Abstract
Uvod: Napovedovanje nevroloških izidov po srčnem zastoju ostaja velik izziv v klinični praksi, ki zahteva natančno in občutljivo ocenjevanje. Obsežen napredek na področju diagnostičnih metod zdravstvenim delavcem omogoča boljšo oceno pacientovega stanja. Medicinske sestre so ključne pri spremljanju nevrološkega stanja pacientov po srčnem zastoju, saj njihova hitra prepoznava sprememb močno prispeva k boljšemu izidu zdravljenja. Namen: Namen diplomskega dela je s pomočjo relevantne literature ugotoviti, katere od nevroloških prognostičnih metod so na razpolago ter kako medicinske sestre v enoti intenzivne terapije sodelujejo z drugimi člani zdravstvenega tima pri spremljanju in izboljšanju nevrološkega stanja pacientov po srčnem zastoju. Metode dela: V diplomskem delu smo uporabili opisno metodo dela pregleda literature, ki je v večini temeljila na tuji strokovni literaturi. Iskanje literature je potekalo s pomočjo ključnih besed: »medicinska sestra« IN »enota intenzivne terapije« / »nurse« AND »intensive care unit« / » srčni zastoj« IN »nevrološka ocena« / »cardiac arrest« AND »neurological assessment« / »nevroprognostifikacija po srčnem zastoju« / »nevroprognostification following cardiac arrest«. Rezultati: Ugotovili smo, da infrardeči pupilometer izstopa zaradi svoje natančnosti, hitrosti in objektivnosti, kar predstavlja prednost pri nevrološki oceni pacienta, sploh pri pri zelo majhnih spremembah zenice ter da je SSEP test v povezavi s pupilometrijo kakovosten pripomoček za zgodnje napovedovanje nevrološke prognoze pacienta. EEG in BIS monitoring se vse pogosteje uporabljata pri spremljanju nevrološkega stanja pacienta po srčnem zastoju, saj prikazujeta podrobnejše informacije o možganski funkciji. Izkazalo se je, da je za uspešno delo z nevromonitoringom izredno pomembno stalno izobraževanje medicinskih sester. Razprava in zaključek: Medicinske sestre so pomemben člen za kakovostno nevrološko ocenjevanje pacientov po srčnem zastoju. Pomembno je izobraževanje medicinskih sester, da so v stiku s sodobnimi smernicami zdravstvene nege, kar poveča natančnost dela pri pacientih po srčnem zastoju. Potrebno je nadaljnje raziskovanje na tem področju zdravstvene nege, kar bo doprineslo k prepoznavnosti obravnavane tematike. Glede na ugotovitve pregleda literature na izbrano temo menimo, da bi bilo treba začeti razvijati in raziskovati to področje v Sloveniji in oblikovati enotna ocenjevalna orodja ter pripraviti obrazce za dokumentiranje nevrološke ocene.

Title:Neuroprognostification following cardiac arrest : diploma work
Abstract:
Introduction: Predicting neurological outcomes after cardiac arrest remains a major challenge in clinical practice, requiring accurate and sensitive assessment. Extensive advances in diagnostic methods are enabling healthcare professionals to better assess the patient's condition. Nurses are key in monitoring the neurological status of patients after cardiac arrest, as their rapid recognition of changes contributes significantly to better treatment outcome. Aim: The aim of this thesis is to use relevant literature to identify which neurological prognostic methods are available and how ICU nurses work with other members of the healthcare team to monitor and improve the neurological status of patients after cardiac arrest. Methods: The literature search was conducted using the keywords: »medicinska sestra« IN »enota intenzivne terapije« / »nurse« AND »intensive care unit« / »srčni zastoj« IN »nevrološka ocena« / »cardiac arrest« AND »neurological assessment« / »nevroprognostifikacija po srčnem zastoju« / »nevroprognostification following cardiac arrest«. Results: 12 units of literature were reviewed in detail. The use of an automated pupillometer is a more appropriate and accurate pupil assessment technique than the conventional method of pupil assessment. The SSEP test in conjunction with pupillometry is a good tool for early prediction of the patient's neurological prognosis. The application of EEG and BIS monitoring is increasingly common in monitoring the neurological status of patients after cardiac arrest, as it provides more detailed information on brain function. Continuous education of nurses is important to work successfully with neuromonitoring. Discussion and conclusion: Nurses are an important part of quality neurological assessment of patients after cardiac arrest. It is important to educate nurses to stay up-to-date with current nursing guidelines to increase the accuracy of their work with patients after cardiac arrest. Further research is needed in this area of nursing, which will contribute to the recognition of the field. The findings of the literature review on the selected topic indicate a need for the development and research of this field in Slovenia, as well as the creation of uniform assessment tools and forms for the documentation of neurological assessments.
