Abstract
Uvod: Terapija, podprta z robotsko tehnologijo, je s ponavljajočimi, intenzivnimi in motivirajočimi obravnavami obetavna oblika terapije, ki zagotavlja izboljšanje v motorični kontroli zgornjega uda in izvedbi vsakodnevnih aktivnosti bolnikov, ki so preživeli možgansko kap. Namen: Namen diplomskega dela je bil oceniti učinkovitost uporabe naprave Armeo Spring pri izboljšanju funkcij okvarjenega zgornjega uda osebe po možganski kapi. Spremljali smo obseg gibov, motorične funkcije in količino uporabe okvarjenega zgornjega uda v vsakodnevnih aktivnostih ter posameznikovo dojemanje izvedbe in zadovoljstva z izvedbo njemu pomembnih okupacij. Metode: V šesttedensko obravnavo je bil vključen 69-letni bolnik z desnostransko hemiparezo, ki je bil ob začetku obravnave 5 mesecev po možganski kapi. Bolnik je dvakrat na teden po 45 minut izvajal obravnavo na napravi Armeo Spring. Naprava zagotavlja podporo teže zgornjega uda in omogoča izvajanje posebnih nalog za povečanje mišične moči ter obsega gibov različnih sklepov z namenom izboljšanja motoričnih funkcij. Terapevtski načrt je bil sestavljen iz osmih nalog, ki so ciljale na izboljšanje obsega gibov zgornjega uda, koordinacijo gibanja zgornjega uda in funkcijo prijema. Obravnavo smo stopnjevali z večanjem zahtevnosti nalog in z manjšanjem podpore teže zgornjega uda. Pred in po koncu obravnave smo izvedli naslednja ocenjevanja: Kanadski test izvedbe okupacije, Lestvico funkcijske neodvisnosti, Wolfov test motoričnih funkcij, Vprašalnik za ocenjevanje motorične dejavnosti in goniometrijo rame, komolca in zapestja. Rezultati: Izboljšalo se je bolnikovo dojemanje izvedbe in zadovoljstva z izvedbo njemu pomembnih okupacij (Kanadski test izvedbe okupacije: izvedba iz 2,25 na 4,5; zadovoljstvo iz 3 na 5), motorične funkcije okvarjenega zgornjega uda (Wolfov test motoričnih funkcij: iz 4,63 s na 3,35 s), količina uporabe okvarjenega zgornjega uda v vsakodnevnih aktivnostih (Vprašalnik za ocenjevanje motorične dejavnosti: količina iz 1,05 na 1,55), obseg gibov okvarjenega zgornjega uda in moč prijema (dinamometer iz 5 kg na 8 kg). Razprava in zaključek: Na podlagi dobljenih rezultatov lahko sklepamo, da je šesttedenska obravnava s pomočjo terapije, podprte z robotsko tehnologijo – napravo Armeo Spring, primerna oblika obravnave za izboljšanje funkcij okvarjenega zgornjega uda v kliničnem okolju za bolnike v kronični fazi po možganski kapi s podobnimi kognitivnimi in motoričnimi sposobnostmi kot jih je imel bolnik vključen v študijo.

Title:Use of Armeo Spring device on stroke patient – case study : diploma work
Abstract:
Introduction: Robot assisted therapy is a promising form of therapy providing improvement of motor control in upper limb and activities of daily living for people living with stroke due to its repetitive, intensive and motivating intervention. Purpose: The goal of this thesis was to evaluate the effectiveness of using Armeo Spring for the improvement of upper limb functions in post-stroke patient. We have monitored range of motion in impaired upper limb, upper limb motor function, the amount of upper limb use in everyday activities and patients' perception of performance and satisfaction with occupations that were meaningful to him. Methods: Participant was a 69-year-old male, who suffered right-sided hemiparesis and was 5 months post stroke at the beginning of the intervention. Patient has performed activities on Armeo Spring exoskeleton for six weeks. The intervention frequency was twice a week for 45 minutes. The therapeutic plan consisted of eight activities that were aiming to improve range of motion in upper limb, coordination of upper limb movements and grip function. We intensified the intervention by increasing the level of difficulty in activities and reducing the arm weight support. The following assessments were conducted at baseline and post-intervention: Canadian Occupational Performance Measure, Functional Independence Measure, Wolf Motor Function Test, Motor Activity Log, and goniometry of shoulder, elbow and wrist. Results: Improvement occurred in patients' perception of performance and satisfaction with occupations that were meaningful to him (Canadian Occupational Performance Measure: performance from 2,25 to 4,5; satisfaction from 3 to 5), upper limb motor function (Wolf Motor Function Test: from 4,63s to 3,35s), the amount of upper limb use in everyday activities (Motor Activity Log: amount from 1,05 to 1,55), range of motion in impaired upper limb and grip strength (dynamometer: from 5kg to 8kg). Discussion and conclusion: On the basis of obtained results we can conclude that six weeks robot therapy intervention with Armeo Spring device could be an appropriate form of therapy for upper limb in clinical setting for patients in chronic phase of stroke with similar cognitive and motor skills to the patient that was included in study.
