Repozicioniranje zdravil : metformin in valprojska kislina za zdravljenje glioblastoma

Abstract
Glioblastom je najpogostejša in najbolj agresivna oblika primarnega možganskega tumorja pri človeku. Zdravljenje, ki omogoča podaljšanje življenjske dobe pacientov do 15 mesecev, obsega operativni poseg, obsevanje in kemoterapijo. Zaradi specifične lokacije tumorja je odkrivanje novih zdravil zapleteno in drago, zato je repozicioniranje že znanih zdravil časovno in ekonomsko učinkovit pristop za odkrivanje alternativnih oblik zdravljenja. Številna že ob stoječa zdravila, katerih prvotni namen ni zdravljenje raka, so že izkazala svoj potencial pri zdravljenju glioblastoma. Pri zdravljenju glioblastoma pozornost posvečamo učinkovinam, ki lahko prehajajo krvno možgansko pregrado: antidiabetikom, antipsihoti kom in antiepileptikom. Raziskave kažejo, da lahko kombinacije antidiabetika metformina oz. antiepileptika valprojske kisline s standardno terapijo za glioblastom podaljšajo preživetje bolnikov. Kljub vsem do sedaj znanim dejstvom, pa je potrebno natančneje opredeliti mehanizme delovanja in vloge repozicioniranih učinkovin pri razvoju in zdravljenju glioblastoma, da preprečimo nepotrebne zaplete in morebitne škodljive učinke.

Title:Drug repurposing : metformin and valproic acid for glioblastoma treatment
Abstract:
Glioblastoma is the most common and lethal brain malignancy. Standard treatment consists of surgery, radiotherapy and chemotherapy, and results in life expectancy of 15 months after diagnosis. Due to the specific location of the tumor, designing new drugs is complicated and expensive. Drug repur- posing is an economic and time-saving way for identification of alternative treatment options. Nu- merous existing drugs whose initial purpose was not to treat cancer, have already proven successful against glioblastoma. For glioblastoma, attention is paid to antidiabetic drugs, antipsychotics and antiepileptics that can easily cross the blood-brain barrier. Studies show that adding the antidiabetic metformin or antiepileptic drug valproic acid to the standard glioblastoma therapy might prolong patient survival. However, the exact mechanisms of action should further be studied and possible interactions should be examined in order to avoid unnecessary complications and adverse effects.
