Optimizacija postopkov za zanesljivo prepoznavo za penicilin občutljivih izolatov bakterije Staphylococcus aureus v medicinskem mikrobiološkem laboratoriju

Abstract
Staphylococcus aureus velja za enega izmed najbolj znanih in razširjenih bakterijskih patogenov. Za zdravljenje okužb, ki jih povzročajo za penicilin občutljivi izolati S. aureus, je najbolj primeren antibiotik penicilin. Za določanje občutljivosti S. aureus za penicilin uporabljamo metodo difuzijskega antibiograma z diski po smernicah EUCAST. Velik premer zaviralne cone okoli diska pa ne izključuje možnosti prisotnega gena blaZ, zato je potrebna vsaj ena potrditvena metoda. V sklopu magistrske naloge smo želeli določiti optimalen in izvedbeno čim bolj preprost postopek za zanesljivo prepoznavo za penicilin občutljivih izolatov S. aureus. Preverili smo uporabnost ocene ostrine robu zaviralne cone ob penicilinskem disku, testa lista deteljice, verižne reakcije s polimerazo za dokaz gena blaZ in masne spektrometrije s testom MBT STAR-BL. 98 % izolatov S. aureus s premerom zaviralne cone, ki kaže na za penicilin občutljiv izolat, je bilo dejansko občutljivih za penicilin. Največjo občutljivost v nalogi je imela metoda ocene ostrine robu zaviralne cone okoli penicilinskega diska, s katero smo pravilno določili vse izolate s prisotnim genom blaZ, vendar smo ugotovili, da smo bili pri ocenjevanju prestrogi, kar je privedlo do slabe specifičnosti metode. Test lista deteljice in dokazovanje odpornosti proti ampicilinu s testom MBT STAR-BL sta bolj specifični metodi, vendar manj občutljivi in zamudnejši v primerjavi z oceno ostrine robu zaviralne cone. Najbolj zanesljiva je uporaba referenčne metode določanja prisotnosti gena blaZ s PCR.

Title:Optimisation of procedures for reliable identification of penicillin-susceptible Staphylococcus aureus isolates in a medical microbiological laboratory
Abstract:
Staphylococcus aureus is one of the most well-known and widespread bacterial pathogens. The preferred antibiotic for treating infections caused by penicillin-susceptible S. aureus isolates is penicillin. To determine the susceptibility of S. aureus to penicillin, EUCAST recommends using the disk diffusion method. However, a large inhibition zone diameter around the disk does not rule out the possibility of the presence of blaZ gene, so at least one confirmation method is necessary. As part of our master's thesis, we wanted to determine an optimal and operationally simple procedure for reliably identifying penicillin-susceptible S. aureus isolates. We evaluated the zone edge test, the clover-leaf test, polymerase chain reaction for detection of blaZ gene and mass spectrometry with the MBT STAR-BL test. 98 % of S. aureus isolates with an inhibition zone diameter indicating to penicillin-susceptible isolates were actually susceptible to penicillin. The zone edge test had the highest sensitivity, correctly identifying all isolates with blaZ gene, but we found that our evaluations were too strict, which resulted in low specificity of the method. The clover-leaf test and detection of ampicillin resistance with MBT STAR-BL test are more specific methods, but less sensitive and time-consuming compared to the zone edge test. The most reliable method is the use of a reference method for determining the presence of blaZ gene with PCR.
