Abstract
Uvod: Kombinirane hormonske kontracepcijske tabletke so najpogosteje izbrano kontracepcijsko sredstvo. Kot vsako drugo zdravilo imajo tudi kombinirane oralne kontracepcijske tabletke določene prednosti in slabosti. Medicinska sestra je pomemben člen pri učenju ženske o pravilnem jemanju kontracepcije. Zato je pomembno, da žensko uči na podlagi znanstveno podprtih dokazov. Namen: Namen diplomskega dela je na teoretični ravni predstaviti vplive kontracepcijskih tabletk na menstruacijski cikel ter opisati vlogo medicinske sestre. Metode dela: V diplomskem delu smo uporabili deskriptivno metodo dela s sistematičnim pregledom literature. Pri iskanju smo uporabili časovni okvir objav, in sicer od leta 2007 do 2017. Literatura je bila iskana v slovenskem in angleškem jeziku v podatkovnih bazah CINAHL, ScienceDirect, Medline (PubMed) in COBIB.SI. Razprava in zaključek: Kontracepcijske tabletke vsebujejo umetno izdelane različice ženskih hormonov. Prvotno so jih razvili z namenom zdravljenja menstrualnih motenj. Danes se jih uporablja z namenom preprečevanja neželene nosečnosti, zdravljenja bolečih menstruacij, lahko pa se uporabljajo tudi za nadzor nad menstruacijsko krvavitvijo. Kontracepcijske tabletke vsebujejo hormon imenovan progestin, ki ščiti pred rakom maternice, jajčnikov, endometrija, kolona in rektuma, uporaba pa se priporoča tudi pri zdravljenju policističnih jajčnikov. Uporaba kontracepcijskih tabletk reši težave mnogih žensk, ki tožijo o bolečih menstruacijah, menstruacijah z veliko izgubo krvi ali menstruacijah, ki trajajo več dni. Pri ženskah, ki uporabljajo kontracepcijske tabletke, je veliko manjša tudi pojavnost anemij zaradi pomanjkanja železa. Zmanjšana je poraba higienskih pripomočkov kot so vložki in tamponi. Ženske so pri odločanju glede jemanja oralnih kontracepcijskih tabletk pogosto negotove, predvsem zaradi pomislekov glede neželenih učinkov, ki seveda niso izključeni, predvsem pri ženskah s pridruženimi dejavniki tveganja. Doslednost jemanja kontracepcijskih tabletk je pomembna za njihovo učinkovitost, zato je pomembno, da medicinska sestra pouči žensko o pravilnem jemanju kontracepcijskih tabletk, hkrati pa jo pouči tudi, kateri so možni stranski učinki in katere so prednosti pri uporabi kombiniranih oralnih kontraceptivov. Pomembno je, da medicinska sestra upošteva želje vsake ženske in poskrbi, da bo le-ta prejela informacije, ki temeljijo na znanstveno podprtih dokazih. Title:The effect of contraceptive pills on menstruation and the nurse role : diploma theses
Abstract:
Introduction: Combined hormonal contraceptive pills are most widely used contraceptive. As every other drug, combined oral contraceptive pills too, have advantages and disadvantages. Nurse has an important role in educating women about appropriate administration of contraceptive. Therefore evidence based on science is important. Purpose: The purpose of this diploma theses is to present the effects of contraceptive pills on the menstrual cycle in theory, and to describe the role of nurse in the process. Methods: The research was based on a descriptive method, with a systematic literature review. We used the time frame from 2007 to 2017 as a search filter, and literature in Slovenian and English language. We searched the databases of CINAHL, ScienceDirect, Medline, (PubMed) and COBIB.SI. Discussion and Conclusion: Combined hormonal contraceptive pills contain synthetic female hormones. They were first developed to treat menstrual disorders. Today they're used to prevent unintended pregnancy, painful menstrual periods, as well as controling menstrual bleeding. Oral contraceptive pills contain a hormone progestin, that protects from cancer of uterus, ovary, endometriosis and colorectal cancer. It is also recommended for treatment of polycystic ovaries. Use of oral contraceptive pills helps manage problems of many women with painful menstrual period, menstrual period with high blood loss, or periods that last for several days. The incidence of iron deficiency anemia with women using oral contraceptive pills is much lower, as well as consumption of feminine sanitary products, such as cotton sanitary pads and tampons is reduced. Women decision to use oral contraceptive pills is rather uncertain, mainly because of concerns they have regarding the side effects that are not excluded, especially by women with associated risk factors. Consistent oral contraceptive administration has a significant impact on it’s efficiency, that’s why a nurse should educate a woman about the correct administration of the oral therapy, and at the same time about all the side effects and advantages using the combined hormonal contraceptive pills. It's important for nurse to consider every women wish, and provide her with information that's based on evidence. Keywords: oral hormonal contraception, menstruation, nursing care, side effects
