Primerjava koncentracij vitamina D in vezalnega proteina za vitamin D v serumih premenopavzalnih in postmenopavzalnih žensk

Abstract
Premenopavzalno in postmenopavzalno obdobje zaznamuje značilno spreminjanje in upadanje koncentracij estradiola, ki ključno vplivajo na razvoj bolezni in kakovost življenja. K temu prispeva tudi sočasna nezadostna preskrba z vitaminom D, ki je pri starejši ženski populaciji pogosta. Pri preprečevanju prehitrega izgubljanja zalog vitamina D v telesu ima pomembno vlogo vezalni protein za vitamin D (VDBP), katerega sinteza pa je močno pod vplivom estradiola. Namen magistrske naloge je bil ugotoviti, kakšne so koncentracije vitamina D in VDBP pri premenopavzalnih in postmenopavzalnih ženskah. Preiskovankam med 43. in 65. letom starosti smo izmerili koncentracije E2, 25(OH) D in VDBP. Na podlagi koncentracij E2 smo jih razvrstili v premenopavzalno in postmenopavzalno obdobje ter premenopavzalne preiskovanke dodatno opredelili v zgodnjo in pozno stopnjo. Izvedli smo primerjavo koncentracij 25(OH)D in VDBP med opredeljenimi skupinami preiskovank in hkrati ocenili preskrbo z vitaminom D ter ugotavljali povezanost koncentracij E2, 25(OH)D in VDBP znotraj vsake skupine. Razlike v koncentracijah 25(OH)D in VDBP smo dokazali med preiskovankami v zgodnji in pozni stopnji premenopavzalnega obdobja. V skupini premenopavzalnih preiskovank smo dokazali statistično značilno povezanost koncentracij E2 in 25(OH)D ter E2 in VDBP in ugotovili, da se z zniževanjem koncentracij E2, znižujejo tudi koncentracije VDBP, medtem ko koncentracije 25(OH)D naraščajo. Opazili smo, da se od zgodnje stopnje premenopavzalnega obdobja proti pozni stopnji in postmenopavzalnemu obdobju koncentracije VDBP znižujejo in koncentracije 25(OH)D povišujejo. O razlikah v koncentraciji 25(OH)D in VDBP ter o povezanosti E2, 25(OH)D in VDBP znotraj postmenopavzalnega obdobja pa nismo mogli sklepati. Z oceno preskrbe z vitaminom D smo ugotovili, da pri preiskovankah v zgodnji in pozni stopnji premenopavzalnega obdobja prevladuje nezadostna preskrba, pri čemer je pomanjkanje vitamina D pogostejše že v zgodnji stopnji. Nasprotno pa pri postmenopavzalnih ženskah prevladuje zadostna preskrba z vitaminom D. Zaključili smo, da se spremembe v koncentracijah E2, 25(OH)D in VDBP ter nezadostna preskrba z vitaminom D pojavijo že znotraj premenopavzalnega obdobja, zaradi česar bi bilo z vidika zmanjševanja tveganja za razvoj osteoporoze in drugih bolezni po nastopu menopavze potrebno več pozornosti nameniti tudi premenopavzalnim ženskam.

Title:Comparison of vitamin D and vitamin D binding protein serum levels in premenopausal and postmenopausal women
Abstract:
The premenopausal and postmenopausal period is characterised by typical changes and a decrease in the concentrations of estradiol, which have a crucial impact on the development of disease and on the quality of life. This is also affected by the simultaneous insufficient supply of vitamin D, which is frequent in older female population. The vitamin D binding protein (VDBP) has an important role in preventing the too rapid loss of vitamin D storages in the body and its synthesis is strongly affected by estradiol. The purpose of the master’s thesis was to determine the concentrations of vitamin D and VDBP in premenopausal and postmenopausal women. We measured the concentrations of E2, 25(OH)D and VDBP in female subjects, aged between 43 and 65. Based on E2 concentrations, we classified them into the premenopausal and postmenopausal period, while premenopausal subjects were additionally classified into the early and late stage. We also compared the concentrations of 25(OH)D and VDBP among the defined groups of subjects and at the same time assessed their vitamin D supply and identified the connection of E2, 25(OH)D and VDBP concentrations within each group. We proved differences in the concentrations of 25(OH)D and VDBP among subjects in the early and late stage of the premenopausal period. In the group of premenopausal subjects, we proved a statistically typical connection between the concentrations of E2 and 25(OH)D and the concentrations of E2 and VDBP and found that by decreasing the E2 concentrations the concentrations of VDBP are also decreasing, while the concentrations of 25(OH)D are increasing. We found that from the early stage of the premenopausal period towards the late stage and in the postmenopausal period concentrations of VDBP are decreasing and the concentrations of 25(OH)D are increasing. We could not make conclusions about the differences in the concentrations of 25(OH)D and VDBP and about the connection between E2, 25(OH)D and VDBP within the postmenopausal period. By assessing the vitamin D supply, we found that in subjects in the early and late stage of the premenopausal period insufficient supply prevails, whereby vitamin D deficiency is more frequent in the early stage. In contrary, adequate vitamin D supply prevails among postmenopausal women. We concluded that changes in the concentrations of E2, 25(OH)D and VDBP and insufficient vitamin D supply manifest already in the premenopausal period, which is why more attention should be paid to premenopausal women in order to reduce the risk of osteoporosis and other disease after the onset of menopause.
