Pojavnost, tipizacija in klinična slika Enterovirusa D68 v populaciji hospitaliziranih otrok v obdobju med 2014 in 2022

Enterovirus D68 (EV-D68) je respiratorni virus, ki povzroča okužbe zgornjih in spodnjih dihal, lahko pa je povezan tudi z različnimi zapleti osrednjega živčevja, vključno z akutnim ohlapnim mielitisom (AFM). S to raziskavo smo želeli oceniti incidenco, sezonskost in molekularno epidemiologijo pojavljanja EV-D68 pri otrocih v Sloveniji in opisati klinične značilnosti otrok, okuženih z EV-D68 med leti 2014 in 2022. V raziskavo smo vključili otroke, ki so bili hospitalizirani ali ambulantno pregledani v Univerzitetnem kliničnem centru Ljubljana. Enterovirus D68 smo dokazovali s specifičnim rtRT-PCR. Analiza je pokazala bienalni vzorec kroženja EV-D68, z vrhom okužb pozno poleti in zgodaj jeseni v letu 2014 in 2016. Za določitev molekularne epidemiologije smo uporabili postopek sekvenciranja po Sangerju in programsko opremo CLC Main Workbench različice 7 in MEGA X. V Sloveniji sta bila leta 2014 sočasno prisotna podklada B1 in B2 EV-D68, v naslednjih letih pa smo kot edinega dokazali EV-D68, ki pripada podkladu B3. Hude okužbe dihal in potencialni nevrotropizem virusa kaže na to, da je nadzor in spremljanje okužb, ki jih povzroča EV-D68, velikega pomena.

• Title: Incidence, typing and clinical presentation of Enterovirus D68 in the population of hospitalized pediatric patients between 2014 and 2022

Abstract

Enterovirus D68 (EV-D68) is a respiratory virus that causes infections of the upper and lower respiratory tract and can also be associated with various central nervous system complications, including acute flaccid myelitis (AFM). The aim of this study was ro assess the incidence, seasonality, and molecular epidemiology of EV-D68 in children in Slovenia, and to describe the clinical characteristics of children infected with EV-D68 between 2014 and 2022. We included children who were hospitalized or examined as outpatients in the University Medical Center Ljubljana. EV-D68 was detected by specific rtRT-PCR. The analysis revealed a biennial pattern of EV-D68 circulation, with peaks in infections occurring in late summer and early autumn in 2014 and 2016. Sanger sequencing and the CLC Main Workbench version 7 and MEGA X software were used to determine the molecular epidemiology. In Slovenia, both subclades B1 and B2 were present simultaneously in 2014, while in the following years, we only identified EV-D68 belonging to subclade B3. Severe respiratory infections and the potential neurotropism of the virus indicate the importance of monitoring EV-D68.
