Abstract
Parodontalna bolezen je ena izmed najbolj razširjenih kroničnih okužb današnjega časa in je vzrok za največ izgubljenih zob pri odraslih. Raziskave so pokazale, da parodontalna bolezen poleg tega, da povzroča izgubo zob, vpliva tudi na sistemsko zdravje s tem, da sproži ali pa poslabša nekatere sistemske bolezni. Namen prvega dela raziskave je bil ugotoviti ali lahko dolgotrajno kronično vnetje, ki je prisotno pri parodontalni bolezni in smo ga ovrednotili s pomočjo ocene velikosti parodontalne rane in parodontalne vnetne obremenitve, povzroči povišane vrednosti glikiranega hemoglobina v krvi pri osebah brez diagnosticirane sladkorne bolezni. Pri tem smo preučili tudi vpliv dejavnikov tveganja na raven glikiranega hemoglobina v krvi kot so spol, starost, kajenje, krvni tlak, indeks telesne mase in obseg pasu. Namen naše raziskave v drugem delu je bil preučiti prisotnost parodontopatogenih bakterij Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Prevotella intermedia, Tannerella forsythia in Treponema denticola ter parazitov Trichomonas tenax in Entamoeba gingivalis v obzobnih žepih oseb s parodontalno in sladkorno boleznijo tipa 1 in 2 ter pri tistih brez diagnosticirane sladkorne bolezni, primerjati skupine med seboj ter ugotoviti vpliv zdravljenja z metodo popolne dezinfekcije ustne votline na raven glikiranega hemoglobina v krvi ter na prisotnost parodontopatogenih bakterij in parazitov po treh mesecih. Rezultati naše raziskave so pokazali, da se z večanjem parodontalne rane in parodontalne vnetne obremenitve povečuje vrednost glikiranega hemoglobina v krvi pri ljudeh brez sladkorne bolezni. Prav tako smo ugotovili, da zdravljenje z metodo popolne dezinfekcije ustne votline izboljša klinične parodontalne parametre ter izboljša urejenost glikemije pri osebah s sladkorno boleznijo tipa 1 in 2 ter pri osebah brez sladkorne bolezni.

Title:The influence of periodontal inflammatory burden on metabolic parameters and subgingival microflora in subjects without and with diabetes
Abstract:
Periodontal disease is one of the most prevalent chronic infectious diseases of our time and the cause of significant tooth loss in adults. Research has shown that periodontal diseases also influence systemic health by triggering or exacerbating certain systemic conditions. The first part of this study aimed to examine whether long-term chronic inflammation present in periodontal disease and determined by an assessment of a periodontal wound and periodontal inflammatory burden could cause an elevation of glycated haemoglobin in patients without diabetes. We also examined the influence of risk factors such as gender, age, smoking, blood pressure, body mass index and waist circumference on the level of glycated haemoglobin. The second part of our study aimed to examine the presence of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Prevotella intermedia, Tannerella forsythia and Treponema denticola periodontopathogenic bacteria and Trichomonas tenax and Entamoeba gingivalis parasites in periodontal pockets of patients with periodontal disease and diabetes type 1 and type 2 and in those without diabetes, to compare the patient groups and to assess the effect of the full mouth disinfection method on the level of glycated haemoglobin, clinical periodontal parameters and presence of periodontopathogenic bacteria and parasites in periodontal pockets after three months. The results of our study showed that the periodontal wound and periodontal inflammatory burden increase the value of glycated haemoglobin in blood of non-diabetics. Based on our research, we can conclude that the full mouth disinfection method improves clinical periodontal parameters and glycemic control in type 1 and type 2 diabetes patients as well as in patients without diabetes.
