Abstract
Uvod: Arterijska hipertenzija je najpogostejši vzrok za obisk ambulante družinske medicine, saj je to velik javnozdravstveni problem in kronično stanje, ki zelo pogosto zahteva dolgotrajno zdravljenje. Predvsem je pomembna preventiva, v katero bi lahko medicinska sestra v okviru svojega delovanja vključila mobilne aplikacije, ki vsebujejo dnevnik, ki beleži meritve izmerjenega krvnega tlaka na organiziran način. Poleg tega te aplikacije nudijo pacientom koristne informacije o bolezni, njenem zdravljenju, ustreznem merjenju krvnega tlaka in vodenju ustreznega življenjskega sloga. Namen: Namen diplomskega dela je ugotoviti možnosti uporabe v preventivi in spremljanju arterijske hipertenzije s pomočjo mobilnih aplikacij s poudarkom na identifikaciji vloge diplomirane medicinske sestre pri obravnavi pacienta z arterijsko hipertenzijo na primarni ravni zdravstva. Cilj diplomskega dela je opisati, kako lahko diplomirana medicinska sestra vključi mobilno zdravje kot del preventivnega pristopa k obvladovanju arterijske hipertenzije v ambulanti družinske medicine. Metode dela: Opravljen je bil pregled domače in tuje strokovne in znanstvene literature. Na podlagi pregleda zadetkov in analize relevantnih virov je opisana uporaba mobilnih aplikacij s strani medicinske sestre v ambulanti družinske medicine pri pacientih z arterijsko hipertenzijo. V pregled literature je bilo vključenih 19 enot literature. Rezultati: Mobilne aplikacije so bolj dostopne kot spletne aplikacije in v državah z visokimi dohodki je bilo ugotovljeno, da le-te povečajo učinkovitost izvajalcev zdravstvenih storitev, jim prihranijo čas in zagotovijo platformo za povezovanje s svojimi pacienti ter za skupno odločanje; mobilne aplikacije so lahko koristne v smislu samovodenja, zdravljenja in nadzora arterijske hipertenzije. Razprava in zaključek: Uspešen nNadzor krvnega tlaka je močno odvisen od farmakološke terapije in spremembe življenjskega sloga, pri čemer imata samopomoč oziroma samooskrba pacienta veliko vlogo. Glede na dostopno literaturo smo ugotovili, da bi mobilno zdravje lahko bilo obetavno dopolnilo klasičnemu pristopu pri zdravljenju arterijske hipertenzije, saj študije dokazujejo, da mobilne aplikacije lahko pozitivno vplivajo na nadzor krvnega tlaka. Preventivne aktivnosti zdravstvene nege so pri pacientih z arterijsko hipertenzijo izredno pomembne, saj pripomorejo k preprečevanju morebitnih zapletov arterijske hipertenzije. Mobilne aplikacije bi bilo smiselno vključiti v preventivne aktivnosti zdravstveno nege, saj bi s tem razbremenili zdravstveni sistem; pacienti bi imeli boljšo dostopnost do zdravstvenih storitev in manjše stroške.

Title:Use of mobile applications to empower the patients with arterial hypertension for self-care : diploma work
Abstract:
Introduction: Arterial hypertension is the most common cause for visiting a family medicine clinic because it is a major public health problem and is a very common chronic condition that requires long-term treatment. Particularly important is preventative health care, in which we could include mobile applications that contain a log that records measurements of measured blood pressure in an organized manner. Applications provide patients with useful information about the disease, their treatment, appropriate blood pressure measurement, and lifestyle management. Purpose: The purpose of the bachelor’s thesis is to identify the possibilities of the use in the prevention and monitoring of arterial hypertension through mobile applications, with the emphasis on identifying the role of graduate nurses in the treatment of patients with arterial hypertension at the primary care level. The bachelor’s thesis aims to describe how a nurse can integrate mobile health as a prevention of arterial hypertension in a family medicine clinic. Methods: In the bachelor’s thesis, the descriptive method with a review of domestic and foreign professional and scientific literature was used. Based on the review and analysis, we described the use of mobile applications in family medicine clinics in patients with arterial hypertension. The literature review included 19 units of literature. Results: Mobile applications are more accessible than online applications. In high-income countries, they were found to increase the efficiency of healthcare providers, save their time, and provide a platform for connecting with their patients and for joint decision-making. Mobile applications can be useful in terms of self-assessment, treatment, and control of arterial hypertension. Discussion and conclusion: Successful bBlood pressure control is highly dependent on pharmacological therapy and lifestyle changes with self-help or self-care of the patient playing a major role. Based on the available literature, we found that mobile health could be a promising alternative in the treatment of arterial hypertension because studies demonstrate that mobile applications can positively affect on blood pressure control. Preventive nursing care activities are extremely important in arterial hypertension because it helps to prevent possible complications of arterial hypertension. Including mobile applications in preventive nursing activities would have positive effect on healthcare system and on providing better access to healthcare services to the patient and lower healthcare costs.
