Primerjava dveh diagnostičnih testov za ugotavljanje občutljivosti urogenitalnih mikoplazem za antibiotike

Abstract
Urogenitalne mikoplazme, kot so patogena Mycoplasma genitalium in potencialno patogene Mycoplasma hominis ter Ureaplasma spp., povzročajo urogenitalna obolenja. Težavo pri zdravljenju teh okužb predstavlja odpornost bakterij proti antibiotikom. Mikrobiološka diagnostika mutacij, ki posredujejo odpornost M. gentalium, poteka z molekularnimi metodami, kot je PCR, diagnostika odpornosti M. hominis in Ureaplasma spp. proti antibiotikom pa z metodo kultivacije. V magistrski nalogi smo ovrednotili in primerjali testa PCR za ugotavljanje občutljivosti bakterije M. genitalium za makrolide dveh proizvajalcev, Allplex MG & AziR (Seegene) in Multiplex Tandem PCR Assays (AusDiagnostic). Testirali smo 228 vzorcev DNA bakterije M. genitalium. Ugotovili smo, da ima test proizvajalca AusDiagnostics v primerjavi s testom proizvajalca Seegene slabšo občutljivost in nizko specifičnost za določanje mutacije, ki posreduje odpornost proti makrolidom. V drugem delu naloge smo z uporabo 100 vzorcev s prisotno M. hominis ali Ureaplasma spp. validirali nov test Mycofast RevolutioN 2 (ELITech Group) in ga primerjali s starejšo različico testa Mycofast RevolutioN ATB+. Ugotovili smo, da je novejši test primernejši za uporabo v laboratoriju, saj se bolje sklada z smernicami CLSI, vsebuje pa tudi boljši nabor, koncentracije in razpored antibiotikov.

Title:Comparison of two diagnostic tests to determine the susceptibility of urogenital mycoplasmas to antibiotics
Abstract:
Urogenital mycoplasmas, such as pathogen Mycoplasma genitalium and potentially pathogenic Mycoplasma hominis and Ureaplasma spp., cause urogenital diseases. A problem in treating these infections is the resistance of bacteria to antibiotics. Microbiological diagnosis mutations that mediate resistance of M. genitalium is performed by molecular methods such as PCR. Diagnosis of resistance of M. hominis and Ureaplasma spp. to antibiotics is preformed by the method of cultivation. In the master's thesis, we evaluated and compared two PCR tests to determine the susceptibility of M. genitalium to macrolides from two manufacturers, Allplex MG & AziR (Seegene) and Multiplex Tandem PCR Assays (AusDiagnostic). We tested 228 samples of M. genitalium DNA. We found that the AusDiagnostics test has lower sensitivity and specificity for determining the mutation that mediates resistance to macrolides compared to the Seegene test. In the second part of the master's thesis, we validated the new Mycofast RevolutioN 2 test (ELITech Group) and compared it with the older version of the test, Mycofast RevolutioN ATB +, using 100 samples with presence of M. hominis or Ureaplasma spp. We found that the newer test is more suitable for use in the laboratory, as it better complies with CLSI guidelines and contains a better set, concentrations and separation of antibiotics.
