Abstract
Kronična bolečina v križu (KBK) ima znaten vpliv na posameznike in zdravstvene sisteme po vsem svetu, saj jo zaznamuje vztrajna bolečina in nelagodje. Njena večfaktorska narava zahteva multidisciplinarno obravnavo. Ta raziskava primerja učinkovitost terapevtske vadbe (TV) in multidisciplinarne biopsihosocialne rehabilitacije (MBR) pri obvladovanju KBK. Sistematični pregled literature v več podatkovnih bazah, vključno z MEDLINE, PEDro, Embase, CINAHL, CENTRAL in PsycINFO, je bil usmerjen v ocenjevanje učinkov TV in MBR na KBK. V skladu z navodili PRISMA se je raziskava osredotočala na integrativne pristope TV in MBR ter njihove oblike zdravljenja, pri čemer so kot kontrolne obravnave služile običajna obravnava in minimalne intervencije. Vključene so bile randomizirane kontrolirane raziskave raziskave, pri čemer sta bila primarna izida bolečina in nemožnost. V statistični analizi je bila uporabljena tako meta-analiza kot mrežna meta-analiza za ocenjevanje izidov v različnih časovnih okvirih, z natančnim raziskovanjem heterogenosti ter dodatnimi analizami podskupin in meta-regresije. Raziskava ponuja vpogled v kompleksno naravo intervencij pri KBK, čeprav ne določa nedvoumno superiorne oblike zdravljenja. Poudarja učinkovitost TV in MBR v primerjavi z običajno oskrbo in minimalnimi intervencijami, pri čemer ima MBR rahlo prednost pri izboljšanju izidov in visoko heterogenost opaženo med zdravljenji. Ugotovitve nakazujejo, da sta tako TV kot MBR pomembni pri obvladovanju KBK, pri čemer MBR kaže manjšo prednost. Vključitev mrežne meta-analize omogoča bolj končno primerjavo, zagovarja pa personaliziran, integriran pristop zdravljenja. Ta raziskava bogati razumevanje oblik zdravljenja KBK in postavlja temelje za prihodnje raziskave o personaliziranih strategijah zdravljenja.

Title:Meta-Analysis of Exercise Therapy and Multidisciplinary Biopsychosocial Rehabilitation for Chronic Low Back Pain
Abstract:
Chronic low back pain (CLBP) significantly impacts individuals and healthcare systems globally, characterized by persistent pain and discomfort. Its multifactorial nature necessitates multidisciplinary interventions. This study compares the effectiveness of exercise therapy (ET) and multidisciplinary biopsychosocial rehabilitation (MBR) in managing CLBP. A systematic literature review across several databases, including MEDLINE, PEDro, Embase, CINAHL, CENTRAL, and PsycINFO, aimed to evaluate the effects of ET and MBR on CLBP. Adhering to PRISMA guidelines, the study focused on the integrative approaches of ET and MBR and their respective treatment modalities, with usual care and minimal intervention used as control treatments. Randomized controlled trials were included, with pain and disability as primary outcomes. Statistical analyses employed both meta-analyses and network meta-analyses to assess outcomes across different time frames, with thorough exploration of heterogeneity and additional subgroup and meta-regression analyses. The research offers insights into the complex nature of CLBP interventions, though it does not establish a definitively superior treatment modality. It highlights the effectiveness of ET and MBR over usual care and minimal intervention, with MBR having a slight edge in improving outcomes and high heterogeneity observed between treatments. The findings suggest that both ET and MBR are valuable in CLBP management, with MBR showing a minor advantage. The inclusion of network meta-analysis provides a more definitive comparison, advocating for a personalized, integrated treatment approach. This research enriches the understanding of CLBP treatment modalities, setting a foundation for future investigations into personalized care strategies.
