Abstract
Uvod: Antifosfolipidni sindrom je eden od pomembnejših razlogov za zaplete v nosečnosti. Je avtoimunska bolezen, ki se lahko pojavi kot samostojna bolezen ali pa sekundarno pri drugi avtoimunski bolezni, kot je sistemski lupus eritematozus. Avtoimunske bolezni večinoma prizadenejo ženske. Ker se pogosto pojavijo v reproduktivni dobi, lahko vplivajo na plodnost in nosečnost ter živorojenost otrok. Namen: Namen diplomskega dela je ugotoviti, kako bi lahko delo medicinskih sester pripomoglo h kakovostnejši obravnavi nosečnic z antifosfolipidnim sindromom. Metode dela: Uporabili smo deskriptivno metodo dela. Pregledali smo slovensko in tujo strokovno ter znanstveno literaturo. V bibliografskih bazah COBIB.SI, CINAHL Ultimate, Medline, PubMed ter Google Scholar smo z uporabo Boolovih operaterjev iskali po naslednjih ključnih besedah in njihovih sopomenkah: antiphospholipid syndrome, pregnancy complication, nursing in management. Zajemali smo literaturo, ki ni bila starejša od 10 let. V analizo smo vključili 8 člankov, ki smo jih glede na dokazno moč ovrednotili po osemstopenjski lestvici. Rezultati: S pregledom literature smo identificirali tri glavne teme: znanje medicinskih sester, znanje nosečnic z antifosfolipidnim sindromom in multidisciplinarni pristop pri obravnavi nosečnic z antifosfolipidnim sindromom. Razprava in zaključek: Ugotovili smo, da je na področju obravnave nosečnic z antifosfolipidnim sindromom zelo malo raziskav o vlogi medicinskih sester. Bolj je raziskano znanje o antifosfolipidnem sindromu tako pri medicinskih sestrah kot tudi pri nosečnicah s to diagnozo. Z analizo člankov smo pokazali, da morajo imeti medicinske sestre dovolj strokovnega znanja, da lahko strokovno opravljajo zdravstveno vzgojo, hkrati pa se morajo vključevati v multidisciplinarni tim za vodenje pri obravnavi pacientk z antifosfolipidnim sindromom. Pomembna je tudi vpetost medicinskih sester v zgodnejše diagnosticiranje in zdravljenje. Vključitev medicinskih sester v obravnavo nosečnic z antifosfolipidnim sindromom v revmatoloških ambulantah bi bila nujna, saj to lahko izboljša obravnavo pacientk in predvsem pozitivno vpliva na končni izid nosečnosti.

Title:The management of pregnant women with antiphospholipid syndrome : diploma work
Abstract:
Introduction: Antiphospholipid syndrome is one of the most important causes of complications in pregnancy. It is an autoimmune disease that can occur as a stand-alone condition or secondary to another autoimmune disease, such as systemic lupus erythematosus. Autoimmune diseases mainly affect women. Because they often occur at reproductive age, they can influence the woman's fertility and change the course of the pregnancy. Purpose: The aim of this thesis is to explore how the work of nurses could contribute to the better management of pregnant women with antiphospholipid syndrome. Methods: We used a descriptive method and conducted a literature review. We reviewed Slovenian and foreign professional and scientific literature. The bibliographic databases COBIB.SI, CINAHL Ultimate, Medline, PubMed and Google Scholar were searched using Boolean operators for the following keywords and synonyms: antiphospholipid syndrome, pregnancy complications, nursing and management. We included literature that was less than 10 years old. 8 articles made it to the analysis and were assessed for strength of evidence on an eight-point scale. Results: The literature review identified three main themes: nurses' knowledge, knowledge of pregnant women with antiphospholipid syndrome and a multidisciplinary approach to the management of pregnant women with antiphospholipid syndrome. Discussion and conclusion: We found that there is very little research in terms of the role of nurses in the management of pregnant women with antiphospholipid syndrome. Knowledge about antiphospholipid syndrome in nurses and pregnant women has been studied more frequently. The results revealed that nurses need to have adequate knowledge to be able to carry out health education in a professional manner, but they also must be involved in multidisciplinary patient management. It is also important that the nurses are engaged in achieving earlier diagnosis and treatment. Therefore, including nurses in the management of pregnant women with antiphospholipid syndrome in the rheumatology outpatient clinic could be beneficial, as this may improve the management of the patients and, above all, have a positive impact on the outcome of the pregnancy.
