Depresija in nekateri drugi dejavniki tveganja v populaciji ogroženih za srčno-žilne bolezni

Abstract
V Sloveniji se pod koordinacijo Nacionalnega inštituta za javno zdravje v ambulantah družinske medicine izvaja Program integrirane preventive kroničnih nenalezljivih bolezni z namenom zgodnjega odkrivanja oseb z biološkimi in vedenjskimi dejavniki tveganja za razvoj kroničnih nenalezljivih bolezni (in visoko ogroženih posameznikov) ter omogočanja pravočasne in strokovno ustrezne obravnave. Med najbolj pogoste kronične nenalezljive bolezni, ki se povezujejo tudi s povečanim tveganjem za depresijo, so srčno-žilne bolezni. Na podatkih 33.457 oseb (15.262 moških), s povprečno starostjo 57 let (SD = 13,3), smo raziskali vpliv depresije in petih drugih dejavnikov tveganja (skrb za zdravje, tvegano pitje, telesna dejavnost, ogroženost zaradi stresa, socialne determinante) na tveganje za razvoj srčno-žilnih bolezni. Rezultati naše raziskave so pokazali, da je med bolj ogroženimi za srčno-žilne bolezni več posameznikov z bolj izraženo depresivno simptomatiko, ki manj tvegano pijejo, so bolj pod stresom, se udeležujejo preventivnih programov in so srednje izobraženi ter imajo povprečen zaposlitveni status in finance. Telesna dejavnost ni imela napovednega vpliva. Rezultate povezujemo z učinkovitostjo morebitnih svetovanj ob predhodnih preventivnih pregledih. V primeru ponovitve raziskave bi bilo treba upoštevati trenutne standarde obravnave srčno-žilne ogroženosti in pridobiti bolj popolno bazo podatkov.

Title:Depression and some other risk factors in population at risk for cardiovascular diseases
Abstract:
In Slovenia, under the coordination of the National Institute of Public Health, the Integrated Prevention of Chronic Non-Communicable Diseases Programme is being implemented in family medicine clinics with the aim of early detection of people with biological and behavioural risk factors for the development of chronic non-communicable diseases (and high-risk individuals), and to provide timely and professionally appropriate treatment. Cardiovascular diseases are among the most common chronic non-communicable diseases, which are also associated with an increased risk of depression. Using data from 33,457 individuals (15,262 men), with a mean age of 57 years (SD = 13.3), we investigated the impact of depression and five other risk factors (health concerns, risky drinking, physical activity, stress, social determinants) on the risk of developing cardiovascular disease. The results of our study showed that individuals at higher risk of cardiovascular disease are more likely to have depressive symptoms, drink less, be more stressed, participate in prevention programmes, have a medium level of education, and have average employment status and finances. Physical activity had no predictive effect. The results are associated with the effectiveness of any counselling given at previous preventive check-ups. If the study were to be replicated, current standards of cardiovascular risk management should be taken into account and a more complete database should be obtained.
