Ocenjevanje sarkopenije z izvedbenimi funkcijskimi testi pri bolnikih z vstavljeno mehansko cirkulatorno podporo levega prekata – sistematični pregled literature

Abstract
Izhodišča: Sarkopenija je pogosto prisotna pri bolnikih s srčnim popuščanjem, tudi pri tistih, ki so izbrani za vstavitev mehanske cirkulatorne podpore levega prekata. Sarkopenija je pomemben negativen dejavnik tveganja za izid zdravljenja bolnikov z mehansko cirkulatorno podporo levega prekata. Namen sistematičnega pregleda literature je bil analizirati metodološke pristope za ocenjevanje sarkopenije z izvedbenimi funkcijskimi testi pri bolnikih pred vstavitvijo mehanske cirkulatorne podpore levega prekata ali po njej. Metode: Sistematično smo pregledali podatkovno zbirko PubMed do vključno novembra 2024. Pregledali smo raziskave glede na uporabljene izvedbene funkcijske teste za oceno sarkopenije, obdobje ocenjevanja bolnikov in izvedljivost funkcijskih testov. Rezultati: V pregled smo vključili 8 raziskav. V izbranih raziskavah so bolnike ocenili z različnimi izvedbenimi funkcijskimi testi, kot so merjenje jakosti prijema roke, testi hoje, časovno merjeni test vstani in pojdi, ročna dinamometrija mišic iztegovalk kolena, skupina testov telesne zmogljivosti in test vstajanja s stola. Ugotovitve pregledanih raziskav kažejo na pomembnost ocenjevanja sarkopenije z izvedbenimi funkcijskimi testi ter poudarjajo tudi težave z izvedljivostjo določenih testov pri teh bolnikih. Najpogosteje uporabljeni test z visoko stopnjo izvedljivosti je bil test merjenja jakosti prijema roke. Zaključek: Metodološki pristopi objavljenih raziskav so precej raznoliki. Ocenjevanje sarkopenije pri bolnikih pred vstavitvijo mehanske cirkulatorne podpore levega prekata ali po njej se izvaja z različnimi izvedbenimi funkcijskimi testi, ki imajo glede na stanje bolnika različno stopnjo izvedljivosti. V nadaljnjih raziskavah bi bilo potrebno prepoznati tiste izvedbene funkcijske teste, ki so klinično najbolj uporabni in izvedljivi ter najbolje napovedujejo izid zdravljenja bolnikov po vstavitvi mehanske cirkulatorne podpore levega prekata.

Title:Functional assessment of sarcopenia in patients with left ventricular assist device – systematic literature review
Abstract:
Background: Sarcopenia is common in patients with heart failure, including those selected for a left ventricular assist device. Sarcopenia is a significant negative risk factor for the outcome in patients with a left ventricular assist device. This systematic literature review aimed to analyze the methodological approaches to the assessment of sarcopenia using functional tests in patients with left ventricular assist devices. Methods: We systematically searched the PubMed database until the end of November 2024. We reviewed the studies based on the functional tests used to assess sarcopenia, the period of patient assessment, and the feasibility of the functional tests. Results: We included eight studies in the review. In the selected studies, sarcopenia was assessed using various functional tests, such as hand-grip strength measurements, walking tests, Timed Up and Go Test, hand-held knee extension dynamometry, a short physical performance battery, and a Sit-to-stand test. The results of the studies examined show the importance of assessing sarcopenia using functional tests and also highlight the difficulties in performing certain functional tests in these patients. The most commonly used test with a high degree of feasibility was the hand-grip strength test. Conclusion: The methodological approaches of the published studies are quite different. The assessment of sarcopenia in patients before or after implantation of a left ventricular assist device is based on different functional tests, the feasibility of which varies depending on the patient’s condition. Further research should identify the functional tests that are most clinically useful and feasible and that best predict patient outcome after left ventricular assist device implantation.
