Točnost kvantifikacije 99mTc-DPD v osteoblastnih skeletnih lezijah : magistrsko delo

Abstract
Uvod: Izračun in predstavitev količine aktivnosti v določeni prostornini tkiva, izražene z izbrano mersko enoto (npr. Bq/ml) poimenujemo kvantifikacija podatkov. S pojavom naprednejših naprav in tehnik jo je dandanes možno izpeljati tako s pomočjo planarnih scintigramov kot tudi s tomografsko (SPECT/CT) zbranimi podatki. Ker popolnoma točne kvantifikacije pri dveh načinih zbiranja podatkov ni možno zanesljivo določiti, se ponuja vprašanje ali sta omenjena načina kvantifikacije podatkov med seboj primerljiva. Namen: Glavni namen raziskave je bil med seboj primerjati rezultate kvantifikacije, ki jo izračunamo pri planarnem in pri SPECT/CT načinu zajemanja podatkov ter ugotoviti obstoj morebitnih korelacij med njima. Poleg tega smo želeli ugotoviti kako na kvantifikacijo vplivajo določeni dejavniki (značilnosti pacientov in načina zbiranja podatkov). Metode dela: Retrospektivna študija je zajemala analizo 89 osteoblastnih skeletnih lezij pri 65 pacientih, pri katerih je bila izvedena planarna scintigrafija skeleta celega telesa in SPECT/CT skeletne regije. Na planarnih scintigramih smo kvantifikacijo aktivnosti v lezijah izračunali na način, kot se ga običajno uporablja na Oddelku za nuklearno medicino Onkološkega inštituta Ljubljana. Za izračun kvantifikacije pri SPECT/CT smo uporabili računalniško orodje Q.Metrix (GE Healthcare inc.). Beležili smo tudi dejavnike kot so spol, starost, indeks telesne mase pacienta, aplicirana aktivnost, čas med aplikacijo radiofarmaka in začetkom zajemanja podatkov ter čas med zajemanjem podatkov s planarno kamero in SPECT/CT. Pridobljene podatke smo ustrezno statistično analizirali. Rezultati: Ugotovili smo, da med dvema načinoma kvantifikacije aktivnosti obstajajo statistično značilne razlike (p?0,001), vendar med njima obstaja tudi močna linearna korelacija (vrednost Pearsonove korelacije je 0,937). Glede na to ugotovitev smo z linearno regresijsko analizo izpeljali model (R2=0,88), s katerim je možno napovedati rezultat enega načina kvantifikacije z uporabo rezultata drugega načina kvantifikacije. Analiza podatkov je pokazala tudi, da na posamezen način kvantifikacije testirani dejavniki nimajo statistično značilnega vpliva. Razprava in zaključek: Z raziskavo smo dokazali obstoj povezave med dvema načinoma kvantifikacije. Rezultati naše raziskave vsekakor predstavljajo dobro izhodišče za nadaljnje raziskave in razmišljanja.

Title:Accuracy of 99mTc-DPD quantification in osteoblastic skeletal lesions : master thesis
Abstract:
Introduction: The calculation and presentation of activity in a given volume of tissue or lesion, expressed by selected unit of measure (eg Bq/ml) is named data quantification. With the advent of more advanced and modern devices and techniques, it is possible to calculate the quantification by planar scintigrams as well as tomographic (SPECT/CT) data acquisition. Beacuse a fully accurate quantification can not be reliably determined, the question is do these two methods of data quantification are comparable to each other. Purpose: Main purpose of the research was to compare the results of the planar scan based and SPECT/CT based technique of activity quantification. In addition, purpose was to determine the potential impact of different patient based or acquisition dependent variables. Methods: The retrospective study included analysis of 89 osteoblastic skeletal lesions in 65 patients who had planar whole body bone scintigraphy and regional SPECT/CT performed. The activity in lesions on planar images was calculated according to the standard procedure used at the Department of Nuclear Medicine in Institute of Oncology Ljubljana. The commercially available SPECT/CT Q.Metrix quantification software (GE Healthcare inc.) was used to calculate quantification in SPECT/CT data. Several variables were analyzed too: patient's gender, age, body mass index, injected activity, time between application of radiopharmaceutical and the beginning of data acquisition, time between planar and SPECT data acquisition. All obtained data were adequately statistically analyzed. Results: The difference between two quantification methods was statistically significant (p⡤0,001). There was a strong linear correlation between both quantification methods (the Pearson correlation value was 0,937). Based on linear regression analysis, we defined the model (R2=0,88) to predict the result of one method of quantification using the result of the second method. Tested variables didn't have statistically significant impact on each of the quantification methods. Discussion and conclusion: The results from this study show that the two methods of data quantification are comparable to each other. The results of our research are a good starting point for further researches and reflections.
