Fuzija mezenhimskih matičnih/stromalnih celic v regenerativni medicini: sistematični pregled literature

Abstract
Mezenhimske matične/stromalne celice (MSC) so predmet mnogih raziskav na področju regenerativne medicine. Predstavljajo edinstveno vrsto celic, ki jih lahko izoliramo iz različnih tkiv odraslih posameznikov, najpogosteje iz kostnega mozga in maščobnega tkiva, narašča pa tudi zanimanje za uporabo manj invazivnih virov pridobivanja, kot so zobna pulpa, popkovnica in druga tkiva. Zmožne so več-linijske diferenciacije v najrazličnejša tkiva, zato se v regenerativni medicini uporabljajo v razvoju novih celičnih terapij za številne bolezni. Dokazano je bilo, da so takšne terapije varne in imajo zelo malo neželenih učinkov. Delujejo lahko preko več mehanizmov: transdiferenciacije, s prenosom mitohondrijev, parakrino, z izločanjem zunajceličnih veziklov in s celično fuzijo, ki smo jo podrobneje obravnavali v naši nalogi. Celična fuzija je lahko naraven ali patološki proces in je vključena npr. v razvoj ploda in imunski odziv v telesu. Kasnejše raziskave so dokazale tudi fuzijo med matičnimi in tkivno specifičnimi celicami, kar ponuja nove možnosti za terapevtsko rabo. Z namenom, da bi ovrednotili pomen celične fuzije kot enega izmed mehanizmov MSC in raziskali njeno potencialno uporabo v regenerativni medicini, smo naredili sistematičen pregled literature. Iskanje smo opravili na treh podatkovnih bazah PubMed, Web of Science in Scopus, dodatno pa smo vključili tudi članke pridobljene s pregledom literature že vključenih virov. Skupno smo vključili 46 člankov, izmed katerih je bilo 32 raziskovalnih, na katere smo se podrobneje osredotočili. Članke smo razdelili po organskih sistemih in ugotovili, da se rezultati zelo razlikujejo. Zgodnje raziskave so se osredotočale predvsem na mehanizme delovanja MSC, pri čemer so nekatere dokazale odsotnost fuzije, druge pa so dokazale redko spontano fuzijo. Z namenom povečanja pogostosti fuzije so raziskovalci uporabili dodatne metode kot sta uporaba virusov ali polietilen glikola. Tako sprožena fuzija lahko postane pomemben mehanizem s katerim pridobimo hibridne oz. fuzijske celice. Koncept so raziskovalci že izkoristili pri razvoju novih terapij na področju zdravljenja srčno-žilnih bolezni, Duchennove mišične distrofije in diabetesa. Kljub dokazanem potencialu fuzije MSC s tkivno specifičnimi celicami to področje še vedno ostaja premalo raziskano in bodo potrebne nadaljnje študije, zlasti glede dolgoročnih učinkov terapij s hibridnimi celicami.

Title:Mesenchymal stem/stromal cell fusion in regenerative medicine: a systematic literature review
Abstract:
Mesenchymal stem/stromal cells (MSCs) are the subject of numerous studies in the field of regenerative medicine. They represent a unique type of cells that can be isolated from various tissues of adult individuals, most commonly from bone marrow or adipose tissue, though there is a growing interest in using less invasive sources, such as dental pulp, umbilical cord, and others. They possess the capacity for multidirectional differentiation into a variety of tissues, a feature utilized in regenerative medicine to develop new cell-based therapies for numerous diseases. These therapies have been shown to be safe and are associated with very few adverse effects. They can act through several mechanisms, such as transdifferentiation, mitochondrial transfer, paracrine signaling, secretion of extracellular vesicles and cell fusion, which is the focus of our thesis. Cell fusion can be either a physiological or pathological process and plays a role in for example, embryonic development and the body's immune response. More recent research has also demonstrated fusion between stem cells and tissue specific cells, which offers new possibilities for therapeutic applications. To evaluate the importance of cell fusion as one of the mechanisms of action of MSCs and to explore its potential application in regenerative medicine, we conducted a systematic literature review. The search was performed using three databases: PubMed, Web of Science and Scopus. Additionally, we included articles identified through screening of references of already included sources. A total of 46 articles were included, of which 32 were research articles that were the focus of this thesis. The studies were categorized by organ systems, and we found that the results varied greatly. Early research focused mainly on the mechanisms underlying MSC action. Some demonstrated an absence of fusion, while others reported fusion to be a rare spontaneous event. To increase the frequency of fusion, researchers used additional techniques such as viral methods or polyethylene glycol. Such induced fusion may become an important mechanism for generating hybrid or fused cells. The concept has already been applied in the development of novel therapies targeting cardiovascular diseases, Duchenne muscular dystrophy and diabetes. Despite the proven potential of MSC fusion, this field remains under-researched and further studies, on the long-term effects of hybridoma therapies, will be needed.
