Abstract
Uvod: Dojenje je naravni proces hranjenja dojenčka z mlekom neposredno iz prsi in je najprimernejši vir prehrane za vse novorojenčke in dojenčke. Mati in otrok sta pri dojenju aktivna partnerja in dojenje velja za dobro, če poteka brez težav za mater in otroka. Ena izmed pomembnih težav, ki lahko otežujejo dojenje, so vdrte prsne bradavice. Ploščate ali vdrte bradavice so opredeljene kot neizstopajoče bradavice, ki ležijo pod nivojem areole. Ženske z vdrtimi prsnimi bradavicami se srečujejo s težavami pri pristavljanju dojenčka, kar lahko povzroči nezadostno pridobivanje mleka, frustracijo matere in se konča s predčasnim prenehanjem dojenja. Namen diplomskega dela je s pregledom literature raziskati, s katerimi težavami se pri dojenju srečujejo ženske z vdrtimi prsnimi bradavicami, ter identificirati pripomočke in intervencije, s katerimi lahko medicinske sestre pripomorejo k lažji vzpostavitvi in ohranitvi dojenja pri ženskah z omenjenim stanjem. Metode dela: Uporabljena je bila opisna oziroma deskriptivna metoda dela s pregledom tuje literature. Iskanje literature je potekalo v obdobju od novembra 2023 do januarja 2024 z uporabo spletnega portala Digitalne knjižnice Univerze v Ljubljani (DiKUL), spletnega iskalnika Google učenjak, ter bibliografskih zbirk PubMed in CINAHL Ultimate. Rezultati: Med intervencije in pripomočke, ki so se izkazali za učinkovite, uvrščamo Hoffmanove vaje, izvajanje večdimenzionalnih poporodnih obiskov, zdravstveno vzgojne izobraževalne programe, uporabo »supple cups« in nastavkov za dojenje. Avtorji so ugotovili, da je pomembno tudi zgodnje odkrivanje žensk, ki bi z dojenjem lahko imele težave. S pregledom literature smo ugotovili, da bi z merjenjem dolžine bradavice ali z zgodnjo oceno dojenja po LATCH lestvici lahko identificirali te ženske, ter začeli izvajati korektivne ukrepe, s katerimi bi lahko povečali stopnjo dojenja. Razprava in zaključek: Ženske se pogosto srečujejo s številnimi izzivi, povezanimi z dojenjem, vendar s pravilno podporo s strani zdravstvenega osebja, izvajanjem intervencij in uporabo pripomočkov, ki temeljijo na dokazih podprti zdravstveni negi, lahko dosežejo uspešno dojenje in zadovoljstvo. V prihodnosti bi bilo potrebno izvesti podrobne raziskave o učinkovitosti terapevtskih pristopov ter na podlagi raziskav oblikovati smernice in priporočila, ki bi zdravstvenim delavcem pomagali pri prepoznavanju in učinkovitem reševanju težav, s katerimi se pri dojenju srečujejo ženske z vdrtimi prsnimi bradavicami.

Title:The nurse's role in establishing and maintaining breastfeeding in women with inverted nipples : diploma work
Abstract:
Introduction: Breastfeeding is the natural process of feeding an infant with milk directly from the breast and is the most suitable source of nutrition for all newborns and infants. Mother and child are active partners in breastfeeding and breastfeeding is considered good if it is carried out without problems for mother and baby. One of the significant challenges that can hinder breastfeeding are inverted nipples. Flat or inverted nipples are defined as nipples that do not protrude below the level of the areola. Women with inverted nipples encounter difficulties in latching the baby, which can lead to insufficient milk supply, maternal frustration, and premature cessation of breastfeeding. Purpose: The aim of this study is to investigate the challenges that women with inverted nipples face during breastfeeding, and to identify the tools and interventions that nurses can use to facilitate easier establishment and maintenance of breastfeeding in women with this condition.Methods: A descriptive method of work was used, involving a review of foreign literature. The literature search took place from November 2023 to January 2024 using the University of Ljubljana's Digital Library (DiKUL), Google Scholar, and bibliographic databases PubMed and CINAHL Ultimate. Results: Interventions and tools that have been shown to be effective include Hoffman's exercises, multidimensional postnatal visits, health educational programmes, the use of supple cups and nipple shields. The authors concluded that early identification of women who may have problems with breastfeeding is also important. A review of the literature suggested that by measuring nipple length or by early assessment of breastfeeding using the LATCH scale, these women could be identified and corrective measures could be implemented to increase breastfeeding rates. Discussion and conclusion: Women often face many challenges related to breastfeeding, but with the right support from health care professionals, implementation of interventions and use of evidence-based care tools, they can achieve successful breastfeeding and satisfaction. In the future, detailed research should be conducted on the effectiveness of therapeutic approaches and based on research, guidelines and recommendations should be developed to help health professionals identify and effectively address the breastfeeding problems experienced by women with inverted nipples.
