Detekcija nukleinskih kislin virusa BK v biopsijah ledvic pred in po transplantaciji

Abstract
Okužba s poliomavirusom BK (BKPyV) je pomembna zaradi možnega vpliva na delovanje in preživetje presajene ledvice. Uporaba imunosupresivne terapije lahko povzroči reaktivacijo in nekontrolirano pomnoževanje virusa v presajeni ledvici, kar lahko vodi v pojav s poliomavirusom BK povezane nefropatije (BKPyVAN) in možno izgubo presadka. Možnosti zdravljenja so omejene, zato sta zgodnje odkrivanje BKPyV in zmanjšanje imunosupresivne terapije ključnega pomena pri obvladovanju bolezni. Biopsija presajene ledvice ostaja zlati standard za diagnosticiranje BKPyVAN, čeprav gre za invaziven poseg s tveganjem za lažno negativne rezultate zaradi žariščne narave bolezni. V naši raziskavi smo z metodo qPCR preverjali prisotnost nukleinskih kislin BKPyV v 289 biopsijskih vzorcih ledvic, med katerimi je bilo 89 vzorcev darovalcev in 200 vzorcev prejemnikov ledvic pred in po presaditvi. Prisotnost DNA BKPyV smo dokazali v 7,0 % testirane slovenske populacije brez znakov BKPyVAN. Na podlagi histoloških in imunohistokemičnih preiskav tkiva smo zbrane vzorce razvrstili v dve skupini: skupino s potrjeno BKPyVAN in skupino z drugačnimi izidi kot BKPyVAN (BKPyVAN negativni). Primerjava dinamike okužbe je pokazala, da je bilo stanje pred presaditvijo v obeh skupinah primerljivo, kar kaže na prisotnost latentne okužbe s samoomejevalnim podvojevanjem. Nasprotno smo po presaditvi v skupini BKPyVAN zaznali izrazito povečanje virusne aktivnosti, značilno za reaktivacijo in prehod v aktivno okužbo. S tem smo uspeli jasno razlikovati med latentno okužbo in reaktivacijo, povezano z razvojem BKPyVAN. Z magistrsko nalogo smo tako prvič preverjali prisotnost okužbe v slovenski populaciji in hkrati ovrednotili zaznavo nukleinskih kislin v tkivu kot možno diagnostično metodo za odkrivanje okužbe z BKPyV.

Title:Detection of BK virus nucleic acids in pre- and post-transplant kidney biopsies/recipients
Abstract:
BK polyomavirus (BKPyV) infection is significant due to its potential impact on the function and survival of the kidney allograft. The use of potent immunosuppressants can lead to reactivation and uncontrolled replication of the virus, which may result in BK polyomavirus-associated nephropathy (BKPyVAN) and, ultimately, graft loss. Treatment options are limited; therefore, early detection of BKPyV and reduction of immunosuppressants are crucial for disease management. Renal biopsy remains the gold standard for diagnosis, despite its invasive nature and the risk of false-negative results due to the focal nature of BKPyVAN. In our study, we used qPCR to detect the presence of BKPyV nucleic acids in 289 kidney samples, including 89 from kidney donors and 200 from transplant recipients, collected before and after transplantation. We confirmed the presence of BKPyV DNA in 7.0 % of the analysed Slovenian population without histological evidence of BKPyVAN. Based on the histological and immunohistochemical evaluations, the samples were divided into two groups: one with confirmed BKPyVAN and the other without the evidence of BKPyVAN (BKPyVAN-negative group). We compared the dynamics of infection between the two groups and found that the pre-transplant viral status was similar in both groups, suggesting the presence of a latent infection with self-limiting replication. In contrast, post-transplantation, the BKPyVAN group exhibited a significant increase in viral activity, characteristic of reactivation and progression to active infection. This distinction enabled us to differentiate latent infection from reactivation associated with the onset of BKPyVAN. The thesis is the first to confirm the presence of BKPyV infection in the Slovenian population and to evaluate whether nucleic acid detection in renal tissue can serve as a valuable supplementary diagnostic method for identifying BKPyV infection.
