Abstract
Uvod: Način prve obravnave novorojenčka v porodni sobi ter posegi in postopki, ki jih izvajajo babice, v veliki meri vplivajo na mater in novorojenčka. Način obravnave vpliva na navezovanje novorojenčka na mater, na njegove odzive na okolje in stopnjo stresa, ki ga doživlja. S svojim ravnanjem lahko babica v veliki meri prepreči resnejše zaplete in novorojenčku olajša prehod na zunajmaternično okolje. V ta namen je svetovna zdravstvena organizacija izdala splošne smernice, ki temeljijo na preverjenih podatkih ter so podane v dobrobit matere in novorojenčka. Namen: Raziskati želimo način prve babiške obravnave novorojenčka, raziskati odstopanja od splošnih smernic ter poiskati razlike v prvi obravnavi zdravega novorojenčka v porodni sobi pri delu babic v slovenskih porodnišnicah. Metode dela: V diplomskem delu je bila uporabljena deskriptivna ter kavzalno neeksperimentalna raziskovalna metoda. Pri pregledu literature smo uporabili strokovno in znanstveno literaturo, kot tudi smernice Svetovne zdravstvene organizacije ter razširjeni strokovni kolegij za pediatrijo in oftalmologijo. Raziskavo smo izvedli s pomočjo anketnega vprašalnika v programu 1KA na namenskem vzorcu babic in babičarjev, zaposlenih v porodnem bloku. Rezultati: Pravilno rešenih anket s strani babic in babičarjev zaposlenih v porodnem bloku je bilo 102. Najslabši odziv je bil iz porodnišnice Brežice (1%), najboljši pa iz porodnišnice Ljubljana (14%). Način in vrstni red prve obravnava novorojenčka se razlikuje med porodnišnicami, kot tudi znotraj posameznih porodnišnic. Odstopanja od smernic svetovne zdravstvene organizacije so predvsem pri kožnem stiku, očesni profilaksi in oskrbi popkovnega krna. Razprava in zaključek: Glede na rezultate bi bilo smiselno ponovno preučiti splošne smernice in slediti spremembam. Prakso bi bilo smiselno poenotiti znotraj porodnišnic, kot tudi na ravni Slovenije. Vpogled v trenutno stanje nam lahko pomaga pri izboljšanju načina dela. Naš cilj ni bil zgolj izpostaviti odstopanja prakse od veljavnih splošnih smernic, temveč tudi izboljšati pogoje za matere in novorojenčke.

Title:The difference in delivery room newborn care in Slovenian maternity hospitals : diploma work
Abstract:
Introduction: First newborn care in the delivery room provided by midwives can have a major impact on the wellbeing of the mother and her newborn. It can affect their interaction and bonding, subsequently altering newborn's responses to the environment. This can lead to increased stress levels. With her actions, the midwife can prevent serious complications and facilitates the transition of the newborn to extrauterine life. To that effect the World Health Organization has released guidelines that are based on verified data. Purpose: The purpuse is to assess first care of a healthy newborn in the delivery room provided by midwives between Slovenian hospitals, examine deviations from general guidelines and identify differences in said care. Methods: A discriptive and causally non-experimental survey method was used. For literature review we used scientific literature as well as World Health Organization and Extended council of experts for pediatrics and oftalmology guideliness. The survey was conducted using a questionnaire in the 1KA programme on a sample of midwives employed in the delivery room. Results: There were 102 correctly answered surveys by midwives employed in the delivery room. The worst response was from the Brežice maternity hospital (1%), and the best from the Ljubljana maternity hospital (14%). The method and order of the first care of a newborn differs between maternity hospitals, as well as within individual maternity hospitals. Deviations from the World Health Organization guidelines are mainly in skin-to-skin contact, eye prophylaxis and umbilical cord care. Discussion and conclusion: In view of the results, general guidliness should be reviewd. The next logical step would be to standardize the practice within the maternity hospitals. Insight into the current situation can help us improve midwifery practice. Our aim was not only to highlight the deviation of practice from the current general guidelines, but also to improve conditions for mothers and newborns.
