Abstract
Nevrotoksin botulin (BoNT) je proteinski nevrotoksin, ki ga proizvajajo gram pozitivne anaerobne bakterije Clostridium botulinum kot sedem različnih serotipov (A, B, C, D, E, F, G). BoNT je močan in nevrospecifičen toksin, z lokalnim injiciranjem ima omejeno prehajanje v tkivo, njegovo delovanje pa s časom izzveni. BoNT je že odobren za zdravljenje strabizma in blefarospazma, hude aksilarne hiperhidroze, kronične migrene, za odpravljanje zmernih do hudih glabelnih gub, gub ob zunanjih očesnih kotičkih in čelnih gub. Številne pa so študije in klinični primeri o učinkih BoNT, ki še niso odobreni (off-label), in sicer za zmanjšanje znojenja, bolečine in srbenja, rast las in druge kozmetične namene. Učinkovit je pri odpravljanju bromhidroze in neprijetnega vonja, kromhidroze, kraniofacialne hiperhidroze, merokrinega nevusa, Freyevega sindroma, žariščne analne hiperhidroze, granulosis rubra nasi, hidradenitis suppurativa, inverzne luskavice, čezmernega znojenja dlani in podplatov ter pomfoliksa. Za zmanjševanje bolečine se uporablja pri bolnikih z razpoko sluznice zadnjika, bruksizmom, kožnimi leiomiomi, erozivnim lichen planus, Fox-Fordyceovo boleznijo, nevralgijo ozeblin, granularno parakeratozo, lichen simplex chronicus, nevropatsko bolečino brazgotin, poherpetično nevralgijo, Raynaudovim fenomenom in vulvodinijo. Učinkovit je tudi proti srbenju nadlakti, koželjnice, srbenju po opeklinah in srbenju povzročenim s histaminom. Za izboljšanje videza so ga aplicirali za odpravljanje gub dekolteja, asimetrije/sinkinezije, eritema in rdečice obraza, nasmeha z vidno dlesnijo, levjega obraza, krčenja prsnih mišic, mastne kože, razširjenih por ter številnih hipertrofij (dvoglave mečne mišice, masetrnih mišic, senčnih mišic, trapezaste mišice ter hipertrofije žlez slinavk). Učinkovito tudi preprečuje in zdravi brazgotine. Uspešno odpravlja androgeno, cefalalgično in s sevanjem povzročeno alopecijo ter folikulitis decalvans. Študije kažejo njegovo učinkovitost pri odpravljanju apokrinih in merokrinih hidrocistomov, z vodo povzročene keratodermije, Darierjeve in Hailey-Haileyjeve bolezni, bulozne dermatoze linearnega imunoglobulina A, psoriaze v lehah in prirojene pahionihije. Da bi uporaba BoNT v omenjenih primerih prišla v odobreno uporabo, bo potrebno poenotiti protokole zdravljenja ter standardizirati redčitve, odmerjanje, časovne intervale in tehnike injiciranja.

Title:Off-label use of botulinum toxin in dermatology
Abstract:
Botulinum neurotoxin (BoNT) is a protein produced by gram-positive anaerobic bacteria Clostridium botulinum in seven serotypes (A, B, C, D, E, F, G). BoNT is a potent and neurospecific toxin, with limited tissue passage and its action wears off over time. BoNT has been approved for treatment of strabismus and blepharospasm, severe axillary hyperhidrosis, chronic migraines, moderate to severe glabellar lines, lateral canthi lines and forehead lines. Many clinical studies and case reports describe the off-label use of BoNT for sweat, pain and pruritus reduction, hair growth and other cosmetic purposes. It is effective in treating bromhidrosis and malodor, chromhidrosis, craniofacial hyperhidrosis, eccrine nevus, Frey's syndrome, focal anal hyperhidrosis, granulosis rubra nasi, hidradenitis suppurativa, inverse psoriasis, palmoplantar hyperhidrosis and pompholyx. BoNT reduces pain in patients with anal fissure, bruxism, cutaneous leiomyomas, erosive lichen planus, Fox-Fordyce disease, frostbite neuralgia, granular parakeratosis, lichen simplex chronicus, neuropathic scar pain, postherpetic neuralgia, Raynaud’s phenomenon, and vulvodynia. It is effective against brachioradial pruritus, burn-induced pruritus and histamine-induced pruritus. For cosmetic enhancement, it was applied for elimination of décolleté wrinkles, facial asymmetry/synkinesis, erythema and flushing, gummy smile, leonine facies, pectoral muscle contraction, oily skin, enlarged pores and numerous hypertrophies (of gastrocnemius muscle, masseter muscle, temporal muscle, trapezius muscle and salivary gland hypertrophy). It is also effective in scar prevention and treatment. It's use was successful in treating androgenetic, cephalalgic and radiation-induced alopecia and folliculitis decalvans. Moreover, studies have shown its effectiveness in treating apocrine and eccrine hidrocystomas, aquagenic keratoderma, Darier's and Hailey-Hailey disease, epidermolysis bullosa simplex, linear immunoglobulin A bullous dermatosis, plaque psoriasis, and pachyonychia congenita. To have the use of BoNT approved, harmonization of treatment protocols and standardization of dilutions, dosing, timing and injection techniques have to be achieved.
