Abstract
Uvod: S stomatološko protetiko želimo doseči ustrezno oskrbo zob stomatognatega sistema pri čemer želimo zadostiti funkcionalnim in estetskim kriterijem. Zlasti pri kompleksnejših primerih je v proces stomatoprotetične oskrbe smiselno vključiti diagnostični prednaredek. Diagnostični prednaredek v procesu stomatoprotetične obravnave ponazarja načrtovani zobni nadomestek ter tako kot komunikacijsko orodje narekuje potek dela. Namen: Namen diplomskega dela je predstaviti vlogo diagnostičnega prednaredka, način izvedbe v določenem kliničnem stanju in podrobneje predstaviti enega izmed načinov izdelave diagnostičnega prednaredka. Metode dela: Metoda diplomskega dela je deskriptivna metoda in temelji na pregledu literature s področja fiksnoprotetične oskrbe, ki se zaradi izvedbe zobnega nadomestka in predstavitve postopkov prepleta z metodo projekcije. Rezultati: Diagnostični prednaredek kot kratkotrajno nošena, začasna protetična predstavitev končne protetične oskrbe, ki jo izdelamo v začasnem materialu, služi kot komunikacijsko orodje med člani delovnega tima. Diagnostični prednaredek je smiselno vključiti v proces podaljšanja kliničnih kron, direktnega dograjevanja zobnih struktur, pri preparaciji zob nosilcev protetične konstrukcije in pri načrtovanju končnega zobnega nadomestka. Pred pričetkom stomatoprotetične obravnave predhodno izdelamo študijska modela, ki sta namenjena arhiviranju in načrtovanju prvega diagnostičnega navoska. Prvi diagnostični navosek namenjen izdelavi kirurškega vodila pri podaljšanju kliničnih kron določa nov nivo dlesni. Podvojeno stanje diagnostičnega navoska je namenjeno izdelavi folije, ki služi kot kirurško vodilo in izdelavi direktega prednaredka. Modelacija novega navoska ponazarja bodoče stanje, ko se dlesen že preoblikuje. Otežen nadzor nad debelino voska ter cervikalno zaporo premosti kombinacija klasične modelacije z digitalno podprto metodo izdelave indirektnega diagnostičnega prednaredka. Zadnjo fazo stomatoprotetične obravnave predstavlja izdelava kovinsko-porcelanskega mostička, katerega izdelava je vodena s pomočjo izdelanih silikonskih predložkov, ki omogočajo preverjanje končnega dela s prednaredkom. Razprava in sklep: Diagnostični prednaredek je pomemben element stomatoprotetične oskrbe, saj pripomore k optimizaciji protetičnega načrta. Med procesom stomatoprotetične oskrbe ponazarja načrtovani zobni nadomestek in s tem omogoča voden potek dela ter ohranja določeno zasnovo zobnega nadomestka od začetka do konca stomatoprotetične obravnave.

Title:The influences of the diagnostic mock-up on the success of dental restorative treatment : diploma work
Abstract:
Introduction: The aim of the dental restorative treatment is to establish a functional and aesthetic stomatognathic system. Especially in more complex cases, it is advisable to include a diagnostic mock-up in the stomatoprosthetic therapy. A diagnostic mock-up visualizes final work in the process of dental treatment and it dictates the decision-making process. Purpose: The purpose of this thesis is to present the use of a diagnostic mock-up, different mock-ups used in different clinical situations and to demonstrate one of the possible production of a diagnostic mock-up. Methods: The main method is the descriptive method which is based on the review of literature in the field of fixed prosthodontics, which is intertwined with the projection method due to the demonstration of practical work. Results: Diagnostic mock-up is a short-term, temporary prosthetic visualization of the final prosthetic work that we produce in temporary material and it serves as a communication tool among the members of the working team. Diagnostic mock-up is used in the process of crown lengthening, in the intra-oral build-up, in the process of tooth preparation and it serves as visualization tool in different steps of dental restorative treatment. Prior to the start of dental treatment, the preliminary study models are prepared for archiving and designing the first diagnostic wax-up. The first diagnostic wax-up, used for making the surgical guide in the crown lengthening process, determines the new gingival level. The duplicated master cast is used for making foil that works as crown-lengthening and direct mock-up guide. The second wax-up is made to produce a new mock-up and is a set of provisional restorations according to the new gingival levels. The difficulty of controlling wax thickness and cervical closure is overcome by a combination of classical modeling together with a CAD-CAM supported method of making an indirect diagnostic mock-up. The last stage of dental treatment is the production of a porcelain metal bridge guided by silicon index. Discussion and conclusion: Diagnostic mock-up is an essential part of dental restorative treatment as it contributes to the optimization of the prosthetic plan during the process of dental treatment, it serves as a visualization tool and dictates workflow from the beginning to the end of dental treatment. Moreover, it enables step by step control in the process and has a positive impact on the success of dental restorative treatment.
