Abstract
Uvod: Razcep ustnice in ustnega neba je najpogostejša prirojena anomalija obraza pri otrocih, ki lahko vpliva na videz, govor, sluh, rast, psihosocialno dobro počutje in socialno integracijo (Taib idr., 2015). Pojavi se zaradi pomanjkljivega zapiranja nosnega in maksilarnega izrastka med embrionalnem razvojem, bodisi na eni strani bodisi na obeh straneh. Nebni izrastek, nasprotni izrastek in vomer tj. ena od neparnih obraznih kosti lobanje, ki se nahaja na srednji sagitalni liniji, se ne združijo v ustih, kar lahko povzroči razvojno anomalijo. Ta v razvoju ustvari odprtino na zgornji ustnici, mehkem in trdem nebu. Razpoka loči zgornjo ustnico na dva enaka dela, ker prečka pod levo ali desno nosnico (Čelešnik, 1963). Namen: Namen diplomskega dela je predstaviti prirojeno anomalijo obraza – shiza in opis ploščice ter izdelava RME z akrilatnimi platoji in kljukicami za obrazno masko. Cilji so opis prirojene anomalije obraza ter ploščice in izdelava RME s platoji in kljukicami za obrazno masko, opis njegove funkcije in sestave. Prav tako bodo opisani materiali uporabljeni za izdelavo, potek seznanjenja otroka s ploščico in napredek zdravljenja. Metode dela: Uporabljene metode v raziskavi vključujejo analizo strokovne in znanstvene literature, pregled zbornikov ter revij v slovenskem in angleškem jeziku, ki obravnavajo področje čeljustne in zobne ortopedije. Literaturo sem preučevala v podatkovnih bazah PubMed, Cochrane, Cinahl, ScienceDirect ter COBISS za dostop literature v knjižicah. Pri iskanju ustreznih virov sem uporabila ključne besedne zveze v slovenščini ter angleščini, kot so razvojne nepravilnosti, cleft lip and palate, shiza ipd. Poleg tega sem s pridobljenim znanjem iz laboratorijskih vaj in literature opisala ortodontski pripomoček akrilatno ploščico in izdelala RME z akrilatnimi platoji in kljukico za obrazno masko. Vsako posamezno fazo izdelave sem dokumentirala s fotografijami ter natančno predstavila v okviru svojega diplomskega dela. Rezultati: V diplomskem delu so podrobno opisane in z uporabo slikovnega gradiva prikazane faze dela akrilatne ploščice in RME z akrilatnimi platoji in kljukicami za obrazno masko. Razprava in zaključek: S pravočasno in primerno oskrbo se lahko shiza uspešno odpravi. Omenjen aparat zgornjo čeljust širi in kasneje s pomočjo obrazne maske potiska naprej. Title:Manifacturing orthopedic plate for cleft lip and palate : diploma work
Abstract:
Introduction: Cleft lip and palate is the most common congenital facial anomaly in children, which can affect appearance, speech, hearing, growth, psychosocial well-being and social integration (Taib et al., 2015). It occurs due to insufficient closure of the nasolabial and maxillary processes during embryonic development, either on one side or on both sides. The palatal process, the contralateral process and the vomer i.e. one of the unpaired facial bones of the skull, located on the mid-sagittal line, fail to fuse in the mouth, which can result in a developmental anomaly. This creates an opening in the upper lip, soft palate and hard palate during development. The cleft separates the upper lip into two equal parts because it crosses under the left or right nostril (Čelešnik, 1963). Purpose: The purpose of the dissertation is to present a congenital facial anomaly - schisis and the fabrication of a plate and RME with acrylic plates and hooks for a face mask. The objectives are to describe the congenital facial anomaly, to produce a plate and RME with plates and hooks for the face mask, to describe its function and composition. The materials used for the fabrication, the process of familiarisation of the child with the plate and the progress of the treatment will also be described. Methods: The methods used in the study include an analysis of professional and scientific literature, a review of proceedings and journals in the Slovene and English languages, dealing with the field of maxillofacial and dental orthopaedics. The literature search was conducted in PubMed, Cochrane, Cinahl, ScienceDirect and COBISS databases for accessing literature in library. In searching for relevant sources, I used key phrases in Slovene and English, such as developmental anomalies, cleft lip and palate, schiza, etc. In addition, I described an orthodontic appliance, an acrylic plate and made RME with plates and hooks for the face mask based on the knowledge gained from the literature and the practice at the faculty. I documented each individual stage of fabrication with photographs and presented it in detail in my thesis. Results: In the thesis, the stages of the work of the acrylate plate and the RME with acrylate faces and face mask pegs are described in detail and illustrated using pictorial material. Discussion and conclusion: With timely and appropriate care, the schism can be successfully corrected. This appliance widens the upper jaw and later pushes it forward with the help of a face mask.
