Povezanost dosežkov na SAGE testu z dosežki na testih izvršilnih sposobnosti : magistrsko delo

Abstract
Posamezniki z blago kognitivno motnjo (BKM) lahko živijo brez večjih težav in opravljajo vsakodnevne naloge, pri polovici pa se v roku petih let razvije resna kognitivna motnja. Zgodnje odkrivanje BKM je zato nujno, saj lahko hitreje načrtujemo zgodnjo specialistično obravnavo z namenom zgodnjega odkrivanja resnejših bolezenskih stanj. Zato so potrebni presejalni testi, ki so učinkoviti, lahko izvedljivi in dostopni. SAGE (angl. Self-administered gerocognitive examination) test je samoadministrativni presejalni test za ugotavljanje BKM, ki ne zahteva prisotnosti izvajalca ali posebne opreme in je primerljiv z drugimi presejalnimi testi kognitivnih sposobnosti. Trenutne raziskave kažejo, da je SAGE test praktično in učinkovito orodje za množično in hitro testiranje blage na BKM, vendar pa je v literaturi moč zaslediti le eno raziskavo, ki je preučevala povezanost med splošnimi dosežki na SAGE testu in dosežki na drugih testih izvršilnih sposobnosti. Hkrati nismo našli študij, ki bi podrobneje raziskovale, kako so dosežki na posameznih domenah SAGE testa povezani z dosežki na testih izvršilnih procesov, kot so delovni spomin, inhibicija in kognitivna fleksibilnost. V naši raziskavi smo se zato osredotočili na preverjanje povezanosti dosežkov na SAGE testu z dosežki na drugih testih izvršilnih procesov (IP) na starostno heterogenem in zdravem vzorcu posameznikov. Natančneje, zanimalo nas je, kako se splošni dosežki na SAGE testu in dosežki na posameznih domenah SAGE testa povezujejo z dosežki na testih delovnega spomina, inhibicije in kognitivne fleksibilnosti. Dodatno nas je zanimalo, kako se dosežki na SAGE testu povezujejo s starostjo in izobrazbo udeležencev. V raziskavo smo vključili 107 posameznikov, 39 moškega in 68 ženskega spola, starih med 20 in 86 let. Vsaka oseba je rešila SAGE test za merjenje splošnih kognitivnih sposobnosti, Test verbalnega delovnega spomina za merjenje delovnega spomina, Stroopovo nalogo za merjenje sposobnosti inhibicije, Test sledenja in Test preklapljanja za oceno kognitivne fleksibilnosti ter Test besedne fluentnosti za merjenje kognitivne fleksibilnosti in fluentnosti. Rezultati so pokazali, da starost in izobrazba ob upoštevanju rezultatov testov IP nista pomembna napovednika skupnega dosežka na SAGE testu. Nadalje smo ugotovili, da samo mera preklapljanja, izračunana iz dosežkov na testih sledenja, ob upoštevanju ostalih testov IP, pomembno napoveduje skupni dosežek na SAGE testu. Ugotovili smo tudi, da se dosežki na Testih verbalnega delovnega spomina nizko, a značilno pozitivno povezujejo z dosežkom na domeni spomina na SAGE testu ter da se dosežki na Testu sledenja nizko, a značilno negativno povezujejo z dosežki na testu sledenja na SAGE testu. To magistrsko delo lahko pomembno prispeva k razvoju novih in adaptaciji obstoječih presejalnih testov za odkrivanje BKM, ki imajo pomembno prednost samoadministrativnosti pred doslej uveljavljenimi testi.

Title:Relationship between SAGE Test Scores and Scores on Executive Function Tests
Abstract:
Most individuals with mild cognitive impairment (MCI) can live without significant difficulties and perform daily tasks, but in half of individuals, serious cognitive impairment can develop within five years. Early detection of MCI is therefore crucial, as it allows for the earlier planning of specialized treatment aimed at diagnosing more serious conditions. Screening tests that are efficient, feasible, and accessible are needed for this purpose. The Self-Administered Gerocognitive Examination (SAGE) test is a self-administered screening test for detecting MCI, which does not require the presence of a trained professional or any special equipment, and is comparable to other screening tests. Current research suggests that the SAGE test is a practical and effective tool for large-scale, rapid testing for mild MCI. In the literature, we found only one study that examined the correlation between overall performance on the SAGE test and performance on other executive function (EF) tests. Moreover, no studies have been found that explore in detail how scores in individual SAGE test domains correlate with performance on tests of executive processes, such as working memory, inhibition, and cognitive flexibility. In our research, we focused on examining the correlation between SAGE test performance and performance on other EF tests in an age-diverse and healthy sample of individuals. Specifically, we aimed to investigate how overall SAGE test scores and scores in individual SAGE domains correlate with performance on tests of working memory, inhibition, and cognitive flexibility. Additionally, we were interested in how SAGE test scores correlate with participants' age and education level. We included 107 individuals, 39 males and 68 females, aged between 20 and 86 years. Each participant completed the SAGE test to measure general cognitive abilities, the Verbal Working Memory Test to measure working memory, the Stroop Task to measure inhibition, the Trail Making Test and Task-Switching Test to assess cognitive flexibility, and the Verbal Fluency Test to measure cognitive flexibility and fluency. We found that demographic data (age and education), considering the results of the EF tests, were not significant predictors of overall performance on the SAGE test when other variables were considered. Furthermore, we found that only the measure of cognitive flexibility (Task switching), calculated from the results of the Trail Making Test, significantly predicted overall performance on the SAGE test when considering other EF tests. We also found that performance on the Verbal Working Memory Test was weakly but significantly positively correlated with the memory domain of the SAGE test. Additionally, performance on the Trail Making Test was weakly but significantly negatively correlated with performance on the tracking domain of the SAGE test. This thesis significantly contributes to the development of new and the adaptation of existing screening tests for detecting MCI, which have the important advantage of being self-administered compared to established tests.
