Abstract
Uvod: Kirurška vodila so protetično načrtovani pripomočki, potrebni za kirurško vstavitev zobnih vsadkov. S pomočjo kirurškega vodila se vstavijo zobni vsadki na predhodno načrtovano mesto v čeljustni kosti. Sodobni računalniško vodeni postopki omogočajo lažje, hitrejše in bolj natančno kirurško delo, kar olajša klinične postopke in zmanjša pooperativne zaplete. Računalniško izdelano kirurško vodilo omogoča natančno vstavitev zobnih vsadkov in izdelavo protetičnih konstrukcij. Namen: V nalogi bomo prikazali in strokovno obrazložili postopek načrtovanja in izdelave kirurških vodil za vodeno implantacijo z uporabo sodobne digitalne tehnologije. Namen diplomskega dela je tudi predstaviti interdisciplinarni pristop laboratorijskega zobnega protetika, protetika specialista in kirurga pri načrtovanju in izvedbi implantatno-protetične oskrbe. Postopki načrtovanja in izdelave kirurških vodil bodo prikazani s slikovnim gradivom in natančnim opisom postopkov izdelave. Metode dela: Glavni vir informacij bo sodobna strokovna literatura, kot so članki v strokovnih revijah in knjigah ter učbeniki s področja laboratorijske protetike in oralne kirurgije. Literatura bo iskana v knjižnicah in strokovnih bazah, kot so npr. PubMed, Medline, EBSCOhost itd. in bo vsebovala tako domače kot tuje strokovno delo. Kirurška vodila bodo izdelana v zobnem laboratoriju s sodelovanjem mentorja in maksilofacialnega kirurga in uporabljena pri klinični oskrbi pacientov z zobnimi vsadki. Rezultati: Izdelano je bilo kirurško vodilo, ki omogoča popolno vodeno vstavitev zobnih vsadkov v čeljustno kost. Vodilo je bilo glede na kirurško in protetično shemo načrtovano v računalniškem programu 3Shape Implant Studio ter natisnjeno s pomočjo 3D tiskalnika oz. postopka stereolitografije. Predstavljeni so posamezni postopki načrtovanja in izdelave kirurškega vodila, ki so podprti s slikovnim gradivom. Razprava in sklep: Dobro načrtovanje in sodelovanje celotne ekipe (laboratorijski zobni protetik, zobozdravnik, oralni kirurg) pripomoreta h kvalitetnemu in natančnemu kirurškemu postopku implantacije, ki je osnova za funkcionalno in estetsko protetično rehabilitacijo. Kirurško vodilo, izdelano s pomočjo računalniške tehnologije, omogoča načrtovanje vstavitve zobnih vsadkov na najbolj ustrezen položaj glede na diagnosticirane kostne pogoje s tridimenzionalnim rentgenskim slikanjem in zato pacientu in kirurgu omogoča minimalno invaziven kirurški poseg. Zato protetično načrtovana vstavitev zobnih vsadkov olajša protetično delo in omogoča izdelavo dobre protetične oskrbe.

Title:Computer guided manufacture of surgical guides for dental implant placement : case report
Abstract:
Introduction: Implant surgical guides are prosthetically fabricated devices necessary for the surgical dental implant placement. Surgical guides aid in insertion of dental implants into the pre-planned implant location in the jawbone. Current state of the art computer-guided procedures allows for easier, faster and more precise surgical work, which consequently facilitates clinical procedures and reduces postoperative complications. A computer guided surgical guide enables the exact insertion of dental implants and the fabrication of prosthetic construction. Aim: The diploma thesis demonstrates and thoroughly describes the design and fabrication procedure of surgical guides for guided implant placement using modern digital technology. The purpose of the diploma thesis is to present the interdisciplinary approach of laboratory dental technicians, prosthetic doctor and surgeons in the design and application of implant-prosthetic care. The procedures for the design and manufacture of surgical guides are supported by images and a detailed description of the manufacturing processes. Methods: The main source of information for the thesis has been provided by the recently published Slovene and foreign scientific and professional articles and books, and textbooks from the field of laboratory prosthetics and oral surgery. Literature search was conducted in the libraries and professional databases, e.g. PubMed, Medline, EBSCOhost, etc. Surgical guides were manufactured in a dental laboratory in cooperation with the mentor and maxillofacial surgeon and were used in the clinical care of patients with dental implants. Results: The manufacturing of a surgical guide, which allows for the complete guided insertion of dental implants into the jawbone. Based on the surgical and prosthetic scheme, the guide was designed in the 3Shape Implant Studio computer programme and printed using a stereolithographic 3D print technology. The individual steps in the design and fabrication of a surgical guide are presented and supported by images. Discussion and conclusion: Meticulous planning and coordination of the entire team (laboratory dental prosthetics, dentist, maxillofacial surgeon) contribute to a high quality and precise surgical procedure of implantation, which provides the basis for functional and aesthetic prosthetic rehabilitation. The surgical guide manufactured using computer technology enables the implementation of dental implants in the most appropriate position according to the diagnosed bone conditions with three-dimensional X-ray imaging, thus exposing the patient and surgeon to minimally invasive procedures. Therefore, the prosthetically planned insertion of dental implants facilitates prosthetic work and provides optimal prosthetic care.
