Abstract
Odpoved prebavil je bolezensko stanje, za katero je značilno zmanjšanje črevesne funkcije pod najmanjšo stopnjo, potrebno za absorpcijo hranil in/ali vode ter elektrolitov, zato je za ohranjanje zdravja in/ali rasti potrebno le-te intravensko nadomeščati. Namen raziskave je uvedba indirektne kalorimetrije kot metode, ki bi omogočila bolj natančno načrtovanje in spremljanje prehranske terapije pri slovenskih bolnikih z odpovedjo prebavil. Naša raziskava je prva raziskava v Sloveniji, kjer smo indirektno kalorimetrijo uporabili kot del standardne ambulantne obravnave bolnikov z odpovedjo prebavil v centru za zdravljenje odpovedi prebavil. V raziskavo smo vključili 20 stabilnih bolnikov z odpovedjo prebavil tipa III (kronična odpoved prebavil), ki prejemajo delno ali popolno parenteralno prehrano, in 20 zdravih prostovoljcev. Bolnike smo razdelili v 3 klinične skupine, glede na tip vnosa energije. Za merjenje porabe energije v mirovanju smo uporabili indirektni kalorimeter Q-NRG® proizvajalca Cosmed. Ugotovili smo, da med stabilnimi bolniki z odpovedjo prebavil tipa III in zdravimi prostovoljci ni statistično značilne razlike v izmerjeni porabi energije v mirovanju (p-vrednost >0,05). Pri bolnikih z odpovedjo prebavil tipa III pa obstaja statistično značilna razlika med izmerjeno porabo energije v mirovanju in dejanskim vnosom energije (p–vrednost = 0,013) in se zelo razlikuje od posameznika do posameznika (SD ± 691 kcal), kar je lahko povezano s stopnjo odpovedi prebavil. Razlike v porabi energije v mirovanju pa med posameznimi kliničnimi skupinami bolnikov nismo zaznali (p-vrednost >0,05).

Title:Comparison of energy expenditure measured by indirect calorimetry with actual energy intake in intestinal failure type III
Abstract:
Intestinal failure is a condition characterised by the reduction of gut function below the minimum level required for the absorption of nutrients and/or water and electrolytes, which must be supplemented intravenously to maintain health and/or growth. The aim of this study is to introduce indirect calorimetry as a method that allows more accurate planning and monitoring of nutritional therapy in Slovenian patients with intestinal failure. Our study is the first in Slovenia to use indirect calorimetry as part of the standard outpatient treatment of patients with intestinal failure in a centre for the treatment of intestinal failure. We included 20 stable patients with type III intestinal failure (chronic intestinal failure) receiving partial or total parenteral nutrition and 20 healthy volunteers. Patients were divided into 3 clinical groups according to the type of energy intake. We used Q-NRG® indirect calorimeter from Cosmed manufacturer to measure resting energy expenditure. We found that there was no statistically significant difference in measured resting energy expenditure between stable patients with type III intestinal failure and healthy volunteers (p-value > 0.05). However, in patients with type III intestinal failure, there is a statistically significant difference between measured resting energy expenditure and actual energy intake (p-value = 0.013), which varies greatly between individuals (SD ± 691 kcal), which might be related to the degree of intestinal failure. However, we could not detect any difference in resting energy expenditure between clinical groups of patients (p-value > 0.05).
