično funkcijo zgornjega uda pri bolnikih po možganski kapi – sistematični pregled literature

Abstract
Izhodišča: Možganska kap sodi med vodilne vzroke invalidnosti ter smrti na svetovni ravni. V akutni fazi se 80–85 % bolnikov sooča z motorično okvaro zgornjih udov, pri 60 % bolnikov pa okvara vztraja tudi v kronični fazi. Skupaj z rehabilitacijskim programom se vse pogosteje uporabljajo nevromodulacijske tehnike. Transkutana avrikularna stimulacija vagusnega živca je neinvazivna alternativa kirurško vstavljeni stimulaciji vagusnega živca, ki lahko doseže primerljive učinke na možgansko aktivnost. Namen sistematičnega pregleda literature je bil ugotoviti, kako transkutana avrikularna stimulacija vagusnega živca skupaj z različnimi oblikami rehabilitacije vpliva na izboljšanje funkcije zgornjega uda pri bolnikih po možganski kapi. Metode: Sistematično smo pregledali podatkovni zbirki PubMed in PEDro. Vključili smo raziskave, objavljene leta 2015 ali kasneje. Rezultati: V pregled je bilo vključenih 6 raziskav. V 2 raziskavah so preučevali vpliv transkutane avrikularne stimulacije vagusnega živca v kombinaciji z robotsko podprto rehabilitacijo zgornjega uda. V preostalih štirih raziskavah so ugotavljali učinke transkutane avrikularne stimulacije vagusnega živca skupaj z različnimi oblikami vadbe za zgornji ud, v eni od raziskav pa so uporabili tudi vaje za nadzor drže, proprioceptivne vaje, nevromuskularno facilitacijo in vadbo hoje. V vseh raziskavah je ob uporabi transkutane avrikularne stimulacije vagusnega živca prišlo do izboljšanja motoričnih funkcij zgornjega uda. Zaključek: Transkutana avrikularna stimulacija vagusnega živca je lahko v kombinaciji z drugimi fizioterapevtskimi postopki učinkovitejša možna oblika fizioterapije kot standardna fizioterapevtska obravnava brez električne stimulacije. V prihodnosti bi bile potrebne nadaljnje raziskave z večjim vzorcem, enotnejšimi parametri, obdobjem in obliko vadbe.

Title:Efficacy of transcutaneous auricular vagus nerve stimulation on upper limb motor function in post-stroke patients – systematic literature review
Abstract:
Background: Stroke is one of the most common causes of disability and death worldwide. In the acute phase, 80-85% of patients experience upper-limb motor impairment, and in 60% of cases, the impairment persists into the chronic phase. Neuromodulation techniques are increasingly being used to supplement rehabilitation programmes. Transcutaneous auricular vagus nerve stimula-tion is a non-invasive alternative to surgically implanted vagus nerve stimulation and produces comparable effects on brain activi-ty. This literature review aimed to investigate the efficacy of transcutaneous auricular vagus nerve stimulation, combined with various forms of rehabilitation, on upper limb motor function in post-stroke patients. Methods: A systematic literature review was conducted in PubMed and PEDro. Studies published after 2014 were included. Results: Six studies were included in the review. Two studies investigated the effects of transcutaneous auricular vagus nerve stimulation, combined with robotic upper-limb rehabilitation. The remaining four studies investigated the effects of transcutane-ous auricular vagus nerve stimulation, combined with various upper-limb exercises, with one study also including postural control exercises, proprioceptive training, neuromuscular facilitation and gait training. All studies reported improvements in upper limb motor function. Conclusion: Transcutaneous auricular stimulation of the vagus nerve in combination with other physiotherapeutic interventions may be a potentially more effective approach than standard physiotherapy without electrical stimulation. Future studies with larger sample sizes, standardised stimulation parameters, intervention durations, and exercise protocols are required.
