Abstract
Uvod: Kronična obstruktivna pljučna bolezen (KOPB) je tretji vodilni vzrok smrti na svetu. Gre za kombinacijo trajnega zoženja dihalnih poti in kroničnega vnetja pljuč, kar privede do motene funkcije pljuč in propadanja pljučnega tkiva. Najpogostejši simptomi so kronični kašelj, oteženo dihanje, občutek stiskanja v prsih in utrujenost, kar omejuje izvajanje vsakodnevnih aktivnosti, prispeva k zmanjšanju telesne dejavnosti in kakovosti življenja. Poleg zdravil se običajno zdravi s programi pljučne rehabilitacije. Tai chi in tai chi qigong izhajata iz tradicionalnih kitajskih borilnih veščin. Tai chi združuje nežno, harmonično gibanje celotnega telesa z osredotočenostjo na zavestno dihanje, sprostitev in meditacijo. Zaradi številnih pozitivnih učinkov na telo in nizke do zmerne intenzitete se zdi primerna oblika vadbe za bolnike s KOPB. Namen: Namen diplomskega dela je na podlagi pregleda strokovne literature ugotoviti vpliv vadbe tai chi na kakovost življenja, telesno zmogljivost in pljučno funkcijo pri bolnikih s kronično obstruktivno pljučno boleznijo. Metode dela: V pregled je bila vključena literatura, objavljena v angleškem jeziku iz elektronskih podatkovnih zbirk PubMed in PEDro. Rezultati: V pregled literature je bilo vključenih 7 raziskav. Izbrane raziskave so vadbo tai chi primerjale še z drugimi vadbenimi programi in kontrolno skupino. V vseh raziskavah, v katerih so ocenjevali 6-minutni test hoje, občutek dispneje in kakovost življenja z respiratornim vprašalnikom St. George, so ugotovili statistično značilno izboljšanje v skupini tai chi. V treh od šestih raziskav pa je prišlo do statistično značilnega izboljšanja pljučnih volumnov v skupini tai chi. Razprava in zaključek: Na podlagi pregledanih raziskav bi lahko zaključili, da ima vadba tai chi pozitiven vpliv na izboljšanje telesne zmogljivosti, splošnega počutja in kakovosti življenja bolnikov s KOPB. Z doslednim izvajanjem vadbe tai chi bi potencialno lahko zmanjšali občutek dispneje. Nekoliko manj dokazov pa je, da vadba tai chi vpliva na pljučno funkcijo in število akutnih poslabšanj. Vadba tai chi se zdi varna, primerna in zanimiva oblika vadbe za dopolnitev pljučne rehabilitacije, saj vsebuje tudi elemente meditacije in sproščanja, primerna se zdi tudi kot vzdrževalni program po zaključeni pljučni rehabilitaciji pri bolnikih s KOPB.

Title:The impact of tai chi exercise on quality of life and physical performance in patients with chronic obstructive pulmonary disease – literature review : diploma work
Abstract:
Introduction: Chronic obstructive pulmonary disease (COPD) is the third leading cause of death worldwide. It is characterized as persistent airway obstruction and chronic lung inflammation, leading to lung tissue damage and impaired lung function. Common symptoms include chronic cough, difficulty breathing, chest tightness and fatigue, which limits the performance of daily activities, contributes to decrease in physical activity and quality of life. Besides medications, it is commonly managed with pulmonary rehabilitation programs. Tai Chi originates from traditional Chinese martial arts and combines gentle, slow, harmonious movements with focus on breathing, relaxation, and meditation. Due to its positive effects and low to moderate intensity, it appears a suitable form of exercise for patients with COPD. Purpose: The purpose of this review was to determine the impact of Tai Chi exercise on the quality of life, physical performance and lung function in patients with COPD. Methods: The review included literature published in English from electronic databases PubMed and PEDro. Results: Seven studies were included in the literature review. They compared Tai Chi with other types of exercise and control group. In all studies, which evaluated the 6-minute walk test, dyspnoea sensation and quality of life using the St. George's Respiratory Questionnaire, statistically significant improvements were found in the Tai Chi group. Three out of six studies reported statistically significant improvements in lung volumes in the Tai Chi group. Discussion and Conclusion: Based on our review, it can be concluded that Tai Chi has positive impact on improving physical performance, overall well-being, and quality of life in patients with COPD. Consistent implementation of Tai Chi exercise could potentially reduce the sensation of dyspnoea. There is somewhat less evidence that Tai Chi exercise affects lung function and the number of acute exacerbations. Tai Chi exercise appears to be a safe, suitable, and engaging form of exercise to complement pulmonary rehabilitation, as it also includes elements of meditation and relaxation or as a maintenance program following completion of pulmonary rehabilitation in patients with COPD.
