Abstract
Uvod: Amiotrofična lateralna skleroza je neozdravljiva progresivna nevrodegenerativna bolezen, za katero je značilna progresivna izguba motoričnih funkcij s spastičnostjo, mišično oslabelostjo, atrofijo ter oslabljeno ali pretirano živahnostjo kitnih refleksov. V veliki meri je prizadeta tudi funkcija zgornjega uda. Namen: Z meritvami zmogljivosti cilindričnega in pincetnega prijema roke smo želeli ugotoviti, ali se zmogljivost bolnikov razlikuje od zdravih posameznikov. Zanimalo nas je tudi, ali so bolniki z amiotrofično lateralno sklerozo dovzetnejši za nastanek izvedbene utrudljivosti pri desetkratnem izvajanju testa devetih zatičev ter ali je pojav izvedbene utrudljivosti povezan z zmanjšano mišično zmogljivostjo cilindričnega in pincetnega prijema roke. Metode dela: V raziskavi je sodelovalo 24 odraslih bolnikov z amiotrofično lateralno sklerozo ter 15 zdravih posameznikov s podobnimi demografskimi podatki. Vsi so za namen testiranja zmogljivosti cilindričnega in pincetnega prijema opravili meritev s hidravličnim dinamometrom ter deset neprekinjenih ponovitev testa devetih zatičev. Rezultati: Pri bolnikih z amiotrofično lateralno sklerozo smo ugotovili manjšo zmogljivost cilindričnega in pincetnega prijema ter daljše absolutne čase izvajanja testa devetih zatičev kot pri zdravih posameznikih. Prav tako smo dokazali povezanost izvedbene utrudljivosti z zmanjšano zmogljivostjo obeh prijemov. Razprava in zaključek: Bolniki z amiotrofično lateralno sklerozo imajo zmanjšano zmogljivost prijemov roke in so dovzetnejši za pojav izvedbene utrudljivosti. Oboje pomembno vpliva na njihovo kakovost življenja, zato bi jima v prihodnosti morali posvečati več pozornosti, prav tako bi ju bilo smiselno upoštevati pri načrtovanju fizioterapevtske obravnave.

Title:Cylindrical hand grip strength, pinch grip strength and performance fatigue at patients with amyotrophic lateral sclerosis : diploma work
Abstract:
Introduction: Amyotrophic lateral sclerosis is an incurable progressive neurodegenerative disease, characterized by progressive motor function loss with spasticity, muscle weakness, atrophy and hypo- or hyperreflexia. It largely affects the function of the upper limb. Purpose: We aimed to determine whether patients with amyotrophic lateral sclerosis have a decreased muscle strength of cylindrical hand grip and pincer grasp in comparison to healthy individuals. We were also interested in whether patients with amyotrophic lateral sclerosis are more susceptible to performance fatigue at performing ten repetitions of the nine-hole peg test, and if performance fatigue and decreased muscle strength show a significant correlation. Methods: The study included 24 adult patients with amyotrophic lateral sclerosis and 15 healthy individuals with similar demographics. Each of them performed cylindrical hand grip and pincer grasp strength testing, followed by ten continuous repetitions of the nine-hole peg test. Results: We found a significantly decreased muscle strength of both grips and significantly longer absolute durations of the nine-hole peg test at patients with amyotrophic lateral sclerosis. Additionally, we found a correlation between performance fatigue and a decreased muscle strength. Discussion and conclusion: Patients with amyotrophic lateral sclerosis have a decreased grip strength and are more susceptible to performance fatigue than healthy individuals. Both impact the quality of patient’s life significantly; therefore, future researches should focus on their influence and management and we should also consider them at planning the physiotherapy treatment.
