Abstract
Uvod: Funkcijski test zgornjega uda (angl. action research arm test – ARAT) ocenjuje fino motoriko, dvigovanje in premikanje predmetov ter grobe gibe. Namenjen je bil pacientom po možganski kapi in z nezgodno poškodbo možganov, uveljavlja se tudi pri drugih populacijah. Namen pregleda literature je bil povzeti merske lastnosti ARAT. Metode: Raziskovalne članke smo iskali v MEDLINE, CINAHL in Science Direct ter s pregledom literature na RehabMeasures, StrokEngine in v člankih. Rezultati: V pregled je bilo zajetih 39 raziskav. ARAT ima odlično zanesljivost pri pacientih po možganski kapi, z nezgodno poškodbo možganov, multiplo sklerozo in Parkinsonovo boleznijo ter visoko pri pacientih s transplantacijo na zgornjem udu. Odzivnost za spremembe pri pacientih po možganski kapi in z okvaro hrbtenjače je srednje velika ali velika. Veljavnost konstrukta je bila raziskana pri pacientih po možganski kapi in z multiplo sklerozo, sočasno veljavnost tudi pri pacientih z nezgodno poškodbo možganov in okvaro hrbtenjače. Učinka tal in stropa, klinično pomembno razliko in napovedno veljavnost so ugotavljali le pri pacientih po možganski kapi. Zaključek: ARAT je primeren za uporabo pri pacientih po možganski kapi v vseh obdobjih. Uporaba narašča tudi pri drugih populacijah pacientov, pri katerih je treba nekatere merske lastnosti še raziskati.

Title:Psychometric properties of the action research arm test
Abstract:
Introduction: Action research arm test (ARAT) assesses fine motor abilities, lifting and movement of the objects, and gross movements. It was designed for patients after stroke and traumatic brain injury, but it is in use in other populations as well. The purpose was to review the measurement properties of ARAT. Methods: Studies of ARAT measurement properties identified in MEDLINE, CINAHL in Science Direct and reference lists at RehabMeasures, StrokEngine and the articles were included in the review. Results: 39 studies were reviewed. ARAT has excellent reliability in patients after stroke, traumatic brain injury, multiple sclerosis and Parkinson's disease, and high reliability in patients with transplantation at the upper limb. Ability to detect change in patients after stroke and with spinal cord injury is moderate to good. Construct validity was established in patients after stroke and with multiple sclerosis. Additionally, concurrent validity was established in patients with traumatic brain injury and spinal cord injury. Floor and ceiling effects, clinical important difference and predictive validity were established only in patients after stroke. Conclusion: ARAT is appropriate for use in patients after stroke at various stages. Its use is increasing in other patients’ populations in which some measurement properties need investigation.
