Primerjava meritev stanja hidriranosti pacientov s parenteralno prehrano na domu z dvema bioimpedančnima napravama

Abstract
Odpoved prebavil je kompleksno bolezensko stanje, ki potrebuje strokovno vodeno prehransko obravnavo in prehransko terapijo. Pri kroničnih oblikah je ključna terapevtska oblika parenteralna prehrana na domu. Bolezenska stanja, ki vodijo v odpoved prebavil, pogosto pripeljejo do tekočinskih neravnovesij in podhranjenosti. Kot metoda meritve sestave telesa se pogosto uporablja bioimpedančna analiza. V nalogi smo med seboj primerjali dve različni bioimpedančni napravi – Quadscan 4000 in Multiscan 5000. Prva je multifrekvenčna bioimpedančna naprava in opravlja meritve pri 5, 50, 100 ter 200 kHz, druga pa spada v kategorijo bioimpedančne spektroskopije (BIS) in deluje na 50 različnih frekvencah med 5 in 1000 kHz. Z njima smo ocenjevali status hidriranosti in prehranjenosti pacientov z odpovedjo prebavil, ki se zdravijo s parenteralno prehrano na domu. Meritve smo izvajali kot del rutinskega pregleda v Enoti za klinično prehrano Onkološkega inštituta Ljubljana. Ugotovili smo, da je naprava Multiscan 5000 boljši indikator podhranjenosti, če primerjamo fazne kote in tudi ob primerjavi meritev z meritvami sečnine in kreatinina. Ugotovili smo, da z napravo Multiscan 5000 bolje prikažemo podhranjenost v povezavi s faznim kotom in opredelimo stanje hidracije. Kadar pa upoštevamo GLIM kriterije, pa sta obe napravi enako učinkovito orodje za ugotavljanje podhranjenosti pacientov. Pri multidisciplinarnem pristopu in skupaj s krvnimi preiskavami ter zdravniškim pregledom sta lahko obe napravi primerno orodje za spremljanje teh bolnikov.

Title:Comparison of two bioimpedance devices for the assessment of hydration status in patients with home parenteral nutrition
Abstract:
Intestinal failure is a complex disease that requires constant nutritional assessment and nutritional therapy. Home parenteral nutrition, which provides sufficient nutrients and fluids is a crucial therapeutical practice in patients with chronic intestinal failure. Regular body composition measurments are therefore essential in the assessment of patients. In this masters thesis we compared two different bioimpedance analysis devices, Quadscan 4000 and Multiscan 5000 – first one being a multifrequency bioimpedance device, measuring at 5, 50, 100 and 200 kHz and the second one being a bioimpedance spectroscopy device which operates at 50 different frequencies between 5 and 1000 kHz. We assessed hydration and nutritional status of patients with intestinal failure, treated with home parenteral nutrition. Our main findings were that both devices can offer comparable measurments, but there are some major differences. Multiscan 5000, proved to be a better indicator of malnutrition when phase angles were compared and when blood urea and creatinine were compared. Both devices were similarly reliable at indicating malnourished patients according to GLIM criteria for malnutrition. We conclude that with a multimodal approach in the assessment of patients with chronic intestinal failure, both devices can be a usefull tool for monitoring of dehydration and malnutrition.
