Povezanost empatije z izgorelostjo in profesionalno izpolnitvijo pri fizioterapevtih : magistrsko delo

Abstract
Uvod: Empatija je pri zdravstvenih delavcih zelo pomembna, saj se s pomočjo empatije vzpostavi zaupanje, večje zadovoljstvo ter manjše doživljanje stisk in anksioznosti pri pacientih. Prispeva tudi k pridobivanju pravilnih diagnoz in višji uspešnosti zdravljenja. Izgorelost se pojavi zaradi dlje časa trajajočega stresa v delovnem okolju. Pripelje do slabše komunikacije in nezadovoljstva pacientov. Izgoreli zdravstveni delavci so izčrpani, nestrpni in nezadovoljni, pogosto pa imajo tudi težave v telesnem zdravju. Namen: Namen našega magistrskega dela je bil ugotoviti, kako se dva vidika duševnega zdravja pri delu, tj. izgorelost (izčrpanost in depersonalizacija) in profesionalna izpolnitev, povezujeta s štirimi dimenzijami empatičnega odzivanja (domišljijsko vživljanje, empatična skrb, zavzemanje perspektive in osebna prizadetost) pri fizioterapevtih. Metode dela: V magistrski nalogi smo najprej uporabili deskriptivno metodo raziskovanja. V empiričnem delu smo za zbiranje podatkov uporabili anketo, ki smo jo objavili na spletu. Za pridobivanje podatkov smo uporabili dva predhodno validirana in v slovenščino prevedena vprašalnika, in sicer krajšo obliko Indeksa medosebne odzivnosti in Vprašalnik profesionalne izpolnitve in izgorelosti. Pridobili smo tudi osnovne demografske podatke, kot so spol, starost in delovna doba. Vzorec v raziskavi je bil pridobljen po principu snežne kepe. Sodelovalo je 210 fizioterapevtov, od tega 27 moških in 183 žensk. Rezultati: Z raziskavo smo ugotovili, da se je visoka raven izgorelosti pojavljala pri 53 % sodelujočih fizioterapevtov, 64 % pa jih je poročalo o visoki ravni profesionalne izpolnitve. Ugotovili smo, da so ženske (xŻ = 2,80) poročale o višji ravni osebne prizadetosti kot moški (xŻ = 2,45; d = 0,48), v ostalih vidikih duševnega zdravja pri delu in empatije pa ni bilo statistično značilnih razlik med spoloma. Delovna doba se je šibko negativno povezovala z depersonalizacijo (r = –0,15) in izčrpanostjo (r = –0,15), domišljijskim vživljanjem (r = –0,17), empatično skrbjo (r = –0,15) in osebno prizadetostjo (r = –0,16) ter šibko pozitivno s profesionalno izpolnitvijo (r = 0,15). Izgorelost se je zmerno pozitivno povezovala z osebno prizadetostjo (r = 0,34). Depersonalizacija se je šibko negativno povezovala z empatično skrbjo (r = –0,17) in šibko pozitivno z osebno prizadetostjo (r = 0,27). Profesionalna izpolnitev se je visoko negativno povezovala z izgorelostjo (r = –0,60) in zmerno negativno z izčrpanostjo (r = –0,53) in depersonalizacijo (r = –0,55), šibko negativno pa se je povezovala tudi z osebno prizadetostjo (r = –0,28). Razprava in zaključek: Zaključimo lahko, da sta empatija in izgorelost med seboj povezana procesa. Z duševnim zdravjem pri delu se povezuje zlasti osebna prizadetost, ki se pri fizioterapevtu kaže kot strah, zaskrbljenost in stiska. Delovna doba ima varovalno vlogo v izgorelosti in profesionalni izpolnitvi. Ugotovitve raziskave nakazujejo na potrebo po ukrepih za preprečevanje in zmanjševanje izgorelosti pri slovenskih fizioterapevtih, posledično pa bi ti ukrepi lahko zmanjševali tudi njihovo osebno prizadetost v delovnih situacijah, povezanih z empatijo.

Title:Empathy and its associations with burnout and professional fulfillment among physiotherapists : master thesis
Abstract:
Introduction: Empathy plays an important role for healthcare workers because it enables the development of trusting relationships and leads to greater satisfaction and less distress and anxiety among patients. It also contributes to accurate diagnoses and higher treatment success. Burnout occurs due to prolonged stress in the work environment. It leads to poor communication and patient dissatisfaction. Burned-out healthcare workers are exhausted, impatient, dissatisfied, and often suffer from physical health problems. Purpose: The purpose of this master thesis was to find out how two aspects of mental health at work, i.e., burnout (exhaustion and depersonalization) and professional fulfillment, relate to the four dimensions of empathic responsiveness (fantasy, empathic concern, perspective taking, and personal distress) in Slovenian physical therapists. Methods: In the master thesis, we first used a descriptive research method. In the empirical part, we used an online survey to collect data. To obtain data, we used two previously validated questionnaires translated into Slovenian language – a short form of the Interpersonal Reactivity Index and the Professional Fulfillment Index. We also gathered basic demographic information, such as gender, age, and length of service. A snowball sampling method was used. The participants were 210 physiotherapists, of which 27 were men and 183 were women. Results: Our findings show a high level of burnout in 53% of the participating physiotherapists, while 64% of them reported a high level of professional fulfillment. Women (x̄ = 2,80) reported higher levels of personal distress than men (x̄ = 2,45; d = 0,48), but there were no statistically significant gender differences in the other aspects of mental health at work and empathy. Years of service were weakly negatively associated with depersonalization (r = –0,15) and exhaustion (r = –0,15), fantasy (r = –0,17), empathic concern (r = –0,15), and personal distress (r = –0,16), and weakly positively associated with professional fulfillment (r = 0,15). Burnout was moderately positively associated with personal distress (r = 0,34). Depersonalization was weakly negatively related to empathic concern (r = –0,17) and weakly positively related to personal distress (r = 0,27). Professional fulfillment was highly negatively associated with burnout (r = –0,60), moderately negatively with exhaustion (r = –0,53) and depersonalization (r = –0,55), and weakly negatively associated with personal distress (r = –0,28). Discussion and conclusion: We can conclude that empathy and burnout are interrelated processes. Mental health at work is particularly associated with personal distress, which manifests itself as fear, anxiety, and distress in physiotherapists. Years of service have a protective role in burnout and professional fulfillment. Our research findings point to the need for interventions aimed at preventing and reducing burnout in Slovenian physiotherapists, which could consequently also reduce their distress in work situations involving empathy.
