Abstract
Uvod: Lateralni epikondilitis komolca je najpogostejši vzrok za bolečino in disfunkcijo komolca. V splošni populaciji se pojavlja s prevalenco 1–3 %, najpogosteje pri ljudeh med 30. in 50. letom starosti. Lateralni epikondilitis komolca je pogostejši pri ljudeh, ki opravljajo poklic, ki zahteva ponavljajoče se gibe ekstenzije zapestja. Glavni simptom je bolečina, ki se pojavi na lateralnem epikondilu nadlahtnice, zlasti ob prijemalnih aktivnostih roke. LEK običajno izzveni sam in večino bolnikov ne poišče zdravljenja. Bolniki, pri katerih simptomi ne izzvenijo, potrebujejo nadaljnje konzervativno ali kirurško zdravljenje. Prejšnje raziskave kažejo na učinkovitost ekscentrične vadbe pri zdravljenju ostalih tendinopatij. Namen: S pomočjo strokovne literature analizirati dokaze o učinkih ekscentrične vadbe pri ljudeh z lateralnim epikondilitisom komolca. Metode dela: Diplomska naloga temelji na pregledu in analizi strokovne in znanstvene literature v angleškem jeziku. Vire smo iskali na podatkovnih zbirkah PEDro, PubMed, ScienceDirect in Springer Link. Uporabljene so bile naslednje ključne besede in njihove kombinacije: lateral epicondylitis and exercise, lateral epicondylitis and eccentric exercise, tennis elbow, lateral elbow tendinopathy, lateral epicondylosis. Rezultati: V podrobno analizo je bilo vključenih šest raziskav. Rezultati raziskav o učinku ekscentrične vadbe na funkcijo, zmanjšanje intenzitete bolečine ter povečanju mišične zmogljivosti so si bili med seboj podobni. V vseh vključenih raziskavah so imele skupine z ekscentrično vadbo boljše rezultate v primerjavi s primerjalno ali kontrolno skupino. Razprava in zaključek: Rezultati so pokazali, da je smiselno vključiti ekscentrično vadbo v fizioterapevtsko obravnavo lateralnega epikondilitisa komolca kot samostojno ali kot kombinirano terapijo. Potrebne so nadaljnje raziskave o učinkovitosti ekscentrične vadbe na lateralni epikondilitis komolca, kjer bi avtorji opazovali tudi dolgoročne učinke vadbe ter v raziskave vključili kontrolne skupine, pri katerih bi opazovali naraven potek zdravljenja.

Title:Effects of eccentric exercise training in patients with lateral epicondylitis of the elbow - literature review : diploma work
Abstract:
Introduction: Lateral epicondylitis of the elbow is the most common cause of elbow pain and dysfunction. Its prevalence in general population is 1–3 % and most often occurs in people between 30 and 50 years of age. Lateral epicondylitis of the elbow is more common in people who work in occupations with repetitive wrist extension movements. The main symptom is pain on the lateral epicondyle of the humerus, especially during grasping activities. It is generally self-limiting, and most patients do not seek treatment. Patients whose symptoms do not disappear need further conservative or surgical treatment. Previous research shows the effectiveness of eccentric exercise in the treatment of other tendinopathies. Purpose: purpose of this thesis was to investigate the effect of eccentric exercises in people with lateral epicondylitis. Methods: we searched literature in the databases PEDro, PubMed, ScienceDirect and Springer Link. The following keywords and their combinations were used: lateral epicondylitis and exercise, lateral epicondylitis and eccentric exercise, tennis elbow, lateral elbow tendinopathy, lateral epicondylosis. Results: Six studies were included and analysed in detail. The main findings on to the efficiency of eccentric exercise to increase in muscle strengh and patient function, and reduce pain intensity were similar across the studies. In all included studies, eccentric exercise group performed better than either a comparison or control group. Discussion and conclusion: the results suggest of inclusion of the eccentric exercise in physiotherapy of patients with lateral epicondylitis of the elbow is effective, either as endependent technique or in combination with other therapies. Further research on the effectiveness of eccentric exercise on lateral epicondylitis should evaluate also, long-term effects of the eccentric exercise and include control without any treatment.
