Abstract
Uvod: Možganska kap je drugi najpogostejši vzrok smrti na svetu, preživeli pa lahko imajo hude posledice, med njimi tudi težave s hojo. Za ponovno vzpostavitev cikla hoje lahko uporabimo ortoze za gleženj in stopalo ter funkcionalno električno stimulacijo. Namen: Namen diplomskega dela je na osnovi pregleda znanstvene in strokovne literature ugotoviti učinek ortoz za gleženj in stopalo na hojo pacientov v subakutni in kronični fazi po možganski kapi. Metode dela: Iskanje literature je potekalo v podatkovnih zbirkah PubMed, CINAHL in PEDro. Izbrane so bile raziskave, ki so ustrezale sledečim vključitvenim merilom: raziskave, objavljene med leti 2011-2022, randomizirane nadzorovane in klinične raziskave, prosto dostopne raziskave, objavljene v angleškem jeziku in raziskave, ki so vključevale bolnike po možganski kapi. Rezultati: V pregled literature je bilo vključenih šest raziskav. Za raziskovanje so uporabili različne vrste ortoz za gleženj in stopalo. V treh raziskavah so izvajali fizioterapijo v domačem okolju, v treh pa je potekala fizioterapija pod nadzorom fizioterapevta v kliničnem okolju. V vseh raziskavah je prišlo do pomembnih kliničnih razlik, vendar nekatere izmed njih niso bile statistično pomembne. Razprava in zaključek: Ortoza za gleženj in stopalo pripomore k izboljšanju hoje in drže telesa: povečata se dolžina koraka in hitrost hoje. V prihodnje bi bile treba nadaljevati z raziskavami visoke kakovosti z večjim vzorcem, obvezno vključenostjo kontrolne skupine in daljšim trajanjem intervencij. Treba bi bilo tudi raziskati, kateri material je najprimernejši za izdelavo ortoze in kako dolgo bi jo bolniki morali nositi.

Title:Effects of ankle-foot orthoses on patients' gait in subacute and chronic phase after stroke - literature review : diploma work
Abstract:
Introduction: Stroke is the second most common cause of death in the world, and survivors can experience severe consequences, including difficulties with walking. To restore the walking cycle, ankle and foot orthoses, along with functional electrical stimulation, can be utilized. Purpose: The purpose of the thesis, is to determine the effect of ankle-foot orthoses on the walking ability of patients in the subacute and chronic phases after a stroke, based on a review of scientific and professional literature. Methods: A literature search was conducted within the databases PubMed, CINAHL, and PEDro. Studies were selected based on the following inclusion criteria: research published between 2011 and 2022, randomized controlled and clinical trials, open-access studies published in English, and studies including patients post-stroke. Results: Six studies were included in the literature review. Various types of ankle-foot orthoses were used for the research. Three studies implemented physiotherapy in a home environment, while three conducted physiotherapy under the supervision of a physiotherapist in a clinical environment. In all studies, significant clinical differences were observed, although some were not statistically significant. Discussion and conclusion: Ankle-foot orthosis helps to improve walking and body posture, leading to increased stride length and walking speed. In the future, research should continue with larger sample sizes, the mandatory inclusion of a control group, and longer intervention durations. It would also be necessary to explore which material is most suitable for making the orthosis and how long patients should wear it.
