Abstract
Vedno večja prevalenca pridobljenih možganskih poškodb ter motenj, ki okrnejo sposobnost kognitivnega nadzora, je sprožila pozive k večjemu številu ekološko veljavnih nevropsiholških testov. Naloga razvrščanja zdravil je bila zasnovana z mislijo na to idejo, neodvisnost od motorične zmožnosti pa še dodatno razširi možnost njene uporabe. Z navzkrižno validacijo naloge na 883 zdravih udeležencih starih 18–79 let smo ugotovili dvofaktorsko strukturo, ki je ob primerjavi s korelacijami več testov delovnega spomina in izvršilnih funkcij ponudila vsebinsko interpretacijo faktorjev: (1) faktor učinkovitosti kognitivnega nadzora in (2) faktor pravilnosti reševanja naloge. V raziskavo je bilo vključenih tudi 31 udeležencev s pridobljeno možgansko poškodbo, ki so poleg že omenjenih testov rešili tudi klinično uveljavljene teste pozornosti, test verbalnega učenja in spomina ter test izvršilnih funkcij. Individualne primerjave dosežkov kliničnih udeležencev z ujemajočimi kontrolnimi udeleženci (±3 leta odstopanja v številu let izobrazbe in starosti) kažejo na pomembno nižjo učinkovitost kognitivnega nadzora kliničnih udeležencev, vendar ne tudi na manjšo pravilnost reševanja naloge. Tako zdravim kot kliničnim udeležencem je z naraščajočo starostjo upadala učinkovitost kognitivnega nadzora, medtem ko je imela višja izobrazba sicer pozitiven vpliv, ki pa je bil statistično pomemben zgolj pri bolnikih s poškodbo prefrontalnega režnja. Dosežek na faktorju kognitivnega nadzora v 83,4 % primerov pravilno klasificira osebo kot zdravo (učinkovit kognitivni nadzor) ali kognitivno oškodovano (pomembno zmanjšana učinkovitost kognitivnega nadzora), kar kaže na visoko razločevalno zmožnost naloge.

Title:Medication scheduling task: assessing cognitive processes and clinical usefulness in patients with brain injury
Abstract:
The increasing prevalence of acquired brain injury and other disorders that impair cognitive control has prompted calls for more ecologically valid neuropsychological tests. The Medication Scheduling Task was designed with this idea in mind, and its independence from motor ability further extends its applicability. Cross-validation of the task on 883 healthy participants aged 18-79 years found a two-factor structure, which, when compared with correlations of several tests of working memory and executive function, offered a substantive interpretation of the factors: (1) a cognitive control factor and (2) factor of accuracy. The study also included 31 people with acquired brain injury who, in addition to the aforementioned tests, also completed clinically established tests of attention, verbal learning and memory, and executive function. Individual comparisons of the clinical participants' performance with matched control participants (±3 years of variance in years of education and age) indicate significantly lower cognitive control performance of the clinical participants, but not lower accuracy. In both healthy and clinical participants, cognitive control performance declined with increasing age. Higher education had a positive effect, though statistically significant only in patients with prefrontal lobe damage. The score on the cognitive control factor correctly classifies a person as healthy (effective cognitive control) or cognitively impaired (significantly reduced cognitive control) in 83.4% of cases, indicating a high discriminatory ability.
