Abstract
Uvod: Strelne rane so bile najprej večinoma vojne poškodbe, sedaj pa se pojavljajo tudi v civilizaciji, zaradi česar se lahko laiki čedalje pogosteje srečujejo s tovrstnimi poškodbami. Pri strelnih ranah je velika verjetnost hudih krvavitev, kar je vodilni vzrok smrti, posledično pa je ključno poznavanje oskrbe le-teh. Krvavitev je izguba krvi iz obtočil. Krvavitve razdelimo glede na vzrok (bolezenske in poškodbene), lokalizacijo (notranje in zunanje) in vrsto (arterijske, venske, kapilarne). Notranje krvavitve so zelo nevarne, saj povzročajo nastanek hemoragičnega oz. hipovolemičnega šoka, saj jih je zelo težko prepoznati. Nevarna je tudi zunanja arterijska krvavitev, a jo je bistveno lažje prepoznati. Namen: Namen diplomskega dela je s pregledom domače in tuje literature podrobneje predstaviti prvo pomoč laika pri krvavitvah, ki so posledica strelnih ran. Cilji so proučiti pristope laika k poškodovancu s strelno rano ter predstaviti načine laične prve pomoči za zaustavljanje krvavitev. Metode dela: V diplomskem delu smo uporabili deskriptivno metodo s sistematičnim pregledom literature, ki je bila objavljena v časovnem obdobju med leti 2009 in 2019, izjema je le nekaj virov, kjer se doktrina ni spreminjala. Iskali smo preko Digitalne knjižnice Ljubljana po podatkovnih bazah MEDLINE, CINAHL, ScienceDirect in Pubmed. Uporabili smo bibliografsko-kataloško bazo podatkov COBISS in spletni brskalnik Google učenjak. Rezultati: Pristop k poškodovancu s strelno rano je zelo specifičen, še posebej zaradi nevarnih okoliščin, zaradi katerih je prišlo do poškodbe. Zelo pomembno in smiselno je, da se ob tem držimo osnovnih načel prve pomoči. Poznamo več različnih tehnik za zaustavljanje krvavitve, vendar se oskrba strelne rane razlikuje glede na mesto poškodbe, hkrati pa je tudi zelo pomembno ukrepati pri hemoragičnem šoku, kateri nastane zaradi hudih krvavitev. Razprava in zaključek: Krvavitve predstavljajo velik zdravstveno-socialni problem in so osrednji vzrok smrti pri strelnih poškodbah. Zaradi razvoja medicine in kopičenja izkušenj so se v preteklih letih močno razvili pripomočki za nudenje prve pomoči, nastali pa so tudi namenski kompleti za ustavljanje krvavitev. Še vedno pa je ključen varen pristop in znanje nudenja prve pomoči s strani strokovnih delavcev in čedalje bolj pogosto tudi laikov. Laična populacija je relativno velika, a bi z usmerjenimi in dobro vodenimi usposabljanji dosegli bistveno večjo odzivnost in uspešnost pri nudenju prve pomoči pri strelnih ranah.

Title:Stopping the bleeding from the side of the laic in gunshot wounds : diploma work
Abstract:
Introduction: Gunshot wounds were initially mostly war injuries, but now also occur in civilization, therefore nonprofessionals can encounter such injuries with increasing frequency. Severe bleeding is likely to occur in gunshot wounds, which is the leading cause of death, knowing how to treat it is consequently crucial. Bleeding is loss of blood from the circulatory system. Bleedings are divided by cause (disease and injury), localization (internal and external) and type (arterial, venous, capillary). Internal bleeding is very dangerous as it causes hemorrhagic or hypovolemic shock because it is very difficult to identify. External arterial bleeding is also dangerous, but significantly easier to recognize. Purpose: Purpose of the diploma thesis is to present in detail first aid provided by nonprofessionals in bleedings that result from gunshot wounds, by reviewing domestic and foreign literature. The objectives are to study how nonprofessionals approach an injured person with a gunshot wound, and to introduce methods of giving nonprofessional first aid to stop the bleeding. Methods: In the diploma thesis, we used a descriptive method with systematic review of literature published between 2009 and 2019, with the exception of few sources where the doctrine has not changed. We searched through the Digital Library of Ljubljana in MEDLINE, CINAHL, ScienceDirect and PubMed databases. We used the COBISS bibliographic-catalogue database and the Google Scholar web browser. Results: Approaching an injured person with a gunshot wound is very specific, especially due to dangerous circumstances that lead to the injury. It is very important and sensible to follow basic principles of first aid. There are several different techniques for stopping bleeding, however treating a gunshot wound varies depending on the location of the injury, and it is also very important to treat hemorrhagic shock that results from severe bleeding. Discussion and conclusion: Bleeding presents a major health-social problem and is the main cause of death in gunshot injuries. Due to development of medicine and accumulation of experience, first aid devices have evolved greatly over the years, also generating kits for stopping bleeding. Safe approach and knowledge of providing first aid by professionals and, with increasing frequency, nonprofessionals, is still crucial. Nonprofessional population is relatively large, but with targeted and quality trainings, significantly greater responsiveness and success in providing first aid in gunshot wounds would be achieved.
