Vpliv dolge glave mišice biceps brachii na kinematiko ramenskega sklepa : pregled literature

Abstract
Uvod: Funkcionalna stabilnost in biomehanika v rami sta rezultat interakcije med statičnimi in dinamičnimi komponentami v sklepu. Statični stabilizatorji so kostna anatomija, negativni intraartikularni pritisk, glenoidni nastavek, sklepna ovojnica in glenohumeralni ligamenti. Dinamični stabilizatorji so kite mišice rotatorne manšete in druge mišice okoli sklepa. Namen: Namen je na podlagi pregleda literature ugotoviti, ali ima dolga glava mišice biceps brachii vpliv na kinematiko ramenskega sklepa. Metode dela: Uporabili smo deskriptivno metodo s pregledom literature. Iskanje literature je bilo omejeno na spletno podatkovno zbirko PubMed in besedila v angleškem jeziku objavljena od leta 2000 do 2016. Rezultati: Vključitvenim merilom je zadostilo trinajst raziskav. Šest raziskav je proučevalo vpliv dolge glave mišice biceps brachii na artrokinematiko v ramenskem sklepu, štiri raziskave so raziskale vpliv na obseg rotacij v sklepu in pet raziskav se je osredotočalo na aktivnost dolge glave mišice biceps brachii in celotnega mišice biceps brachii pri gibih v ramenskem sklepu. Razprava in sklep: Študije na kadavrih kažejo, da dolga glava mišice biceps brachii ob obremenitvi vpliva na translacijo glave nadlahtnice v vse smeri, vendar te raziskave predpostavljajo aktivnost dolge glave mišice biceps brachi in ne upoštevajo nenehnih sprememb v aktivnosti drugih obsklepnih mišic, zato je dejanski vpliv na artrokinematiko in vivo vprašljiv. Obremenjenost dolge glave mišice biceps brachii v abdukciji (več kot 45⁰) omeji tako končno zunanjo kot notranjo rotacijo pri kadavrih. Elektromiografske študije so si nasprotujoče glede aktivnosti dolge glave mišice biceps brachii in celotne mišice biceps brachii pri gibu fleksije v ramenskem sklepu. Pri športih metanja je mišica biceps brachii najaktivnejša v fazi zaustavljanja roke pri izmetu in nekoliko manj v fazi poznega napenjanja. Večja aktivnost v mišici biceps brachii je prisotna pri pacientih za anteriorno nestabilnostjo v rami in pri pacientih z delno raztrganino rotatorne manšete, kjer dolga glava mišice biceps brachii bolj pripomore k depresiji rame kot pri zdravi rami.

Title:The effect of the long head of the biceps brachii muscle on shoulder kinematics : literature review
Abstract:
Background: Functional stability and biomechanics of the shoulder depend on the interaction of both static and dynamic-stabilizing structures. Static stabilizers include the bony anatomy, negative intra-articular pressure, the glenoid labrum, and the glenohumeral ligaments along with the joint capsule. The dynamic stabilizing structures include the rotator cuff muscles and other muscular structures surrounding the shoulder joint. Purpose: Using review of literature in order to understand the role of the long head of the biceps in shoulder kinematics. Methods: We used descriptive method for literature review. The PubMed electronic database was searched from year 2000 to 2016. Results: Thirteen studies were included. Six of them studied the effect of the long head of the biceps on arthrokinematics, for effect on rotational range of motion and five studies researched activity of the long head of the biceps and whole biceps during shoulder movement. Discussion and conclusion: Studies on cadavers show limitation of humeral head translations in all directions after loading the long head of the biceps, however these studies assume activity in muscle of the long head of the biceps and do not assume activity changes in other muscles around the joint. Therefore its actual influence on arthrokinematics in vivo is questionable. Loading the long head of the biceps in abduction (more than 45⁰) limits external and internal rotation in cadavers. Electromyography studies on activity of the long head of the biceps and the whole biceps in shoulder flexion movement are contradicted. Biceps brachii is most active in throwing sports during arm deceleration, slightly less during arm cocking. Patients with anterior shoulder instability and rotator cuff tears have higher activity in biceps. The long head of the biceps helps depress shoulder in rotator cuff tears patients than in healthy people.
