Abstract
Uvod: Hoja je pomemben kazalnik telesnega funkcioniranja. Hoja naravnost je simetrična in predstavlja porazdelitev telesne mase in omogoča posamezniku, da se premika samostojno in varno iz enega kraja v drugega. Poraba časa je lahko različna pri testih hoje na deset metrov, kar vpliva na končni izid pri izvedbi sproščene in hitre hoje. Test hoje na deset metrov (10MWT) je zaradi dobrih merskih lastnosti pogosto uporabljeno merilno orodje za oceno premičnosti, vendar lahko izvedba testa predstavlja prostorsko omejitev. Test hoje v krogu na deset metrov (TH10mK) v desno in levo smer lahko odpravi prostorske omejitve, vendar hoja po ukrivljeni poti predstavlja asimetrične porazdelitve mase telesa, spremembo v ravnotežju, večjo pozornost za izvedbo v primerjavi s hojo naravnost. Namen: Raziskave je bil ugotoviti ponovljivost, zanesljivost in veljavnost TH10mK pri starejših odraslih osebah stanujočih v domu starejših občanov za sproščeno in hitro hojo. Metode dela: Sodelovalo je 32 starejših odraslih oseb iz doma starejših občanov. Povprečna starost udeležencev je bila 85,8 (SO 4,9) let. Ponovljivost treh zaporednih meritev za eno sejo smo izračunali z analizo variance za ponovljene meritve (ANOVA). Za zagotovitev zanesljivosti meritev po 48 urah premora so bili izračunani intraklasni korelacijski koeficienti (ICC), standardna napaka merjenja (SEM), koeficient variacije (CoV), meje skladnosti (LoA). Veljavnost meritev smo določili s Pearsonovim korelacijskim koeficientom (r). Rezultati: Rezultati analize so izkazali odlično ponovljivost za TH10mK za sproščeno hojo, in sicer TH10mK v desno (ICC= 0,95-0,99) in TH10mK v levo (ICC= 0,98). Odlična zanesljivost TH10mK za sproščeno hojo (ICC= 0,94-0,97), in sicer TH10mK v desno (ICC= 0,95-0,97) in TH10mK v levo (ICC= 0,90-0,92). Zmerna do odlična linearna povezanost TH10mK v desno z 10MWT za sproščeno hojo (r= 0,59-0,60) in TH10mK v levo z 10MWT (r=0,82-0,90), TH10mK v desno z TH10mK v levo (r=0,62-0,63). Odlična ponovljivost za TH10mK za hitro hojo, in sicer TH10mK v desno (ICC= 0,97-0,99) in TH10mK v levo (ICC= 0,99). Odlična zanesljivost za TH10mK za hitro hojo, in sicer TH10mK v desno (ICC= 0,96-0,98) in TH10mK v levo (ICC= 0,94-0,96). Zmerna do odlična linearna povezanost TH10mK v desno z 10MWT za hitro hojo (r=0,77-0,81) in TH10mK v levo z 10MWT (r=0,84-0,88), TH10mK v desno z TH10mK v levo (r=0,78-0,89). Zaključek: Hoja v krogu vključuje številne prilagoditve, kar se je v naši raziskavi izkazalo s povečanjem porabe časa in posledično zmanjšanjem hitrosti pri TH10mK v obe smeri. Ugotovili smo, da so meritve ponovljive, zato izvedba treh meritev ni potrebna in zadošča izvedba ene meritve. Ugotovili smo, da so meritve zanesljive, če smo vključili 48 urni interval premora med meritvami, zato ni potrebna izvedba dveh sej, ampak zadošča ena seja z eno meritvijo. Linearna povezanost TH10mK s 10MWT je zmerna do odlična, kar nakazuje, da je TH10mK veljavno ocenjevalno orodje za ocenjevanje premičnosti. Smiselno bi bilo vključiti večje število udeležencev in jih razvrstiti v skupine glede na uporabo pripomočkov za hojo ter raziskati še druge merske lastnosti za TH10mK glede na diagnoze in bolezenska stanja.

Title:Measurement proprerties of ten meter walk test in circle for elderly nursing home residents : master thesis
Abstract:
Introduction: Walking is an important indicator of the physical functioning. Walking straight is symmetrical and represents distribution of body mass and allows individuals' ability to move independently and safely from one place to another. Time consumption vary for ten meter walk tests, which affects the final outcome of comfortable and fast walk. Ten meter walk test (10MWT) is a measuring tool often used for assessing mobility due to its psychometric properties, it can however represent a spatial limitation.Ten meter walk test in circle (10MWTC) in the right and the left direction can eliminate spatial limitations, however, walking on a curved path represents asymmetrical body mass distributions, balance changes, greater attention to the performance compared to walking straight. Purpose: Was to determine the repeatability, reliability and validity of 10MWTC for comfortable and fast walking. Methods: 32 elderly nursing home residents were included in study. The mean age of the participants were 85,8 (SD 4,9) years. The repeatability of the three consecutive measurements for one session was calculated by the analysis of the variance for repeated measurements (ANOVA). To ensure the reliability of measurements after 48 hour rest intraclass correlation coefficients (ICC), standard error measurement (SEM), coefficient of variation (CoV), limits of agreement (LoA) were calculated. The validity of the measurements was determined by Pearson's correlation coefficient (r). Results: The results showed excellent repeatability for 10MWTC for comfortable walking, namely 10MWTC in the right (ICC= 0.95-0.99) and 10MWTC in the left direction (ICC= 0.98). Excellent reliability for 10MWTC for comfortable walking, namely 10MWTC in the right (ICC= 0.95-0.97) and 10MWTC in the left direction (ICC= 0.90- 0.92), moderate to excellent linear connection 10MWTC to the right with 10MWT for comfortable walking (r= 0.59) and 10MWTC in the left direction to 10MWT (r=0,81-0.89) and betwen 10MWTC to the right with 10MWTC to the left direction (r= 0.62-0.63). Excellent repeatability for 10MWTC for fast walking, namely 10MWTC in the right (ICC=0.96-0.98) and 10MWTC in the left direction (ICC= 0.98). Excellent reliability for 10MWT for fast walking, namely 10MWTC in the right (ICC= 0.96-0.98) and 10MWTC in the left direction (ICC= 0.94-0.96), moderate to excellent linear connection 10MWTC to the right with 10MWT for fast walking (r= 0.77-0.82) and 10MWTC to the left direction with 10MWT (r=0,84-0,88) and between 10MWTC to the right to 10MWTC in the left direction (r= 0.78-0.89). Conclusion: Walking in a circle involves a number of adjustments, which in our study is evident from an increase in time consumption and consequently reduction of walking speed in the 10MWTC in both directions. We found that the measurements are repeatable, there for the implementation of three consecutive measurements is not required, and single measurement is sufficient. With the inclusion of 48 hour rest interval between measurements we found that the measurements are reliable, therefore there is no need to perform two sessions, a single session with one measurement suffices. The linear connection between 10MWTC and 10MWT is moderate to excellent. 10MWTC can be used as a mobility test for elderly adults. It would be sensible to include a larger number of participants and combine them with the use of walking aids and explore other measurement properties for 10MWTC based on diagnosis and disease conditions.
